    Pathogenesis and treatment of multiple myeloma bone disease

    Multiple myeloma (Plasma cell myeloma), a malignancy of the plasma cells, exhibits tumor expansion preferentially in the bone marrow and the development of bone-destructive lesions. Multiple myeloma is still an incurable disease with changes in the bone marrow microenvironment in favor of the survival and proliferation of multiple myeloma cells and bone destruction. In this review, we described the recent findings on the regulators involved in the development of myeloma bone diseases, and succinctly summarize currently available therapeutic options and the development of novel bone modifying agents for myeloma treatment

In non-human mammals, the indirect, multi-synaptic connections between the bilateral primary motor cortices and the spinal cord also participates in controlling dexterous hand movement. However, it remains unknown how the direct and indirect corticospinal pathways work in concert to control unilateral hand movement with lateralized preference in humans. Here we demonstrated the asymmetric functional organization of the two corticospinal networks, by combining network modelling and simultaneous functional magnetic resonance imaging techniques of the brain and the spinal cord. Moreover, we also found that the degree of the involvement of the two corticospinal networks paralleled lateralization of hand preference. The present results pointed to the functionally lateralized motor nervous system that underlies the behavioral asymmetry of handedness in humans

    Symptomatic cervical disc herniation in teenagers: two case reports

    INTRODUCTION: The development of a symptomatic herniated cervical disc before the age of 20 is extremely rare. Sporadically reported cases of patients with cervical disc herniation under the age of 20 usually have had underlying disease. CASE PRESENTATION: Case 1: A 19-year-old Asian man visited our clinic and presented with progressive pain in his upper left scapula and weakness of the left deltoid and biceps brachii muscles. C5 radiculopathy by soft disc herniation at C4-C5 without calcification was diagnosed. Microsurgical posterior foraminotomy was performed and he recovered completely eight weeks after the surgery. Case 2: A 15-year-old Asian man presented with difficulty in lifting his arm and neck pain on the right side. Neurological examination showed weakness of the right deltoid and biceps brachii muscles. A magnetic resonance imaging scan demonstrated a herniated intervertebral disc in the right C4-C5 foramen. The patient was treated conservatively and put under observation only, and had completely recovered eight weeks after admission. CONCLUSION: Although extremely rare, symptomatic cervical disc herniations may occur even in the younger population under the age of 20 without any trauma or underlying disease. Favorable outcomes can be achieved by conventional treatments for cervical disc herniation

    Volumetric Assessment of the Frontal Sinus in Female Adolescents and Its Relationship with Craniofacial Morphology and Orthodontic Treatment : A Pilot Study

    The present study aimed to evaluate the correlation between frontal sinus morphology and craniofacial morphology, and to investigate the effects of orthodontic treatment on the development of the frontal sinus in female adolescents (mean age: 13.9 ± 1.3 years). In total, 53 patients were recruited and underwent cephalography and computed tomography before and after orthodontic treatment. Of these patients, most had a bilaterally symmetrical fan-shaped frontal sinus without any fusion. The average size and volume of the frontal sinus before orthodontic treatment were 45.8 ± 12.3 mm in breadth, 29.8 ± 7.3 mm in height, 22.7 ± 5.1 mm in depth, and 5151.6 ± 2711.4 mm2 in volume. Sinus volume in patients with skeletal Class III malocclusion tended to be larger than that in patients with skeletal Class I and II malocclusion. Upon comparison with the pretreatment measurements, the sizes and volumes of the frontal sinus were significantly larger following orthodontic treatment, regardless of the skeletal pattern; however, since these changes were small, the increases in the size and volume of the frontal sinus may have been caused by pubertal growth and not orthodontic treatment.     Increased LDL susceptibility to oxidation accelerates future carotid artery atherosclerosis

    <p>Abstract</p> <p>Background</p> <p>We analyzed the causal relationship between LDL susceptibility to oxidation and the development of new carotid artery atherosclerosis over a period of 5 years. We previously described the determinants related to a risk of cardiovascular changes determined in a Japanese population participating in the Niigata Study, which is an ongoing epidemiological investigation of the prevention of cardiovascular diseases.</p> <p>Methods</p> <p>We selected 394 individuals (169 males and 225 females) who underwent a second carotid artery ultrasonographic examination in 2001 - 2002 for the present study. The susceptibility of LDL to oxidation was determined as the photometric absorbance and electrophoretic mobility of samples that had been collected in 1996 - 1997. The measurements were compared with ultrasonographic findings obtained in 2001 - 2002.</p> <p>Results</p> <p>The multivariate-adjusted model showed that age (odds ratio (OR), 1.034; 95% confidence interval (95%CI), 1.010 - 1.059), HbA1c (OR, 1.477; 95%CI, 0.980 - 2.225), and photometric O/N (OR, 2.012; 95%CI, 1.000 - 4.051) were significant variables that could independently predict the risk of new carotid artery atherosclerosis.</p> <p>Conclusion</p> <p>The susceptibility of LDL to oxidation was a significant parameter that could predict new carotid artery atherosclerosis over a 5-year period, and higher susceptibility was associated with a higher incidence of new carotid artery atherosclerosis.</p
