This study aimed to examine pre-service Turkish EFL teachers’ conceptualizations of an EFL teacher through metaphors. To this end, a total of 59 senior pre-service EFL teachers, who are majoring in ELT at Gazi University, a state university in Ankara took part in this study. Of these participants who are aged between 20 and 24, 53 were female and 6 were male. The data for the study were collected via a metaphor elicitation task in which the participants were asked to fill in the prompt “An English teacher is like…………. because………..” in Turkish. Before eliciting the metaphors of pre-service teachers, a 15- minute workshop was held to explain what metaphor is and how it is utilized to reflect individuals’ thoughts and actions in daily life. Qualitative research method was used in the present study and content analysis was carried out to analyze the data accordingly. The results of the study showed that senior pre-service EFL teachers conceptualize an EFL teacher mostly through roles that are not peculiar to an EFL teacher, but a teacher in general. Furthermore, the results suggest that senior pre-service teachers tend to adopt traditional teacher roles like provider of knowledge in their conceptualizations of an EFL teacher. Article visualizations

Comorbid Conditions in Child and Adolescent Patients Diagnosed with Attention Deficit/Hyperactivity Disorder

    Get PDF
Attention deficit/hyperactivity disorder (ADHD) is one of the most common childhood psychiatric disorders. The worldwide prevalence of ADHD in children has been reported at 4-7%. Numerous population- and clinical-based studies have reported that more than half of cases of ADHD have at least one psychiatric comorbidity. The presence of psychiatric comorbidities complicates the diagnosis, treatment, and prognosis of ADHD; thus, diagnosis of comorbidities is of great importance. Possible comorbidities should therefore be investigated in cases diagnosed with ADHD before treatment planning

A psoriatic arthritis patient who developed hypertriglyceridemia while receiving adalimumab treatment

    Get PDF
Anti-TNF agents have prominent effect on inflammatory diseases and they probably have various effects on lipid metabolism. Adalimumab (ADA) is the first fully human TNF-alfa antagonist. Our study presents a male patient who developed evident hypertriglyceridemia while receiving ADA treatment. We retrospectively collected data of one psoriatic arthritis patient treated with adalimumab at the University Hospital of Recep Tayyip Erdogan (November 2016). Adalimumab treatment significantly increased triglyceride from 278mg/dl to 4046mg/dl. The influence of adalimumab treatment on lipid profile seems to be proatherogenic, but further investigation is needed to confirm this hypothesis

Pneumoperitoneum is free air existence in abdomen and usually caused by the perforation in gastrointestinal system. Peptic ulcer disease is the most common cause of perforation and pneumoperitoneum. The presence of free air in abdomen usually indicates emergency surgery. However, surgical approach is not required in some cases of pneumoperitoneum if there is no evidence of perforation and no sign of peritoneal irritation. Herein, we present an 82-year–old male patient who had pneumoperitoneum after colonoscopy and treated non-surgically

Abstract

Objective

To describe aspects of the natural history and pathophysiology of coronary arteriovenous fistula and to propose potential treatment strategies.

Methods

Eleven adult patients were treated surgically for coronary arteriovenous fistulas (8 male, 3 female) during the last three years. Mean age was 48,7 ± 9,5 years (range 32-65 years). Diagnosis was made by coronary angiography and transesophageal echocardiography

Results

All patients were symptomatic due to the associating cardiac disorder or fistula. Presenting symptoms were chest pain, exertional dyspnea and palpitation. All patients were diagnosed by selective angiography. Transthoracic and transoesophageal echocardiography was performed to identify the Qp/Qs ratio in one patient. One patient who had an LAD to pulmonary artery coronary arteriovenous fistula with a vascular malformation needed early reoperation due to recurrence of the fistula. Echocardiographic evaluation at the postoperative third month revealed no residual shunts in all patients.

Conclusion

Because of the severe complications that may develop due to coronary arteriovenous fistula, we believe that every coronary artery fistula should be treated invasively by surgery or transcatheter closure. But both treatment modalities still need to be evaluated with randomized multicenter studies for long term survival and effectiveness.</p

Coronary artery bypass grafting is one of the routine daily surgical procedures in the current era. Parallel to the increasing life expectancy, cardiac surgery is commonly performed in octogenarians. However, literature consists of only seldom reports of coronary artery bypass grafting in patients above 90 years of age. In this report, we present our management strategy in a 105-year-old patient who underwent coronary artery bypass grafting at our institution

Objectives In this study covering all of Turkey, we aimed to define cutaneous and systemic adverse reactions in our patient population after COVID-19 vaccination with the Sinovac/CoronaVac (inactivated SARS-CoV-2) and Pfizer/BioNTech (BNT162b2) vaccines. Methods This prospective, cross-sectional study included individuals presenting to the dermatology or emergency outpatient clinics of a total of 19 centers after having been vaccinated with the COVID-19 vaccines. Systemic, local injection site, and non-local cutaneous reactions after vaccination were identified, and their rates were determined. Results Of the 2290 individuals vaccinated between April 15 and July 15, 2021, 2097 (91.6%) received the CoronaVac vaccine and 183 (8%) BioNTech. Systemic reactions were observed at a rate of 31.0% after the first CoronaVac dose, 31.1% after the second CoronaVac dose, 46.4% after the first BioNTech dose, and 46.2% after the second BioNTech dose. Local injection site reactions were detected at a rate of 35.6% after the first CoronaVac dose, 35.7% after the second CoronaVac dose, 86.9% after the first BioNTech dose, and 94.1% after the second BioNTech dose. A total of 133 non-local cutaneous reactions were identified after the CoronaVac vaccine (2.9% after the first dose and 3.5% after the second dose), with the most common being urticaria/angioedema, pityriasis rosea, herpes zoster, and maculopapular rash. After BioNTech, 39 non-local cutaneous reactions were observed to have developed (24.8% after the first dose and 5% after the second dose), and the most common were herpes zoster, delayed large local reaction, pityriasis rosea, and urticaria/angioedema in order of frequency. Existing autoimmune diseases were triggered in 2.1% of the patients vaccinated with CoronaVac and 8.2% of those vaccinated with BioNTech. Conclusions There are no comprehensive data on cutaneous adverse reactions specific to the CoronaVac vaccine. We determined the frequency of adverse reactions from the dermatologist's point of view after CoronaVac and BioNTech vaccination and identified a wide spectrum of non-local cutaneous reactions. Our data show that CoronaVac is associated with less harmful reactions while BioNTech may result in more serious reactions, such as herpes zoster, anaphylaxis, and triggering of autoimmunity. However, most of these reactions were self-limiting or required little therapeutic intervention
