    The Micro-Shear Bond Strength of Various Resinous Restorative Materials to Aged Biodentine

    Introduction: The type of materials and application time of veneering restorations on calcium silicate cements are important factors which influence the interfacial properties. The aim of this study was to measure the micro-shear bond strength of a resin composite (RC) using several adhesive systems and a resin-modified glass ionomer cement (RM-GIC) to different aged Biodentine specimens. Methods and Materials: A total of 15 Biodentine blocks were prepared and assigned to three aging periods: 12 min, one week and one month. Then they were subdivided into five sub-groups to receive cylinders of resinous materials. RC was applied using different adhesive systems: A) no adhesive B) etch and rinse C) two-step self-etch and D) universal adhesive in self-etch mode and E) RM-GIC applied directly over Biodentine. Micro-shear bond strength was measured and the data were analyzed using one-way and two-way ANOVA. The level of significance was set at 0.05. Result: There was significant interaction between Biodentine aging periods and resinous materials (P<0.05). The highest value was obtained in group D bonded to the recently set Biodentine. Increasing the aging period to one week resulted in increased micro-shear bond strength in all groups expect for group D. One-month incubation time led to reduced shear bond strength in group A, C and D. Micro-shear bond strength values of group E increased to the longer aged Biodentine.     Comparative evaluation of the effects of three hydraulic calcium silicate cements on odontoblastic differentiation of human dental pulp stem cells: an in vitro study

    Objective: The study aimed to compare the response of human dental pulp stem cells (hDPSCs) towards three hydraulic calcium silicate cements (HCSCs) by measuring cytotoxicity and expression of dentinogenic genes. Methodology: Dental pulps of five impacted mandibular third molars were extirpated as a source for hDPSCs. Next to culturing, hDPSCs were subjected to fluorescence-activated cell sorting after the third passage to validate stemness of the cells. Human DPSCs were exposed to diluted supernatants of OrthoMTA (OMTA), Biodentine (BD) and Calcium-Enriched Mixture (CEM) at concentrations 10, 25, 50 and 100% at the first, third and fifth day of culture. Then, cells were exposed to 10% concentrations supernatant of HCSCs to determine DSPP and DMP1 gene expression, using a quantitative polymerase-chain reaction. Data were analyzed using one-way and three-way ANOVA, followed by Tukey post hoc statistical tests. Results: Optimal cell proliferation was observed in all groups, regardless of concentration and time-point. HCSC supernatants were non-cytotoxic to hDPSCs at all three time-points, except for 100% Biodentine on day five. On day seven, OMTA group significantly upregulated the expression of DSPP and DMP1 genes. On day 14, expression of DMP1 and DSPP genes were significantly higher in BD and OMTA groups, respectively. Conclusion: Biodentine significantly upregulated DMP1 gene expression over 14 days, whereas CEM was associated with only minimal expression of DSPP and DMP1 . &nbsp

    Isolation and differentiation of adipose-derived stem cells into odontoblast-like cells: a preliminary in vitro study

    Objective The aim of present study was to isolate and differentiate human adipose-derived stem cells (ASCs) into odontoblast-like cells. Materials and Methods In this experimental study, human adipose tissues were taken from the buccal fat pad of three individuals (mean age: 24.6 ± 2.1 years). The tissues were transferred to a laboratory in a sterile culture medium, divided into small pieces and digested by collagenase I (2 mg/mL, 60-90 minutes). ASCs were isolated by passing the cell suspension through cell strainers (70 and 40 µm), followed by incubation at 37ºC and 5% CO2in Dulbecco’s modified eagle medium (DMEM) supplemented with fetal bovine serum (FBS 5%) and penicillin/streptomycin (P/S). After three passages, the ASCs were harvested. Subsequently, flow cytometry and reverse transcriptase polymerase chain reaction (RT-PCR) were used to detect expression levels of NANOG and OCT4 to evaluate stemness. Then, a differentiation medium that included high-glucose DMEM supplemented with 10% FBS, dexamethasone (10 nM), sodium β-glycerophosphate (5 mM) and ascorbic acid (100 µM) was added. The cells were cultivated for four weeks, and the odontogenic medium was changed every two days. Cell differentiation was evaluated with Alizarin red staining and expressions of collagen I (COL1A1), dentin sialophosphoprotein (DSPP) and dentin matrix protein-1 (DMP1). Results The ASCs were effectively and easily isolated. They were negative for CD45 and positive for the CD105 and CD73 markers. The ASCs expressed OCT4 and NANOG. Differentiated cells highly expressed DSPP, COL1A1 and DMP1. Alizarin red staining revealed a positive reaction for calcium deposition. Conclusion ASCs were isolated successfully in high numbers from the buccal fat pad of human volunteers and were differentiated into odontoblast-like cells. These ASCs could be considered a new source of cells for use in regenerative endodontic treatments

    Science map of Cochrane systematic reviews receiving the most altmetric attention score: a network analysis

    The present study aimed to analyze and visualize the science map of Cochrane systematic reviews (CSRs) with high Altmetric attention score (AAS). On 2020-07-29, the altmetric data of the Cochrane Database of Systematic Reviews were obtained from the Altmetric database (Altmetric LLP, London, UK). Bibliometric data of the top 5% AAS of CSRs were extracted from the Web of Science. Keyword co-occurrence, co-authorship and co-citation network analyses were then employed using VOSviewer software. The random forest model was used to rank the importance of the altmetric resource. A total of 11222 CSRs with AAS were found (Total mentions: 305265), with Twitter being the most popular Altmetric resource. Consequently, the top 5% AAS (649 articles, mean AAS: 204.95, 95% confidence level: 18.95, mean citations: 123.68, 95% confidence level: 13.9) were included. Density mapping revealed female, adult and child as the most popular author keywords. According to network visualization, Helen V. Worthington (University of Manchester, Manchester, UK), the University of Oxford and UK had the greatest impact on the network at the author, organization and country levels respectively. AAS were weekly correlated with citations (rs=0.21) although citations were moderately correlated with policy document and blog mentions (rs=0.46 and rs=0.43). Cochrane systematic reviews received high levels of online attention, particularly in the Twittersphere and mostly from the UK. However, CSRs were rarely publicized and discussed using recently developed academic tools, such as F1000 prime, Publons and PubPeer

    Overview of recent advances in pulp/dentine complex tissue engineering

    Background and Aims: Pulp necrosis in immature teeth disrupts root development and makes the teeth susceptible to fracture. Regenerative endodontics is a relatively new modality of treatment where the necrotic pulp is replaced with newly formed healthy tissue which has normal functionality. Many clinical reports have demonstrated the potential of this strategy to induce root maturation and apical closure. However, clinical outcomes are patient-dependent and unpredictable. Developing predictable protocols can be achieved through the interplay of three basic elements of tissue engineering, namely, scaffolds, stem cells, and signaling molecules. Furthermore, the clinical success of this treatment is influenced by both the method of preparing the inner space of the root and the type of biomaterial utilized in the coronal part. In this review, we discuss recent advances in tissue engineering-based strategies for regeneration of the pulp/dentine complex along with their advantages and limitations

    Methodological assessment and overall confidence in the results of systematic reviews with network meta-analyses in endodontics

    Aim The aims of the study were to assess the methodological quality of systematic reviews with network meta-analyses (NMAs) in Endodontics using the ‘A MeaSurement Tool to Assess systematic Reviews’ (AMSTAR 2) tool, and to evaluate the overall confidence in the results of the individual reviews included in the analysis. Methodology Systematic reviews with NMAs within the specialty of Endodontics published in English were identified from the PubMed, EbBSCOhost and SCOPUS databases from inception to July 2021. Two reviewers were involved independently in the selection of the reviews, data extraction, methodological quality assessment and overall confidence rating. Disagreements were resolved by discussion between the reviewers to achieve consensus; if disagreements persisted, a third reviewer made the final decision. The methodological quality of the included NMAs was appraised using the AMSTAR 2 checklist, which contains 16 items. The reviewers scored each item—‘Yes’—when the item was fully addressed, ‘Partial Yes’—when the item was not fully addressed, or ‘No’—when the item was not addressed. The overall confidence in the results of each review was classified as ‘High’, ‘Moderate’, ‘Low’ or ‘Critically low’ based on the criteria reported by the AMSTAR 2 developers. Results Twelve systematic reviews with NMAs were included. All the NMAs adequately reported Item 1 (‘Did the research questions and inclusion criteria for the review include the components of PICO?’), Item 8 (‘Did the review authors describe the included studies in adequate detail?’), Item 9 (‘Did the review authors use a satisfactory technique for assessing the risk of bias (RoB) in individual studies that were included in the review?’) and Item 16 (‘Did the review authors report any potential sources of conflict of interest, including any funding they received for conducting the review?’), whereas only one NMA reported Item 10 adequately (‘Did the review authors report on the sources of funding for the studies included in the review?’). The overall confidence in the results of eight reviews was categorized as ‘Critically low’, one review was ‘Low’, two reviews were ‘Moderate’ and one review was ‘High’. Conclusion The overall confidence in the results for the majority of systematic reviews with NMAs in Endodontics was judged to be ‘Critically low’ as their methodological quality was below the necessary standard. AMSTAR 2 and PRISMA for NMA guidelines are available to guide authors to produce high-quality systematic reviews with NMAs and for editors and peer-reviewers when assessing submissions to journals

    Push-out bond strength of bioceramic materials in a synthetic tissue fluid.

    This study compared the push-out bond strength of EndoSequence Root Repair Material (ERRM) and Bioaggregate (BA), new bioceramic materials, to that of mineral trioxide aggregate (MTA) after incubation in phosphate-buffered saline (PBS), a synthetic tissue fluid, for either 1 week or 2 months.One-hundred and twenty root sections were filled with ProRoot MTA, BA, or ERRM. Each tested material was then randomly divided into two subgroups (n = 20): root sections were immersed in PBS for 1 week or 2 months. The bond strengths were measured using a universal testing machine. After that, the failure modes were examined with stereomicroscopy and scanning electron microscopy (SEM). The push-out data and failure mode categories were analyzed by two-way ANOVA and chi-square tests, respectively.The bond strength of ERRM was significantly higher than that of BA and MTA at both incubation periods. No significant difference was found between the bond strength of MTA and BA at either 1 week or 2 months. Increasing the incubation time to 2 months resulted in a significant increase in bond strength of all the materials. The failure mode was mainly mixed for MTA and BA, but cohesive for ERRM at both incubation periods.ERRM had significantly higher bond strength to root canal walls compared to MTA and BA. Increasing the incubation time significantly improved the bond strength and bioactive reaction products of all materials

    Dental patient-reported outcomes in endodontics - a narrative review

    Recently in oral healthcare settings, the focus of assessing treatment outcomes has shifted from the perspective of the clinician towards that of the patient. Endodontology is a specialty of dentistry concerned with the prevention and treatment of pulp and periapical diseases. Research in endodontology and its associated treatment outcomes have focused mainly on clinician-reported outcomes (CROs) and not patient-reported outcomes (PROs). As a result, there is a need to emphasize the importance and relevance of dPROs to researchers and clinicians. The aim of this review is to present an overview of dPROs and dPROMs within clinical endodontics in an attempt to create a better understanding of the patient experience, highlight the need to place the patient at the center of treatment, enhance patient care and encourage more research into dPROs. The key dPROs following endodontic treatment include pain, tenderness, tooth function, need for further intervention, adverse effects (exacerbation of symptoms, tooth discoloration) and Oral Health–Related Quality of Life. dPROs are important following endodontic treatment because they assist clinicians and patients when they discuss and select the most appropriate management options, help clinicians make decisions on pre-operative assessment, prevention and treatment, and improve the methodology and design of future clinical studies. Clinicians and researchers in endodontology should prioritize patient welfare and undertake routine analyses of dPROs using appropriate and robust measures. Due to the lack of agreement over the reporting and definition of endodontic treatment outcomes, a project to define a "Core Outcome Set for Endodontic Treatment Methods (COSET)" is currently ongoing. In the future, a new and exclusive assessment tool should be developed to reflect the viewpoints of patients receiving endodontic treatment more accurately
