    Autoimmune Pancreatitis Accompanied by Cholecystitis, Periaortitis and Pseudotumors of the Liver

    A variety of extrapancreatic lesions have been associated with autoimmune pancreatitis (AIP), and these lesions can be difficult to diagnose. We report a patient referred to Shizuoka Cancer Center with the diagnosis of a possible biliary carcinoma with liver metastasis who was shown to have AIP accompanied by pseudotumors of liver. Clinical imaging revealed diffuse enlargement of the head of the pancreas with irregular narrowing of the main pancreatic duct and inferior common bile duct, multiple liver masses, mediastinal lymphadenopathy, and thickening of the wall of the gallbladder and abdominal aorta. Cytology and biopsy from the pancreaticobiliary tract was negative for malignancy. Serum carcinoembryonic antigen (CEA) and carbohydrate antigen (CA19-9) levels were in the normal range, but soluble interleukin 2 receptor (sIL2R), IgG4 and antinuclear antibody were abnormally high (sIL2R: 2,550 U/ml; IgG4: 764 mg/dl). Corticosteroid therapy was effective and these abnormal findings all improved. This case demonstrates the clinical importance of AIP accompanied by other systemic disorders in the differential diagnosis of patients with a pancreatic mass lesion

    A Radial Sclerosing Lesion Mimicking Breast Cancer on Mammography in a Young Woman

    A spiculated mass on a mammogram is highly suggestive of malignancy. We report the case of a 32-year-old woman with a radial sclerosing lesion that mimicked breast cancer on mammography. She visited her physician after palpating a lump in her left breast. Mammography showed architectural distortion in the upper inner quadrant of the left breast. Ultrasonography showed a low echoic area with an ambiguous boundary. Core needle biopsy was performed because of the suspicion of malignancy. Histological examination did not reveal any malignant cells. After 6 months, the breast lump became larger and the patient was referred to our hospital. Mammography performed in our hospital showed a spiculated mass, and therefore mammotome biopsy was performed. Histological examination revealed dense fibroelastic stroma with a wide variety of mastopathic changes, leading to a diagnosis of a radial sclerosing lesion. One year after the biopsy, the lump on her left breast had disappeared and mammography showed no spiculated mass

    Globalization has made progress in our modern society over recent years. To adapt the globalization, developing richness of spirit and a healthy body are needed in physical education. However now there are many problems that children\u27s physical strength weak and their concerns and desires toward exercise are decreasing. To solve such problems, our college has introduced the new test of physical fitness from 2000.At this report, we compared the new test of physical fitness results of our college students with the national men\u27s averages to know physiques and motor abilities on the basis of 2011 the new test of physical fitness results of our students.As a result, physiques of our college students were more than its national men\u27s average. But in terms of motor abilities, the results were less than its national men\u27s average in particular the muscular endurance, the whole body endurance and the speeding (running ability). It can be said that supports for our school students in the field of promoting their health are one of the most important tasks

    Skeletal Metastasis in Renal Cell Carcinoma: Photon Deficiency on Bone Scintigraphy

    Bone scintigraphy with 99mTc-labeled phosphorous compound is an excellent technique for the detection of bone metastasis. Bone metastases are usually visualized as multiple hot spots on bone scintigraphy. However, photon deficiency in the metastatic lesion on bone scintigrams is reported in few cases with malignancy. In this study, we have evaluated the photopenic bone metastasis from renal cell carcinoma by the radiographic and computed tomographic examinations. Photon deficient metastatic lesions were seen on bone scintigraphy in 5 patients with renal cell carcinoma. All of the photon deficient lesions were osteolytic on bone radiography and computed tomography. The pure photon deficient lesions not associated with surrounding hot margins were seen on the thoracic vertebrae and rib and the ones with surrounding hot margins were seen on the sternum, pelvis and femur. This difference may be due to the differences in the growth rate of the tumor and reactive hyperemia and new bone formation in the metastatic lesions

    Surgical outcome for advanced esophageal cancer patients is not satisfactory in spite of improvements of diagnostic tools, operative techniques, postoperative cares and adjuvant chemotherapy. Postoperative prognosis used to be predicted by the grades of histologic disease progression. However, it is limited to accurate prediction of its prognosis. Recent studies have been focused on biologic behavior of malignant cells, in particular, nuclear DNA contents which play an important role in cell proliferation and metabolizm. Clinical use of flow-cytometric measurement of nuclear DNA is now prevalent to assess the grades of malignancy for malignant tumors. In contrast, it is difficult to know nuclear DNA contents preoperatively. The purpose of this study is to clarify whether or not preoperative biologic behaviors is clinically feasible, from biopsied specimens in comparison with that from the surgical specimens
