The Use of Plasmapheresis in Treatment of Patients with Infertility, Peritoneal Endometriosis and Nat2 Gene Polymorphism
It is known that 30–40 % of patients with peritoneal endometriosis suffer from infertility. Half of the patients with endometriosis are identified point mutation in NAT2 – gene, which plays an important role in the acetylation of aromatic and heterocyclic amines, in the accumulation of endotoxins, activation of free radical oxidation, impaired microcirculation. These factors involve the use of methods of gemapheresis which have detoxification, the blood rheology corrective and immune corrective effects. The purpose of this study was to evaluate the efficacy of therapeutic plasma exchange in treatment of patients with peritoneal form of endometriosis, infertility and point mutations in the gene NAT2. The study included 140 patients with infertility, peritoneal form of endometriosis and point mutations in the gene NAT2. All patients are performed laparoscopy, coagulation foci of endometriosis. In the following 93 (66.4 %) patients were treated with a the course of therapeutic plasmapheresis using the apparatus «PCS-2» with the removal of 20–25 % the volume of circulating plasma with replacement plasma of crystalloid and colloid solutions. Before treatment were shown the signs of endotoxemia, activation of oxidative stress. After treatment with the use of plasmapheresis was revealed the significant reduction of endogenous intoxication parameters and oxidative stress. Also is noted the increase in the pregnancy rate, both independently and in IVF programs, especially during the first 3 months after treatment. The findings suggest that the efficiency of the proposed comprehensive treatment techniques (laparoscopy and subsequent course of therapeutic plasmapheresis) of patients with peritoneal endometriosis and infertility and with point mutations in the gene NAT2. The use of plasmapheresis is pathogenetically justified in patients of the studied group

Study of the structure formation during compression for selecting multiaxial deformation conditions for an Mg-Ca Alloy
The structure homogeneity of the Mg-0.8% Ca alloy is shown to increase with temperature during uniaxial compression. Plastic deformation localizes in narrow and wide deformation bands at 250-350°C. A small number of regions with recrystallized grains is observed only after deformation at 400°C. A relatively homogeneous recrystallized structure with an average grain size of about 20 μm forms after uniaxial compression at t = 450°

A clinical case of congenital hyperinsulinism, diffuse form, pharmacoresistant course (heterozygous mutation of p. 1361 1363 dup CGG in the GCK gene) in an early child is presented as an example of an orphan severe disease with an extremely unfavorable course and a probability of deterioration of the long-term prognosis. The goal was to highlight the clinical manifestations, course options, and complexity of treatment of this pathology to a wide range of doctors of different specialties in the field of Pediatrics in terms of improving the quality and timeliness of diagnosis, reducing the number of complications with the formation of irreparable consequences. Attention is drawn to the most severe course of hypoglycemic conditions in the early neonatal period, the dependence of the formation of a pronounced neurological deficit on the degree and duration of hypoglycemia, which emphasizes the importance of their timely correction to preserve the quality of life of this contingent of children
Myocardial strain parameters and autonomic balance in various variants of the postinfarction course in patients receiving high-dose atorvastatin therapy
Aim. To study parameters of left ventricular (LV) strain and heart rate variability (HRV) in patients with ST-segment elevation myocardial infarction (STEMI) in various variants of postinfarction receiving high-dose atorvastatin therapy.Material and methods. The study included 96 patients with STEMI. All patients underwent two-dimensional speckle tracking echocardiography and 24-hour electrocardiographic monitoring with HRV analysis. The included individuals were divided into groups without pathological LV remodeling (R (-), n=57) and with pathological post-infarction LV remodeling (R (+), n=39), the criterion for which was an increase in end-diastolic and/or end-systolic volume >20% and >15%, respectively. Further, depending on the achievement of target low-density lipoprotein cholesterol (LDL-C) (1,5 mmol/l and/or a decrease of more than 50%), the following subgroups were identified: 17 (43,6%) people, who reached target LDL-C levels (group 1), 22 (56,4%) patients, who did not reach target LDL-C levels (group 2). Similar subgroups were identified in the R (-) group as follows: 28 (49,1%) people, who reached target LDL-C levels (group 3) and 29 (50,9%) people, who did not reach target LDL-C levels (group 4).Results. Starting from the 12th week of follow-up, worsening of global longitudinal (GLS), circumferential (GCS), and radial (GRS) strain (p<0,01) was registered in group 2. In group 1, worsening of these parameters was registered only by the 48th week (p<0,01). In group 3, an increase in GLS was revealed by the 48th week (p=0,001), GCS and GRS — by the 12th week (p<0,05). In group 4, favorable changes were observed only in GLS and GCS by the 48th week (p<0,01). A significant change in HRV parameters was observed in group 3. By the 48th week, there was an increase in temporal indicators (SDNN, SDNNi, SDANN, RMSSD, pNN50) (p<0,0001) and total spectrum power (p=0,0001) due to an increase in all frequency indicators (p<0,001), while the index of autonomic balance L/H decreased by 33% (p=0,002). In group 4, only SDNN, SDANN and pNN50 (p<0,05), as well as the total spectrum power (p=0,001) increased, but no change in frequencies. According to intergroup analysis in the acute period of STEMI, group 4 patients had higher HRV than in group 3 (p<0,05).Conclusion. High-dose atorvastatin therapy improves LV strain characteristics and autonomic balance during the rehabilitation period after STEMI
Study of the Effect of a Biologically Active Compound Tris(2-hydroxyethyl)ammonium 4-Chlorophenylsulfanylacetate on the Growth of Listeria monocytogenes and Staphylococcus aureus
Background. Development of nutrient media ensuring the maximum growth rate of pathogens of dangerous infectious diseases while preserving their biological properties is extremely important. A promising direction in this area seems to be the use of synthetic microbial growth biostimulants.The aim of the work is to study the possibility of improving nutrient media for the cultivation of Listeria and Staphylococcus using a biologically active compound tris(2-hydroxyethyl)ammonium 4-chlorophenylsulfanylacetate.Materials and methods. The object of the study was experimental nutrient medium for the cultivation of Listeria used for the culturing of the test strain Listeria monocytogenes 766. As a comparison medium, commercial medium Fraser broth to which agar was added at a concentration of 1.5 %, was used. The test strain Staphylococcus aureus ATCC 6538-P (FDA 209-P) was cultivated on meat-peptone agar with 1% glucose. The compound tris(2-hydroxyethyl) ammonium (4-chlorophenyl)sulfanylacetate at a concentration of 10–4 wt. % was studied as a growth stimulator. A nutrient medium without a stimulant served as a control. The specific activity of nutrient media (germination rate, medium sensitivity, growth rate and stability of the main biological properties of microorganisms) was evaluated by the microbiological method.Results. Studies have shown that the addition of a growth stimulator to nutrient media contributes to the growth of colonies (by 10–50 %) and a decrease in the time of their development. When growth stimulator was added to the nutrient medium for the cultivation of Listeria, the initial growth of colonies of the L. monocytogenes 766 test strain after 12 hours of cultivation and growth of colonies of the test strain S. aureus ATCC 6538-P after 6 hours of cultivation on the meat-peptone agar with 1% glucose was observed.Conclusion. Thus, the addition of a growth biostimulator tris(2-hydroxyethyl)ammonium 4-chlorophenylsulfanyl acetate at a concentration of 10–4 wt. % in the nutrient medium accelerates the growth of Listeria and Staphylococcus, allows to reduce the time of issuance of the analysis result in half
