    Estimation of atmospheric total organic carbon (TOC) – paving the path towards carbon budget closure

    The atmosphere contains a rich variety of reactive organic compounds, including gaseous volatile organic carbon (VOCs), carbonaceous aerosols, and other organic compounds at varying volatility. Here we present a novel and simple approach to measure atmospheric non-methane total organic carbon (TOC) based on catalytic oxidation of organics in bulk air to carbon dioxide. This method shows little sensitivity towards humidity and near 100 % oxidation efficiencies for all VOCs tested. We estimate a best-case hourly precision of 8 ppb C during times of low ambient variability in carbon dioxide, methane, and carbon monoxide (CO). As proof of concept of this approach, we show measurements of TOC+CO during August–September 2016 from a coastal city in the southwest United Kingdom. TOC+CO was substantially elevated during the day on weekdays (occasionally over 2 ppm C) as a result of local anthropogenic activity. On weekends and holidays, with a mean (standard error) of 102 (8) ppb C, TOC+CO was lower and showed much less diurnal variability. TOC+CO was significantly lower when winds were coming off the Atlantic Ocean than when winds were coming off land if we exclude the weekday daytime. By subtracting the estimated CO from TOC+CO, we constrain the mean (uncertainty) TOC in Atlantic-dominated air masses to be around 23 ( ±≥8 ) ppb C during this period. A proton-transfer-reaction mass spectrometer (PTR-MS) was deployed at the same time, detecting a large range of organic compounds (oxygenated VOCs, biogenic VOCs, aromatics, dimethyl sulfide). The total speciated VOCs from the PTR-MS, denoted here as Sum(VOC), amounted to a mean (uncertainty) of 12 ( ±≤3 ) ppb C in marine air. Possible contributions from a number of known organic compounds present in marine air that were not detected by the PTR-MS are assessed within the context of the TOC budget. Finally, we note that the use of a short, heated sample tube can improve the transmission of organics to the analyzer, while operating our system alternately with and without a particle filter should enable a better separation of semi-volatile and particulate organics from the VOCs within the TOC budget. Future concurrent measurements of TOC, CO, and a more comprehensive range of speciated VOCs would enable a better characterization and understanding of the atmospheric organic carbon budget

    Chordoma: clinical characteristics, management and prognosis of a case series of 25 patients

    <p>Abstract</p> <p>Background</p> <p>Adequate surgery still remains the only curative treatment of chordoma. Interesting clinical data on advanced disease with molecularly targeted therapies were reported.</p> <p>Methods</p> <p>We described the clinical outcome of a series of chordoma patients followed at Regina Elena National Cancer Centre of Rome from 2004 to 2008.</p> <p>Results</p> <p>Twenty-five consecutive patients with sacral (11 patients), spine (13 patients), and skull base (1 patient) chordoma went to our observation. Six patients (24%) had primary disease, 14(56%) a recurrent disease, and 5(20%) a metastatic spreading. Surgery was the primary option for treatment in 22 out of 25 patients. Surgical margins were wide in 5 (23%) and intralesional in 17(77%) patients; 3 out of 4 in-house treated patients obtained wide margins. After first surgery, radiotherapy (protons or high-energy photons) were delivered to 3 patients. One out of the 5 patients with wide margins is still without evidence of disease at 20 months from surgery; 2 patients died without evidence of disease after 3 and 36 months from surgery. Sixteen out of 17 (94%) patients with intralesional margins underwent local progression at a median time of 18 months with a 2-year local progression-free survival of 47%. The 5-year metastasis-free survival rate was 78.3%. Seventeen patients with locally advanced and/or metastatic disease expressing platelet-derived growth factor receptor (PDGFR) β were treated with imatinib mesylate. A RECIST stabilization of the disease was the best response observed in all treated cases. Pain relief with reduction in analgesics use was obtained in 6 out of 11 (54%) symptomatic patients. The 5- and 10-year survival rates of the entire series of patients were 76.7 and 59.7%, respectively.</p> <p>Conclusions</p> <p>Despite progress of surgical techniques and the results obtained with targeted therapy, more effort is needed for better disease control. Specific experience of the multidisciplinar therapeutic team is, however, essential to succeed in improving patients' outcome.</p

    HIV/STD pattern and its associated risk factors among male STD clinic attendees in China: a foci for HIV intervention

    <p>Abstract</p> <p>Background</p> <p>Previous studies suggested a high prevalence of STDs including HIV among female sex workers and men who have sex with men in China, but little was known about the prevalence in male patients attending public STD clinics. The aim of this study was to investigate STD patterns and HIV prevalence among male STD clinic attendees in different areas in China and the associated risk factors. The feasibility of Provider-initiated HIV testing and counseling (PITC) was evaluated as well.</p> <p>Methods</p> <p>A cross-sectional study was conducted at 46 public STD clinics in 4 provinces in China. Between July 2009 and September 2009, a total of 3243 eligible subjects were invited to participate in an interview with a structured-questionnaire for collecting socio-demographic characteristics and sexual behavioral information. They also were asked to provide venous blood samples for serological determinations of HIV and syphilis infection, and first void urine specimens for detecting Chlamydia trachomatis and Neisseria gonorrhoeae infections,</p> <p>Results</p> <p>Out of the 3243 eligible patients, 2951(91%) men agreed to take part in the HIV and syphilis testing. The overall prevalence rate of HIV infection was 0.7% while the rates of syphilis, <it>N. gonorrhoeae, C. trachomatis </it>infections were 10.7%, 4.3% and 6.9%, respectively, with the highest syphilis and <it>N. gonorrhoeae </it>rates in Jiangsu Province. Patients from Guangxi province, homosexual/bisexual practices and intravenous drug use were significantly associated with HIV infection in multivariate logistic regression analyses. Provider-initiated HIV testing and counseling (PITC) was well accepted by attendees, with 91% of eligible attendees agreeing to undergo HIV testing and counseling. All HIV positive patients were properly managed accordingly.</p> <p>Conclusions</p> <p>A modest prevalence of HIV infection and substantial prevalence of other STD infections were found among male patients attending public STD clinics in China. The findings further support the introduction of HIV and syphilis PITC strategy into this important setting.</p

    Recent increase of surface particulate matter concentrations in the Seoul Metropolitan Area, Korea

    Recent changes of surface particulate matter (PM) concentration in the Seoul Metropolitan Area (SMA), South Korea, are puzzling. The long-term trend of surface PM concentration in the SMA declined in the 2000s, but since 2012 its concentrations have tended to incline, which is coincident with frequent severe hazes in South Korea. This increase puts the Korean government&apos;s emission reduction efforts in jeopardy. This study reports that interannual variation of surface PM concentration in South Korea is closely linked with the interannual variations of wind speed. A 12-year (2004-2015) regional air quality simulation was conducted over East Asia (27-km) and over South Korea (9-km) to assess the impact of meteorology under constant anthropogenic emissions. Simulated PM concentrations show a strong negative correlation (i.e. R = -0.86) with regional wind speed, implying that reduced regional ventilation is likely associated with more stagnant conditions that cause severe pollutant episodes in South Korea. We conclude that the current PM concentration trend in South Korea is a combination of long-term decline by emission control efforts and short-term fluctuation of regional wind speed interannual variability. When the meteorology-driven variations are removed, PM concentrations in South Korea have declined continuously even after 2012

    Particles and iodine compounds in coastal Antarctica

    Aerosol particle number concentrations have been measured at Halley and Neumayer on the Antarctic coast, since 2004 and 1984, respectively. Sulphur compounds known to be implicated in particle formation and growth were independently measured: sulphate ions and methane sulphonic acid in filtered aerosol samples and gas phase dimethyl sulphide for limited periods. Iodine oxide, IO, was determined by a satellite sensor from 2003 to 2009 and by different ground‐based sensors at Halley in 2004 and 2007. Previous model results and midlatitude observations show that iodine compounds consistent with the large values of IO observed may be responsible for an increase in number concentrations of small particles. Coastal Antarctica is useful for investigating correlations between particles, sulphur, and iodine compounds, because of their large annual cycles and the source of iodine compounds in sea ice. After smoothing all the measured data by several days, the shapes of the annual cycles in particle concentration at Halley and Neumayer are approximated by linear combinations of the shapes of sulphur compounds and IO but not by sulphur compounds alone. However, there is no short‐term correlation between IO and particle concentration. The apparent correlation by eye after smoothing but not in the short term suggests that iodine compounds and particles are sourced some distance offshore. This suggests that new particles formed from iodine compounds are viable, i.e., they can last long enough to grow to the larger particles that contribute to cloud condensation nuclei, rather than being simply collected by existing particles. If so, there is significant potential for climate feedback near the sea ice zone via the aerosol indirect effect
