Rare presentation of metastatic hepatocellular carcinoma in the finger: Case report and review of literature
Hepatocellular carcinoma (HCC) is a primary tumor of the liver that primarily develops in the setting of chronic liver disease. HCC is difficult to diagnose due to it requiring the use of multiple imaging modalities with the goal to detect tumors when they are less than or equal to 2 cm in size to allow all possible treatment options to be used. Herein, we discuss a 69-year-old male with stage IV liver cancer residing in hospice presenting with left middle finger osteomyelitis and extreme pain. Radiology revealed a destructive lesion but pathology provided the diagnosis of metastatic hepatocellular carcinoma in his finger. Even though HCC can be diagnosed on imaging alone, this case highlights the need for biopsy to provide an accurate diagnosis
Midwives’ knowledge and utilization of non-pneumatic anti shock garment in reducing complication of postpartum haemorrhage in selected health care facilities in Bayelsa state Nigeria
Background: Non-pneumatic anti-shock garment (NASG) is a first-aid lower-body pressure device that reverses hypovolaemic shock and decreases obstetric hemorrhage thereby decreasing maternal morbidity and mortality due to post-partum haemorrhage (PPH).This study assessed the knowledge and utilization of non-pneumatic anti shock garment in the management of postpartum hemorrhage among midwives in selected health care facilities in Bayelsa state. Â Methods: This is a descriptive cross-sectional study in which a sample size of 112 nurses were selected using purposive sampling technique. The instrument for data collection was questionnaire and data collected were analyzed using descriptive and inferential statistics. Â Results: The study revealed that majority of the respondents regardless of their educational level had good knowledge of the description, mechanism of action, and uses of NASG. It was also revealed that majority of the respondents do not use NASG in their centers and its application in management of post-partum hemorrhage was not part of the protocols in these centers, which was a clear indication of underutilization of the NASG in the primary health care centers. There was no significant association found between years of working experience of respondents and their utilization of NASG with (X2 = 8.577, df = 2, P = 0.114) as well as between the level of knowledge of midwives and their utilization of the NASG with (X2 = 0.387, df = 1, P = 0.534).Conclusions: It was recommended that non-pneumonic antishock garment should be made available by government and its utilization should be included in all health care centers policy as a management protocol for post-partum hemorrhage.
Microsatellite marker assisted molecular and morpho-physiological genetic diversity assessment in 38 genotypes of sesame (Sesamum indicum L.)
Identification of genetic diversity and their relationships among breeding materials is crucial in crop improvement strategies. In this study, 38 sesame genotypes were characterized for their genetic diversity. The results revealed significant variations among various traits such as plant height, maturity, capsule plant-1 and seeds capsule-1. The number of capsule plant-1 showed significant positive correlation with seeds capsule-1. The highest heritability was found for the numbers of capsules plant-1 (98.67%). The 38 genotypes were separated into six distinct clusters. Comparison within the populations of the cluster IV and those of cluster VI had the highest capsules plant-1, seeds capsule-1 with enormous genetic diversity. For molecular characterization, 7 microsatellite markers and 5 SSR primers with polymorphism were finally chosen for genetic diversity analysis. Altogether, 19 alleles were identified among the 38 genotypes, and the average number of alleles per locus was 3.80. The lowest and the highest numbers of alleles were 3 and 5, respectively. The polymorphism information content (PIC) ranged from 0.3201 to 0.5934 and SI-ssr30 showed to be highest at 0.5934. The UPGMA based clustering depicted a significant variation at molecular level among the sesame genotypes, having a coefficient of similarity between 0.29 and 1.00. The present study confirmed that extensive genetic diversity existed among the sesame genotypes and valuable agronomic traits may result in the development of high yielding genotypes
Influence of Age on Perception of Midwives and Their Performance in Objective Structured Clincial Examination (Osce) in Nigeria
Objective structured clinical examination (OSCE) is a means of assessing clinical competence based on objective testing through direct observation. OSCE, as a method of evaluation, was introduced in 1990 by the Nursing & Midwifery council of Nigeria (N&MCN). The aim of this quantitative study is to ascertain if age has any influence on midwives’ perception of OSCE and their performance in the examination. Ex-post facto design was adopted for this study. That is because the independent variable was studied in retrospect in order to establish possible relationship with the dependent variables. We were concerned with ascertaining and establishing the status quo and facts at the time of the research and used such facts to analyze data, interpret and extrapolate and also draw inferences. Eight institutions of midwifery education in Akwa Ibom and Cross River states of Nigeria were used for this study. 502 out of 532 midwives who graduated from the institutions between 2004 to 2006 participated in the study. Two validated instruments were used for data collection namely: OSCE stake holder’s questionnaire and achievement test on OSCE. The reliability index for perception stood at (r) 0.60 while the index performance stood at (r) 0.66.Data were analyzed using linear regression analysis and one way analysis of variance. Findings show that older midwives had significant higher perception of OSCE than the younger midwives. Conclusion: age has significant influence on midwives’ perception of OSCE but perception has no influence on their performance in OSCE

Consistent Effects of Early Remdesivir on Symptoms and Disease Progression Across At-Risk Outpatient Subgroups: Treatment Effect Heterogeneity in PINETREE Study
INTRODUCTION: In the PINETREE study, early remdesivir treatment reduced risk of coronavirus disease 2019 (COVID-19)-related hospitalizations or all-cause death versus placebo by 87% by day 28 in high-risk, non-hospitalized patients. Here we report results of assessment of heterogeneity of treatment effect (HTE) of early outpatient remdesivir, focusing on time from symptom onset and number of baseline risk factors (RFs). METHODS: PINETREE was a double-blind, placebo-controlled trial of non-hospitalized patients with COVID-19 who were randomized within 7 days of symptom onset and had ≥ 1 RF for disease progression (age ≥ 60 years, obesity [body mass index ≥ 30], or certain coexisting medical conditions). Patients received remdesivir intravenously (200 mg on day 1 and 100 mg on days 2 and 3) or placebo. RESULTS: In this subgroup analysis, HTE of remdesivir by time from symptom onset at treatment initiation and number of baseline RFs was not detected. Treatment with remdesivir reduced COVID-19-related hospitalizations independent of stratification by time from symptom onset to randomization. Of patients enrolled ≤ 5 days from symptom onset, 1/201 (0.5%) receiving remdesivir and 9/194 (4.6%) receiving placebo were hospitalized (hazard ratio [HR] 0.10; 95% confidence interval [CI] 0.01-0.82). Of those enrolled at > 5 days from symptom onset, 1/78 (1.3%) receiving remdesivir and 6/89 (6.7%) receiving placebo were hospitalized (HR 0.19; 95% CI 0.02-1.61). Remdesivir was also effective in reducing COVID-19-related hospitalizations when stratified by number of baseline RFs for severe disease. Of patients with ≤ 2 RFs, 0/159 (0.0%) receiving remdesivir and 4/164 (2.4%) receiving placebo were hospitalized; of those with ≥ 3 RFs, 2/120 (1.7%) receiving remdesivir and 11/119 (9.2%) receiving placebo were hospitalized (HR 0.16; 95% CI 0.04-0.73). CONCLUSIONS: In the outpatient setting, benefit of remdesivir initiated within 7 days of symptoms appeared to be consistent across patients with RFs. Therefore, it may be reasonable to broadly treat patients with remdesivir regardless of comorbidities. TRIAL REGISTRATION: ClinicalTrials.gov number NCT04501952
Influence of Marital Status on Attitude of Midwives towards OSCE and Their Performance in the Examination in Akwa Ibom and Cross River States, Nigeria
This quantitative study investigated the influence of marital status on the midwives’ attitude towards OSCE and how this affects their performance in the examination. Two hypotheses guided the study. HO 1 sort to find out if there exist a significant influence of marital status of midwives on their attitude towards OSCE as well as performance in the examination and HO 11 states: there is no significant joint influence of midwives’ attitude and marital status on their performance in OSCE. Ex-post facto design was used to carry out the study. 502 midwives from 8 institutes of midwifery education in Akwa Ibom & Cross River states were engaged in the study. Two validated instruments, OSCE stake holders’ questionnaire (OSCEQ) and Achievement test on OSCE were used to collect data for the study. Data were analyzed using linear regression analysis of variance (ANOVA). The findings show that marital status significantly influences midwives’ perception of OSCE. However, there is no significant influence of marital status on midwives’ attitude towards OSCE.  The implication of these findings is, it will be useful and helpful to midwife educators in counseling of new midwifery students on OSCE
