Ten questions about antiangiogenic in hepatocellular carcinoma
La validation du sorafenib dans le carcinome hépatocellulaire
(CHC) avancé a permis un progrÚs majeur dans la prise en
charge des patients atteints de cette maladie et a entraßné un
dĂ©ferlement dâinvestigations portant sur des agents ciblĂ©s dans
cette pathologie. Un concept important est que la tolérance et
lâefficacitĂ© de cet agent reposent sur une sĂ©lection optimale des
patients. Cependant, plusieurs questions pertinentes restent
non rĂ©solues concernant lâutilisation du sorafenib en routine
clinique. En effet, si le sorafenib a permis une amélioration significative
de la survie globale des patients atteints dâun CHC
avancé sur cirrhose Child A comparativement au placebo, ceci
contraste avec de faibles taux de réponse obtenus par cette
molécule selon les critÚres RECIST. Ainsi, de nouveaux paramÚtres
intégrant des modifications de densité intratumorale
pourraient ĂȘtre pris en compte en plus des critĂšres dimensionnels
pour juger de lâefficacitĂ© objective de cette drogue.
Actuellement, les recherches en cours doivent définir la place
des antiangiogéniques en association avec les autres approches
thérapeutiques et identifier des biomarqueurs fiables prédictifs
de réponse.The recent approval of sorafenib for advanced hepatocellular
carcinoma (HCC) has been a breakthrough for patient care and
has led to a flurry of clinical studies in HCC. Undeniably, patient
selection is a crucial issue for treatment tolerance and efficacy.
Routine clinical practice raised several important questions. Even
if sorafenib showed a clear improvement in overall survival,
minimal responses by traditional RECIST criteria were observed.
Beside dimensional measurement, new parameters including
intratumor density are requested for appropriate evaluation.
Finally, future researches integrate antiangiogenic agents in
combination with other approaches and identify reliable predictive
biomarkers of response
Update on novel antiangiogenic compounds
Les antiangiogéniques de 2e génération sont, pour la majorité,
des inhibiteurs de récepteurs tyrosine-kinase membranaires dont
nous retenons, dans cet article, les composés ayant atteint les
phases II ou III randomisées. Leur rÎle consiste, soit à améliorer la
puissance dâinhibition sur des cibles dĂ©jĂ validĂ©es (VEGFR,
PDGFR, KIT), soit Ă Ă©tendre leur spectre dâinhibition envers des
cibles supplémentaires telles que FGFR, RET ou EGFR. En dehors
de leurs propriétés sur le plan biologique, les chances de succÚs
de ces nouveaux composés reposent également sur le choix
judicieux des pathologies auxquelles ils sâadresseront.The second generation of antiangiogenics mainly includes
membrane tyrosine kinase receptor inhibitors. This chapter
summarizes clinical results of compounds that reached phase II
and/or randomized phase III clinical trials. The aims of these
agents are to improve the inhibition on validated targets (VEGFR,
PDGFR, KIT) or to enlarge the spectrum of inhibition toward additional
targets including FGFR, RET or EGFR. Beside intrinsic
biological properties, the selection of appropriate disease appears
critical to ensure future success in the development of those novel
compounds
Specificities and challenges of imaging response in pancreatic neuroendocrine tumors treated with targeted therapies
Les tumeurs neuroendocrines pancrĂ©atiques bien diffĂ©renciĂ©es (pNET) sont richement vascularisĂ©es et ont une imagerie caractĂ©ristique au scanner. Lorsque ces tumeurs sont traitĂ©es par une thĂ©rapie ciblĂ©e, en particulier un antiangiogĂ©nique tel que le sunitinib, ces lĂ©sions prĂ©sentent peu de changement de taille, mais des modifications de la densitĂ© tumorale. Les faibles taux de rĂ©ponse objective (< 10 %) rapportĂ©s dans les 2 Ă©tudes randomisĂ©es ayant permis lâAMM pour le sunitinib et lâĂ©vĂ©rolimus dans les pNET, confirment que les critĂšres RECIST sont insuffisants Ă Ă©valuer ces thĂ©rapies ciblĂ©es, nĂ©cessitant lâexploration de nouveaux critĂšres tels que ceux de Choi, qui intĂšgrent les variations de densitĂ© tumorale.Well-differentiated pancreatic neuroendocrine tumors (pNET) are highly vascularized and display specific imaging patterns on CT-scans. When treated with targeted therapies such as the VEGFR inhibitor sunitinib, target lesions show limited if any variation in dimensions, but modifications in tumor density. The low rates of objective response (< 10%) reported in the 2 randomized trials leading to the approval of sunitinib and everolimus in pNET highlight the limits of RECIST criteria to evaluate the effects of targeted therapies, warranting to explore new endpoints involving variation of tumor density, such as Choi criteria
PI3K/AKT /mTOR signalling pathway
PI3K/AKT/mTOR is a cell signalling pathway that plays a key role
in cellular homeostatis through its role in regulation of apoptosis,
cell growth, cell cycle and angiogenesis. This pathway is often
disregulated in human cancers, as most of its components can
be found mutated, upregulated or deleted. mTOR is an interesting
target for therapeutics as no mutation of that protein has been
described so far.
Rapamycin is an antibiotic that has first been developed for its immunosuppressive
characteristics, as a preventive treatment of graft
rejection. Rapamycin is an inhibitor of mTOR also having antitumoral
properties. Three analogs of rapamycin are being developed : temsirolimus,
everolimus, and deforolimus. Their first applications are in
clear cell renal cancers, but many clinical trials are ongoing in other
types of tumors and their indications should enlarge in the future.La voie PI3K / AKT / mTOR est une voie de signalisation
intracellulaire jouant un rĂŽle-clĂ© dans lâhomĂ©ostasie cellulaire
par sa fonction de rĂ©gulation de lâapoptose, de la croissance
et du cycle cellulaire, ainsi que de lâangiogenĂšse. Elle est
fréquemment dérégulée dans les cellules cancéreuses, la
plupart des protĂ©ines la constituant pouvant ĂȘtre mutĂ©es,
délétées ou surexprimées selon les cas. La protéine mTOR
est une cible thérapeutique particuliÚrement intéressante,
aucune mutation de cette protĂ©ine nâĂ©tant connue Ă lâheure
actuelle.
La rapamycine est un antibiotique, initialement développé pour
ses propriétés immunosuppressives et utilisé dans le traitement
antirejet aprĂšs greffe dâorgane, et qui possĂšde des propriĂ©tĂ©s
antitumorales par son action inhibitrice sur la protéine mTOR.
Différents analogues de la rapamycine sont en cours de développement
dans le traitement antitumoral : le temsirolimus (ToriselÂź),
lâeverolimus, et le deforolimus. Leurs premiĂšres indications sont
dans le carcinome rénal à cellules claires, mais de nombreux
essais sont en cours dans dâautres localisations tumorales et
leurs indications devraient sâĂ©largir dans les prochaines annĂ©es
