    Evaluation of Kidney Dose in Neuroendocrine Tumors Patients after Peptide Receptor Radionuclide Therapy using 177Lu-DOTATATE

    Radiation dose to the kidneys (kidney dose) in 177Lu-DOTATATE - Peptide Receptor Radionuclide Therapy(PRRT) is considered to be the main potential side-effect from the treatment. Prospective assessment of kidney radiation dose can be made with SPECT, however, this requires an intensive imaging regime over a number of days. For this reason, a retrospective investigation of kidney uptake using quantitative SPECT was performed. The aim of the study was to compare the estimated radiation dose to kidneys for each cycle. Seventeen patients treated with 177Lu-DOTATATE for metastatic neuro-endocrine tumors had full imaging for each of their treatment cycles on a Siemens Intevo SPECT/CT gamma camera. One course of treatment consisted of 3 or 4 cycles approximately 8 weeks apart spanning 6 months. SPECT/CT scans of the abdomen were acquired at 3 time points (4, 24 and 96-120 hours) after administration of ~7.8 GBq of 177Lu-DOTATATE. Nine patients received three cycles in total and eight patients had four cycles. Volumes of interest (VOIs) were defined on a CT scan co-registered with the SPECT images and repeated over all time points, to give the radioactivity in the kidneys. Whole organ dosimetry was estimated using OLINDA/EXM using an exponential clearance model. This gives an estimate of radiation absorbed dose to kidneys, in the unit of absorbed dose of organ per administered activity(Gy/GBq) for each treatment cycle. The mean of the 3 or 4 cycles and variation can then be determined. The result shows that the average kidney radiation dose was 0.23 Gy/GBq (range: 0.06 – 0.42) and the average variation between cycles  for all subjects expressed as a percentage was (12.5±7.8) % (median: 11.4 %, range: 1.8 % - 29.4 %). From this study, it can be concluded that the estimated radiation dose to the kidneys for PRRT shows good reproducibility (typically <20 % variation) within an individual across all cycles within one course of treatment  (up to 4 cycles). The errors introduced by assuming that the dosimetry estimate per unit GBq administered from the initial cycle could be used for subsequent cycles within a course are unlikely to contribute significantly to the overall estimate of radiation burden and are considered to be safe

    Theranostic SPECT reconstruction for improved resolution: application to radionuclide therapy dosimetry

    BACKGROUND: SPECT-derived dose estimates in tissues of diameter less than 3× system resolution are subject to significant losses due to the limited spatial resolution of the gamma camera. Incorporating resolution modelling (RM) into the SPECT reconstruction has been proposed as a possible solution; however, the images produced are prone to noise amplification and Gibbs artefacts. We propose a novel approach to SPECT reconstruction in a theranostic setting, which we term SPECTRE (single photon emission computed theranostic reconstruction); using a diagnostic PET image, with its superior resolution, to guide the SPECT reconstruction of the therapeutic equivalent. This report demonstrates a proof in principle of this approach. METHODS: We have employed the hybrid kernelised expectation maximisation (HKEM) algorithm implemented in STIR, with the aim of producing SPECT images with PET-equivalent resolution. We demonstrate its application in both a dual 68Ga/177Lu IEC phantom study and a clinical example using 64Cu/67Cu. RESULTS: SPECTRE is shown to produce images comparable in accuracy and recovery to PET with minimal introduction of artefacts and amplification of noise. CONCLUSION: The SPECTRE approach to image reconstruction shows improved quantitative accuracy with a reduction in noise amplification. SPECTRE shows great promise as a method of improving SPECT radioactivity concentrations, directly leading to more accurate dosimetry estimates in small structures and target lesions. Further investigation and optimisation of the algorithm parameters is needed before this reconstruction method can be utilised in a clinical setting

    Why do people who stutter attend stuttering support groups?

    DATA AVAILABILITY : The data that support the findings of this study are not openly available due to reasons of sensitivity and confidentiality.BACKGROUND : Stuttering support groups (SSGs) have been a long-standing invaluable resource for people who stutter (PWS) but research into SSGs is only emerging. Speech-language therapists (SLTs) need further insight to successfully facilitate SSGs. OBJECTIVES : To determine PWS’ perspectives regarding why they attend SSGs in South Africa. METHOD : Thirteen PWS who attend SSGs, between 20 and 58 years old, were a part of this qualitative study. Purposive sampling was utilised. Semi-structured telephonic interviews were used and data was analysed thematically. RESULTS : Four themes, namely, ‘altered perceptions’, ‘increased sense of community’, ‘support group reciprocity’, and ‘support group environment, participants and topics’, were identified. The results yielded clinical implications which included SLTs encouraging: (1) improved perceptions of being a PWS through education and self-empowerment, (2) PWS’ connections between meetings to increase the sense of community, (3) reciprocity in meetings, (4) sharing personal stories to promote learning and general self-management and (5) support, praise and education to empower and encourage PWS. This study’s findings show that SSGs helped PWS accept their stutter and gain confidence. This study showcased how SSGs can help PWS manage their fluency and gain confidence. Additionally, this study supports current research which suggests that dysfluency and social-emotional well-being should be equally addressed. CONCLUSION : Recommendations were generated from PWS’ perspectives and included focusing discussions on fluency, emotions and sharing personal stories. Insights from PWS helped better inform SLTs of their role within SSGs including guiding and facilitating conversations.     GraphStep: A System Architecture for Sparse-Graph Algorithms

    Many important applications are organized around long-lived, irregular sparse graphs (e.g., data and knowledge bases, CAD optimization, numerical problems, simulations). The graph structures are large, and the applications need regular access to a large, data-dependent portion of the graph for each operation (e.g., the algorithm may need to walk the graph, visiting all nodes, or propagate changes through many nodes in the graph). On conventional microprocessors, the graph structures exceed on-chip cache capacities, making main-memory bandwidth and latency the key performance limiters. To avoid this “memory wall,” we introduce a concurrent system architecture for sparse graph algorithms that places graph nodes in small distributed memories paired with specialized graph processing nodes interconnected by a lightweight network. This gives us a scalable way to map these applications so that they can exploit the high-bandwidth and low-latency capabilities of embedded memories (e.g., FPGA Block RAMs). On typical spreading activation queries on the ConceptNet Knowledge Base, a sample application, this translates into an order of magnitude speedup per FPGA compared to a state-of-the-art Pentium processor

    Depression, anxiety, stress, social interaction and health-related quality of life in men and women with unexplained chest pain

    <p>Abstract</p> <p>Background</p> <p>Unexplained chest pain (UCP) is a common reason for emergency hospital admission and generates considerable health-care costs for society. Even though prior research indicates that psychological problems and impaired quality of life are common among UCP patients, there is lack of knowledge comparing UCP patients with a reference group from the general population. The aim of this study was to analyse differences between men and women with UCP and a reference group in terms of psychosocial factors as depression, anxiety, stress, social interaction and health-related quality of life (HRQOL).</p> <p>Methods</p> <p>A self-administered questionnaire about psychosocial factors was completed by 127 men and 104 women with acute UCP admitted consecutively to the Emergency Department (ED) or as in-patients on a medical ward. A reference group from the general population, 490 men and 579 women, participants in the INTERGENE study and free of clinical heart disease, were selected.</p> <p>Results</p> <p>The UCP patients were more likely to be immigrants, have a sedentary lifestyle, report stress at work and have symptoms of depression and trait-anxiety compared with the reference group. After adjustment for differences in age, smoking, hypertension and diabetes, these factors were still significantly more common among patients with UCP. In a stepwise multivariate model with mutual adjustment for psychosocial factors, being an immigrant was associated with a more than twofold risk in both sexes. Stress at work was associated with an almost fourfold increase in risk among men, whereas there was no independent impact for women. In contrast, depression only emerged as an independent risk factor in women. Trait-anxiety and a low level of social interaction were not independently associated with risk in either men or women. Patients with UCP were two to five times more likely to have low scores for HRQOL.</p> <p>Conclusion</p> <p>Both men and women with UCP had higher depression scores than referents, but an independent association was only found in women. Among men, perceived stress at work emerged as the only psychosocial variable significantly associated with UCP.</p

    Diagnosis and treatment of musculoskeletal chest pain: design of a multi-purpose trial

    <p>Abstract</p> <p>Background</p> <p>Acute chest pain is a major health problem all over the western world. Active approaches are directed towards diagnosis and treatment of potentially life threatening conditions, especially acute coronary syndrome/ischemic heart disease. However, according to the literature, chest pain may also be due to a variety of extra-cardiac disorders including dysfunction of muscles and joints of the chest wall or the cervical and thoracic part of the spine. The diagnostic approaches and treatment options for this group of patients are scarce and formal clinical studies addressing the effect of various treatments are lacking.</p> <p>Methods/Design</p> <p>We present an ongoing trial on the potential usefulness of chiropractic diagnosis and treatment in patients dismissed from an acute chest pain clinic without a diagnosis of acute coronary syndrome. The aims are to determine the proportion of patients in whom chest pain may be of musculoskeletal rather than cardiac origin and to investigate the decision process of a chiropractor in diagnosing these patients; further, to examine whether chiropractic treatment can reduce pain and improve physical function when compared to advice directed towards promoting self-management, and, finally, to estimate the cost-effectiveness of these procedures. This study will include 300 patients discharged from a university hospital acute chest pain clinic without a diagnosis of acute coronary syndrome or any other obvious cardiac or non-cardiac disease. After completion of the clinic's standard cardiovascular diagnostic procedures, trial patients will be examined according to a standardized protocol including a) a self-report questionnaire; b) a semi-structured interview; c) a general health examination; and d) a specific manual examination of the muscles and joints of the neck, thoracic spine, and thorax in order to determine whether the pain is likely to be of musculoskeletal origin. To describe the patients status with regards to ischemic heart disease, and to compare and indirectly validate the musculoskeletal diagnosis, myocardial perfusion scintigraphy is performed in all patients 2–4 weeks following discharge. Descriptive statistics including parametric and non-parametric methods will be applied in order to compare patients with and without musculoskeletal chest pain in relation to their scintigraphic findings. The decision making process of the chiropractor will be elucidated and reconstructed using the CART method. Out of the 300 patients 120 intended patients with suspected musculoskeletal chest pain will be randomized into one of two groups: a) a course of chiropractic treatment (therapy group) of up to ten treatment sessions focusing on high velocity, low amplitude manipulation of the cervical and thoracic spine, mobilisation, and soft tissue techniques. b) Advice promoting self-management and individual instructions focusing on posture and muscle stretch (advice group). Outcome measures are pain, physical function, overall health, self-perceived treatment effect, and cost-effectiveness.</p> <p>Discussion</p> <p>This study may potentially demonstrate that a chiropractor is able to identify a subset of patients suffering from chest pain predominantly of musculoskeletal origin among patients discharged from an acute chest pain clinic with no apparent cardiac condition. Furthermore knowledge about the benefits of manual treatment of patients with musculoskeletal chest pain will inform clinical decision and policy development in relation to clinical practice.</p> <p>Trial registration</p> <p>NCT00462241 and NCT00373828</p
