    Endoscopic and immunohistochemical aspects in an inverted papilloma with unilateral total obstruction of the nasal fossa

    An inverted papilloma is a medical condition in which epithelial cells placed on the surface grow in depth into the underlying supporting tissue. The case of a 68-year-old man with a 6-month history of right-sided complete nasal obstruction, who was referred to the ENT Department of our hospital, is presented. The associated symptoms were represented by repeated epistaxis, rhinorrhea and anosmia. From a clinical point of view, a polypous reddish-gray irregular mass, with cauliflower-like appearance, has been described. The formation was friable, richly vascularized, and with minor bleeding on light touch. The patient was diagnosed with inverted papilloma which was treated by endoscopic surgery. By analyzing the clinical, histochemical and immunohistochemical aspects of this case, the cause of tumor growth and its potential for malignant transformation were established. Many particularities are presented in this article: increased regional aggressiveness, its ethmoidal sinus origin, specific symptomatology, paraclinical examinations, all of which were useful in choosing a customized therapy and follow-up program

    Pneumoniile infecţioase sunt produse cel mai frecvent de virusuri şi bacterii, la copil etiologia fi ind dominată de infecţiile virale, care reprezintă până la 90% din toate pneumoniile copilului. Prezenta lucrare este rezultatul unui studiu retrospectiv efectuat pe un lot de 165 de sugari cu pneumonie acută care au fost internaţi în Clinica de Pediatrie a Spitalului Filantropia din Craiova în perioada 1 ianuarie-1 iulie 2010, în care ne-am propus să urmărim frecvenţa şi tendinţa actuală de evoluţie a pneumoniei acute la sugar, precum şi factorii de risc ce duc la instalarea tulburărilor respiratorii. Pneumoniile acute la cazurile studiate s-au vindecat fără sechele, atât formele grave, cât şi cele cu evoluţie prelungită. Formele cu spitalizare de peste 10 zile au necesitat investigaţii suplimentare pentru excuderea unor complicaţii, afecţiuni sau accidente. La aceste cazuri evoluţia prelungită a fost corelată cu particularităţile legate de vârstă şi terenul imunodefi citar (distrofici, rahitici) după ce s-au exclus alte cauze

    CORPII STRĂINI TRAHEOBRONŞICI LA COPIL – ASPECTE DIAGNOSTICE ŞI TERAPEUTICE

    Obiectiv. Lucrarea a avut ca scop studiul aspectelor diagnostice şi terapeutice în suspiciunile de corp străin traheobronşic la copil. Material şi metodă. Studiul retrospectiv a fost efectuat pe 106 cazuri cu suspiciune de corp străin traheobronşic (CSTB), internate şi tratate în Clinica ORL a Spitalului Clinic Judeţean de Urgenţă din Craiova în intervalul dintre anii 2006 şi 2010. Rezultate. Din 77 cazuri (72%) confi rmate în urma bronhoscopiei, 68% au fost înregistrate la băieţi. La grupa de vârstă 1-3 ani au fost întâlnite majoritatea cazurilor (63,63%). Sindromul de penetraţie a fost întâlnit în 88,31%. Cel mai frecvent simptom a fost tusea, într-un procent de 96,10, urmat de dispnee cu 81,81% şi cianoza în 58,44%. Cel mai frecvent semn radiologic întâlnit a fost emfi zemul (27%) urmat de atelectazie (18%). Corpii străini organici predomină în lotul studiat (83,11%) urmaţi de cei anorganici (16,89%). Concluzii. Evoluţia şi prognosticul aspirării de corp străin traheo-bronşic depind de precocitatea diagnosticului şi a intervenţiei terapeutice

    Otitis and middle ear disease present high morbidity rates in spite of current medical treatment, especially in the younger population. Otitis media with effusion (OME) is a chronic disease that is often found in school age children. Its evolution, recurrence, complications and resolution still present a challenge to otologists. For some time, middle ear ventilation tubes were used for this pathology, thus assuring proper middle ear ventilation and higher disease resolution rates. In the current paper, we investigate the connection between ventilation tube usage, the materials out of which they are made, the techniques involved and how these factors may influence disease resolution rates. </jats:p

    Abstract Diffraction techniques and methods with X-rays and less diffraction of electrons, neutrons, etc. are frequently used to investigate the fine structure. X-ray techniques have the advantage that they are cheaper and provide statistical information at the level of fine ordered structure and are not significantly disturbed by the magnetic state of the substance. But the experimental reality complicates matters. The geometric divergence (vertical and horizontal) of the incident beam, the deviation from the ideal monochromatism and the diffraction geometry induce certain corrections for the expression of the integral diffraction intensity. This article addresses the calculation of the integral diffraction intensity affected by these factors for certain methods of X-ray structural analysis.</jats:p

    Influence of Environmental Factors on Rhinosinusal Tumours

    Cancer of the nasal cavity and paranasal sinuses is a rare malignancy. The tumours of the nasal cavity are thus associated with the maxillary and ethmoidal sinuses. Those that start from the nasal vestibule and nasal septum raise particular problems and, although rare, form a particular group of cases among the rhinosinusal tumours. Radiation exposure, viral infections and constitutional factors were associated with the occurrence of rhinosinusal neoplasia also, but the evidence is inconclusive and indicates that only a small proportion of all squamous carcinomas can be attributed. The purpose of this study is the complex retrospective analysis of the cases of rhinosinusal malignancies diagnosed and treated in the Otolaryngology Clinic of the St. Spiridon Emergency Clinical Hospital in Iasi. We evaluated the characteristics of the patient group using a statistically significant analysis of the age, gender, background, and factors of affiliation. Specific tests and indicators have been used. It is not to be neglected the effects of the poor economic conditions andthe lack of education of the population, by postponing the moment of presentation tophysician, influencing the therapeutic decision, postoperative evolution. Thesepatients often present different degrees of malnutrition,immunodepression, etc. Statistical processing showed that thesepatients have a twofold risk of developing rhinosinusal tumours. </jats:p

    Off-Label Uses of Abrocitinib: Review of Emerging Therapeutic Applications beyond Atopic Dermatitis

    Abrocitinib, an oral small-molecule Janus Kinase 1 (JAK1) inhibitor, is primarily approved for treating moderate-to-severe atopic dermatitis (AD) in adults and adolescents aged 12 and older. This review examines the emerging off-label uses of Abrocitinib. We identified 37 papers reporting on the use of Abrocitinib in various conditions other than AD. The most commonly reported uses were for vitiligo, prurigo nodularis, and hand eczema, with 12 cases each. There were also 10 cases of lichen sclerosus and chronic pruritus of unknown origin and 5 cases each of pityriasis rubra pilaris alopecia areata. Additionally, erythematotelangiectatic rosacea and steroid-induced rosacea were reported in four cases each. Other conditions treated with Abrocitinib were noted, but these mostly had only one or two reported cases. Interestingly, out of the 103 patients reviewed, all studies reported favorable clinical outcomes and satisfactory results, with the exception of one isolated case where Abrocitinib was used to treat erythematotelangiectatic rosacea
