Антеролатеральная связка коленного сустава как важный стабилизатор ротационной нестабильности коленного сустава

Currently, anterior cruciate ligament grafting is one of the most popular operations on the knee joint, but despite the fact that the technique has been used for years, there is a high percentage of unsatisfactory long-term postoperative results, reaching 25%, according to various authors. The concern over this problem has led to an increase in the number of studies aimed at the understanding of the causes of these failures. Most researchers have concluded that the unsatisfactory results are associated with instability in the knee joint, which is caused by the damage of a ligamentous structure found in many dissections, namely, anterolateral ligament (ALL). In this literature review, we have analyzed the articles dedicated to the ALL, its anatomical and histological structure, its biomechanics, as well as its role in the anterolateral stability of the joint. Also in this review we consider the existing surgical methods for the correction of anterolateral instability associated with the ALL damage.В настоящее время пластика передней крестообразной связки является одной из самых популярных операций на коленном суставе, но, несмотря на отработанную годами технику, отмечается высокий процент неудовлетворительных отдаленных послеоперационных результатов, достигающий, по данным разных авторов, 25%. Озабоченность указанной проблемой привела к увеличению числа исследований, направленных на поиск понимания причин таких неудач. Большинство исследователей пришли к выводу, что неудовлетворительные результаты связаны с нестабильностью в коленном суставе, которая вызвана повреждением обнаруженной на множестве диссекций связочной структуры - антеролатеральной связки. В представленном литературном обзоре проанализированы статьи, касающиеся анатомической и гистологической структуры, биомеханики антеролатеральной связки, а также ее роли в переднелатеральной стабильности сустава. МЕТОД ОДНОМОМЕНТНОГО ВОССТАНОВЛЕНИЯ ПОЛИАКСИАЛЬНОЙ СТАБИЛЬНОСТИ КОЛЕННОГО СУСТАВА У ПАЦИЕНТОВ С СОЧЕТАННЫМ ПОВРЕЖДЕНИЕМ ПЕРЕДНЕЙ КРЕСТООБРАЗНОЙ И АНТЕРОЛАТЕРАЛЬНОЙ СВЯЗОК КОЛЕННОГО СУСТАВА

This research paper gives us information about the clinically tested results of the one-stage technique of anterior cruciate ligament (ACL) and anterolateral ligament (ALL) of the knee. The developed technique was applied in the orthopedic department at State Clinical Hospital of the V.M. Buianov of Moscow in 14 patients (mean age 32 ± 3 years, 8 women and 6 men) with combined single-stage damage to the ACL. The functional state was assessed on the VAS scale: before the operation - 7 points, after 6 months - 8.5, and also on the KOOS scale: before the operation 76 ± 4 points, 6 months after 87 ± 6 points. This operation allows to achieve maximum stability in the knee joint in the course of single-step recovery of all damaged ligamentous structures. An application for the patent of the Russian Federation No. 83 / PM dated January 29, 2017 has been filed for this technique. This research article gives us information about the one-stage technique in order to restore the stability of the joint between anterior cruciate ligament (ACL) and anterolateral ligament (ALL) of the knee. © 2019 Ministry of Health.
