    The conversion kinetics of N(o-carboxyphenyl)naphthalimide has been studied in buffer solutions over a pH range 5-12.6 and temperature range 25-70 °C. The naphthalimide hydrolysis is shown to be a reversible reaction. The mechanism of the alkaline hydrolysis of N(o- carboxyphenyl) phthalimide is proposed. The structure dependence of the imide hydrolytic stability is explained on the basis of the suggested mechanism

    HORMONEMEDIATED REGULATION OF CYTOKINES IN PATIENTS WITH ADENOMYOSIS AND UNSUCCESSFUL IVF ATTEMPTS

    Aim. The study was to evaluate the effect of cytokine dysfunctions on the endometrial implantation potential in patients with adenomyosis and unsuccessful IVF attempts and the possibility of hormone-mediated regulation of the cytokine balance at the pre-gravitational stage. Materials and methods. The study included 90 patients with infertility, an unsuccessful attempt of IVF in the history and diffuse adenomyosis of 1 or 2 degrees. Patients were randomized into two groups: the 1st group consisted of 44 patients (group 1) − no pre-education training was conducted. The second group − 46 patients − pregravid preparation of dienogest (2 mg daily for 3 months before the IVF cycle). Control group (group 3) − 30 healthy fertile women. To assess the implantation potential of the endometrium, initially, and after pre-gravity training, the concentration of LIF, IL1в, IL-6, IL-10, RAIL in cervical mucus was determined.Results. Analysis of the indices in patients with adenomyosis showed a decrease in their implantation potential of the endometrium. It has been revealed that hormone-modulating therapy with dienogest partially neutralizes cytokine regulation defects and leads to a certain increase in the effectiveness of IVF programs.Conclusion. Shows expediency of including dienogest in the pregravide preparation plan for patients with adenomyosis, but for full immunocorrection in pregravidual training in women with adenomyosis 1 and 2 The degree of additional directional immunotropic therapy is probably necessary

    The segmentation cycle assisted reproductive technologies as a tool to improve the effectiveness of overcoming infertility

    Purpose. Evaluate the efficiency of IVFICSI in dependency of the patiences’ age, ovarian reserve and their response to the ovulation stimulation. Materials and methods. Analyzed 726 cycles IVF ICSI and 358 cycles of unfrozen embryo transfer with a patients, undergoing infertility treatment in years 2017-2018. Analysis concerned age of patients, quantity of antral follicle and oocyte-cumulus complexes, evaluated mature oocytes only, quantity and quality of embryos on the third day, fifth day and at the moment of transferring, quantity of embryos, undergone freezing and biopsy, as well as quantity of progressing and regressing pregnancies. Results. Depending on the patient’s response to the stimulation, the efficiency indicators of IVF ICSI varied. For patients older 39 years, the factor risk of cancelling the IVF ICSI program, due to absence of embryo to the 3-5 days of cultivation, was the availability of less than seven antral follicles to the day of protocol start. With a small amount of genetic material, non-selective method is the risk factor, announcing the low efficiency of IVF ICSI cycle up to 21% clinical pregnancy rate. Segmentation of a cycle, for holding cryo transplantation with a result of pre implantable genetic aneuplodia testing (PGT-A), increased the efficiency of program from 33% up to 50% for the high cryo tolerance embryo transfer. Conclusion. Shown, that the contol on the embiyological stage of IVFICSI and the ability of choosing the high-quality embryo for a transfer, reliably increased the percentage of successful IVF ICSI cycles by the frequency of clinical pregnancies from 21% up to 34% (p<0,05) among women aged 35-36 years, with a sub-optimal response to the stimulation. The holding of PGT-A for the selection of embryos in cycles of cryo transfer, was forming the IVF ICSI cycle efficiency uptrend among women aged over 36 years.Цель. Оценить эффективность протоколов ЗКО ИКСИ в зависимости от возраста пациенток, овариального резерва и их ответа на стимуляцию овуляции. Материал и методы. Проведен анализ 726 циклов ЭКО ИКСИ и 358 циклов переноса размороженного эмбриона у пациенток, проходивших лечение бесплодия в 2017-2018 годах. Анализ касался возраста пациенток, количества антральных фолликулов и ооцит-кумулюсных комплексов, оценивались зрелые ооциты. количество и качество эмбрионов на 3 сутки, 5 сутки и на момент переноса, число эмбрионов, подвергшихся заморозке и биопсии. Критериями успешности исхода программы считали количество клинических беременностей, а также количество прогрессирующих и регрессирующих беременностей. Результаты. В зависимости от ответа пациентки на стимуляцию отличались показатели эффективности программы ЗКО ИКСИ. У пациенток старше 39 лет фактором риска отмены программы ЭКО ИКСИ из-за отсутствия эмбриона к 3-5 суткам культивации являлось наличие менее 7 антральных фолликулов на день старта протокола. Неселективный перенос эмбриона при малом количестве генетического материала являлся фактором риска малой эффективности цикла ЭКО ИКСИ (до 21% частота клинической беременности). Сегментация цикла для проведения криопереноса с результатом предимплантационного генетического тестирования на анеуплоидии (ПГТ-А) повышало эффективность программы с 33% до 50% на перенос эмбриона с высокой криотолерантностью. Выводы: Показано, что контроль на эмбриологическом этапе программы ЭКО ИКСИ и возможность выбора высококачественного эмбриона на перенос достоверно увеличивало долю успешных циклов ЭКО ИКСИ по частоте клинических беременностей с 21 % до 34% (р<0,05) у женщин в возрасте 35-36 лет с субоптимальным ответом на стимуляцию. Проведение ПГТ-А для отбора эмбрионов в циклах криопереноса формировало тенденцию к повышению эффективности цикла ЭКО ИКСИ у женщин в возрасте старше 36 лет

    Article is devoted to studying the Significance of orthostatic reactions of ventricular rate (OR VR) in the clinical course of permanent atrial fibrillation (AF) to improve the effectiveness of its control. It was found that among patients with AF there are all 3 types of OR VR as during sinus rhythm, positive OR VR predo-minate over negative and absent. It was shown, that positive OR are favorable to reduce the severity of symptoms of AF, according to European Heart Rhythm Association (EHRA), less favorable – absent, and unfavorable – negative OR VR. It was established, that the control of AF with by beta adrenergic anta-gonists (BAA) is possible in any type of OR VR except qualified positive, combination of BAA and amioda-rone – for BAA inefficiency, and control of AF with amiodarone is preferable in qualified positive and ne-gative OR VR and when there are contraindications to BAA

    Abstract. Cytokines provide important connecting links between immunity, blood clotting, and nonspecific resistance that become altered in local frostbites. The aim of study was to determine the contents of cytokines in blood of patients with local frostbites at various terms following the lesion. Fifty patients, 17 to 50 years old, with frostbites of extremities (grade II to IV) were under observation. Arterial and venous blood, as well as venous blood from damaged and intact extremities were examined. Cytokine concentrations were determined using ELISA technique. It was shown that blood concentrations of TNFα, IL-1β, IL-4, IL-8 and IL-18 reached their maximum at early reactive period of the trauma (a 2.3- to 19-fold increase). IL-1β concentration exceeded appropriate control values during all the periods of local frostbites. IL 18 levels were increased at early reactive period of the trauma. IL-1β and IL-8 concentration at pre-reactive period proved to be higher in arterial blood of damaged extremities, than in effluent venous blood. Both during early and late reactive periods, the levels of pro-inflammatory cytokines in effluent blood from damaged extremities were 1.2- to 8-fold higher than in venous blood from intact extremities, whereas IL-4 levels were 2- to 5-fold lower

    Clinical and immunological aspects of early reproductive losses in patients with adenomyosis

    Aim. To evaluate the clinical and immunological features of the early gestational period in patients with adenomyosis.Materials and methods. The study included 110 patients with first spontaneous pregnancy in 6–9 weeks. 75 pregnant women with a history of grade 1 adenomyosis were divided into two groups: group 1 comprised 43 patients with a threatened miscarriage, group 2 included 32 patients without a threat of pregnancy termination. Group 3 (control) included 35 pregnant women without a burdened history with a physiological pregnancy course. The cytokine profile of peripheral blood and cervical mucus was studied, as well as the level of LL-37 (cathelicidin) antimicrobial peptide.Results. A pro-inflammatory shift of the cytokine balance as well as increased pro-angiogenic activity, determined by the VEGF-R2 / VEGF-A ratio, were observed in pregnant women with grade 1 adenomyosis during a physiologically ongoing pregnancy in the 1st trimester. A threatened miscarriage in the presence of adenomyosis is accompanied by statistically significant changes in the cytokine profile, a decrease in pro-angiogenic activity and an increased level of LL-37 in cervical mucus.Conclusion. Pregnancy with adenomyosis of the 1st degree, in the absence of other risk factors, is complicated by a threatened miscarriage in more than half of cases. The revealed clinical and immunological aspects of early reproductive losses require further in-depth study, since these signs may indicate the presence of chronic endometritis in patients with adenomyosis not determined by non-invasive research methods
