    Right ventricle to pulmonary artery coupling after transcatheter aortic valve implantation—Determinant factors and prognostic impact

    IntroductionRight ventricular (RV) dysfunction and pulmonary hypertension (PH) have been previously associated with unfavorable outcomes in patients with severe aortic stenosis (AS) undergoing transcatheter aortic valve implantation (TAVI), but little is known about the effect of right ventricle (RV) to pulmonary artery (PA) coupling. Our study aimed to evaluate the determinant factors and the prognostic value of RV-PA coupling in patients undergoing TAVI.MethodsOne hundred sixty consecutive patients with severe AS were prospectively enrolled, between September 2018 and May 2020. They underwent a comprehensive echocardiogram before and 30 days after TAVI, including speckle tracking echocardiography (STE) for myocardial deformation analysis of the left ventricle (LV), left atrium (LA), and RV function. Complete data on myocardial deformation was available in 132 patients (76.6 ± 7.5 years, 52.5% men) who formed the final study population. The ratio of RV free wall longitudinal strain (RV-FWLS) to PA systolic pressure (PASP) was used as an estimate of RV-PA coupling. Patients were analyzed according to baseline RV-FWLS/PASP cut-off point, determined through time-dependent ROC curve analysis, as follows: normal RV-PA coupling group (RV-FWLS/PASP ≥0.63, n = 65) and impaired RV-PA coupling group (RV-FWLS/PASP < 0.63, n = 67).ResultsA significant improvement of RV-PA coupling was observed early after TAVI (0.75 ± 0.3 vs. 0.64 ± 0.3 before TAVI, p < 0.001), mainly due to PASP decrease (p < 0.001). LA global longitudinal strain (LA-GLS) is an independent predictor of RV-PA coupling impairment before and after TAVI (OR = 0.837, p < 0.001, OR = 0.848, p < 0.001, respectively), while RV diameter is an independent predictor of persistent RV-PA coupling impairment after TAVI (OR = 1.174, p = 0.002). Impaired RV-PA coupling was associated with a worse survival rate (66.3% vs. 94.9%, p-value < 0.001) and emerged as an independent predictor of mortality (HR = 5.97, CI = 1.44–24.8, p = 0.014) and of the composite endpoint of death and rehospitalization (HR = 4.14, CI = 1.37–12.5, p = 0.012).ConclusionOur results confirm that relief of aortic valve obstruction has beneficial effects on the baseline RV-PA coupling, and they occur early after TAVI. Despite significant improvement in LV, LA, and RV function after TAVI, RV-PA coupling remains impaired in some patients, it is mainly related to persistent pulmonary hypertension and is associated with adverse outcomes

    AbstractWe have dealt with the research from the perspective of the definition given to health by W.H.O.: “health is that certain est ate of wellness from physical, mental and social point of view and it doesn’t represent the lack of illness or disability”. The most important problem which sedentariness produce rises from that view according to which it doesn’t represent only a momentary estate, it becomes a real lifestyle an extremely unhealthy way to live. Physical inactivity affects not only the individuals but also the whole social communities group, and the effects of sedentariness over the world's population mean and imply extremely high costs. The purpose of the paper is to promote the body autoplasty concept through physical exercise by stimulating the psychological resources of the self-consciousness and self image, by making each individual aware of the necessity of practicing physical exercises for body shape in order to improve the quality of his/ her life. The experiment proves that some independent variables of the quality of life are influenced by reaching body autoplasty through physical exercise when its use takes into account the appropriate didactical strategy which is focused on the operational objectives meant to improve the physical state and/or individual body shaping/re-shaping

    Sustainability assessment of waste electric and electronic equipment management systems: Development and validation of the {SUSTWEEE} methodology

    Waste electric and electronic equipment (WEEE) became an important waste stream due to its recycling potential and negative effects on the social, economic and environmental aspects. Currently, the evaluation of WEEE management systems focus on specific parts of their lifecycle (mostly on recycling options), providing information on environmental impacts and/or economic benefits. Therefore, a methodology that would address the entire WEEE lifecycle and the 3 pillars of sustainable development (environmental, economic, social) is necessary. The proposed SUSTWEEE methodology dedicated exclusively to the WEEE stream comprises a 4 level methodology, with indicators to be addressed on background information, WEEE generation, WEEE technical performance and sustainability. The technical performance and sustainability aspects are evaluated on 7 main categories of indicators, both quantitative and qualitative, assessed according to 44 criteria. All the indicators went through a score allocation and a normalization step where a percentage, qualitative score and color coding were assigned, resulting in a WEEE evaluation matrix. In the validation step, the methodology was tested successfully at national level to evaluate the sustainability of WEEE management system in Romania in reference year 2014. The results obtained in this case show a rather mixed performance, with indicators in extreme situations. The main hot spots were the very low collection rates and the non-existent reuse rate, at least in official data, while the other indicators indicated at least medium performances. The best performances are in the area of WEEE collected that undergoes suitable treatment, the existence of WEEE facilities in Romania capable to perform the treatment, high recycling rate and the overall environmental performance indicator
