Women's sexual health and contraceptive needs after a severe obstetric complication ("near-miss"): a cohort study in Burkina Faso
BACKGROUND: Little is known about the reproductive health of women who survive obstetric complications in poor countries. Our aim was to determine how severe obstetric complications in Burkina Faso affect reproductive events in the first year postpartum. METHODS: Data were collected from a prospective cohort of women who either experienced life threatening (near-miss) pregnancy-related complications or an uncomplicated childbirth, followed from the end of pregnancy to one year postpartum or post-abortum. Documented outcomes include menses resumption, sexual activity resumption, dyspareunia, uptake of contraceptives, unmet needs for contraception and women's reproductive intentions.Participants were recruited in seven hospitals between December 2004 and March 2005 in six towns in Burkina Faso. RESULTS: Reproductive events were associated with pregnancy outcome. The frequency of contraceptive use was low in all groups and the method used varied according to the presence or not of a live baby. The proportion with unmet need for contraception was high and varied according to the time since end of pregnancy. Desire for another pregnancy was highest among near-miss women with perinatal death or natural abortion. Women in the near-miss group with induced abortion, perinatal death and natural abortion had significantly higher odds of subsequent pregnancy. Unintended pregnancies were observed mainly in women in the near-miss group with live birth and the uncomplicated delivery group. CONCLUSIONS: Considering the potential deleterious impact (on health and socio-economic life) of new pregnancies in near-miss women, it is important to ensure family planning coverage includes those who survive a severe complication

Managing water addition to a degraded core
In this paper we present information that can be used in severe accident management by providing an improved understanding of the effects of water addition to a degraded core. This improved understanding is developed using a diagram showing a sequence of core damage states. Whenever possible, a temperature and a time after accident initiation are estimated for each damage state in the sequence diagram. This diagram can be used to anticipate the evolution of events during an accident. Possible responses of plant instruments are described to identify these damage states and the effects of water addition. The rate and amount of water addition needed (1) to remove energy from the core, (2) to stabilize the core or (3) to not adversely affect the damage progression, are estimated. Analysis of the capability to remove energy from large cohesive and particulate debris beds indicates that these beds may not be stabilized in the core region and they may partially relocate to the lower plenum of the reactor vessel

An implicit steady-state initialization package for the RELAP5 computer code
A direct steady-state initialization (DSSI) method has been developed and implemented in the RELAP5 hydrodynamic analysis program. It provides a means for users to specify a small set of initial conditions which are then propagated through the remainder of the system. The DSSI scheme utilizes the steady-state form of the RELAP5 balance equations for nonequilibrium two-phase flow. It also employs the RELAP5 component models and constitutive model packages for wall-to-phase and interphase momentum and heat exchange. A fully implicit solution of the linearized hydrodynamic equations is implemented. An implicit coupling scheme is used to augment the standard steady-state heat conduction solution for steam generator use. It solves the primary-side tube region energy equations, heat conduction equations, wall heat flux boundary conditions, and overall energy balance equation as a coupled system of equations and improves convergence. The DSSI method for initializing RELAP5 problems to steady-state conditions has been compared with the transient solution scheme using a suite of test problems including; adiabatic single-phase liquid and vapor flow through channels with and without healing and area changes; a heated two-phase test bundle representative of BWR core conditions; and a single-loop PWR model
