    Acute chylous peritonitis due to idiopathic pancreatitis mimicking acute appendicitis

    The acute development and the accumulation of chyle into the peritoneal space is a rare condition and one needs to be able to distinguish it from chylous ascites, which is most commonly chronic and associated with related diseases. It is frequently idiopathic and additionally, clinical and imaging findings are nonspecific. In almost all cases, the diagnosis is made intraoperatively during laparoscopy or laparotomy, most diagnostic procedures being indicated by acute complaints pertaining to the abdomen. A 23-year-old man was admitted to the Emergency Department with abdominal pain, with right iliac fossa tenderness and peritonism. Laparoscopy was performed due to acute abdomen findings. Further exploration revealed chylous effusion with milky-like fluid. The peritoneal lavage and the insertion of drains were subsequent to the careful inspection of the cavity. The biochemical analysis of the peritoneal fluid was an important aid which provided the diagnosis of chylous peritonitis due to acute pancreatitis

Just as it happens in any other field, in the case of professional sports, modern technology has produced important changes. And if we think about everyday life, instead of going up the stairs, you use the elevator, instead of writing on paper, you use your computer or laptop at the office, the phone, you pay with a card when you go to the table. Technology becomes part of our lives once we come into contact with it, so it is normal to see the world of professional sports very interested in new technologies. We already see many of these technologies in tennis, where the ball can be tracked with amazing precision, in football, where we have the replay and detailed analysis of any movement made by the players on the field.     Periodontal status and inflammatory markers in gingival crevicular fluid of patients with periodontitis and colorectal cancer

    Objectives. The present pilot study was carried out to evaluate the periodontal status, the levels of interleukin-6 and tumor necrosis factor alpha in gingival crevicular fluid on hospitalized patients with colorectal cancer. Materials and Methods. The sample of our study was represented by 74 patients with colorectal cancer. They were assigned to one of the two study groups, based on cancer’s stages, as follows: (i) stage III-IV: 51 patients (A-group); (ii) stage I-II: 23 patients (B-group). For each participant periodontal parameters were recorded and gingival crevicular fluid samples were collected and immunologically assessed. Results. Statistically significant differences were found between the two groups, regarding probing pocket depth, clinical attachment loss, bleeding on probing, interleukin-6 and tumor necrosis factor alpha. In A-group, statistically significant correlations were found between the inflammatory markers and probing pocket depth and between their levels and bleeding on probing. In B-group, statistically significant correlation was found between the levels of interleukin-6 and bleeding on probing. Conclusions. Our results suggest an association between the severity of periodontitis, the studied immunologic inflammatory markers and colorectal cancer’s severity

    Correlation between periodontal status and Parkinson's disease; a literature review

    This systematic review aims to explore the relationship between chronic inflammation of periodontal disease and neurodegenerative disorders (especially Parkinson's disease), focusing primarily on pathophysiological, clinical and immunological aspects. An exhaustive search on this topic was performed in several databases (including PubMed, Scopus and Web of Science) selecting articles published between 2006 and 2023. After reviewing the titles, abstracts and protocols of each study, 13 articles were extracted for detailed assessment. The main indicators in the study included clinical signs of gingival inflammation, bleeding on probing (BoP), bone loss (BL), periodontal probing depth (PPD), and clinical attachment loss (CAL). Additionally, levels of inflammatory markers such as epidermal growth factor (EGF), interleukin-8 (IL-8), interleukin-17 (IL-17), interferon γ-induced protein 10 (IP-10), and monocyte chemoattractant protein-1 (MCP-1) were monitored. The investigation also explored the presence of one of the main periodontal pathogens (Porphyromonas gingivalis) in the microbiota of Parkinson's disease patients. In conclusion, the data presented further support the intricated relationship between periodontal health and neurodegenerative processes, including aspects related to changes in clinical periodontal indices, immunological indices, as well as oral hygiene and patient medication

    Current data regarding homeostasis of tissues oxygenation in pathophysiological and therapeutic circumstances

    Oxygen is essential for cellular respiration and energy production. Tissue oxygenation refers to delivering oxygen to cells throughout the body. Microcirculation brings blood through small sanguine vessels to maintain the cells' supply of nutrients and oxygen. Optimal tissue oxygenation and microcirculation are essential for maintaining healthy tissue. Conversely, poor oxygenation can cause tissue damage, impair wound healing, and increase infection risk. Several factors can affect tissue oxygenation and microcirculation, including age, lifestyle factors (such as smoking and stress), and underlying medical conditions (such as diabetes and high blood pressure). To improve tissue oxygenation and microcirculation, individuals can engage in healthy lifestyle habits such as regular exercise, healthy eating, stress management, and avoiding smoking. Tissue oxygenation can also be looked therapeutic, given that topical and cosmetic treatments such as massage, pelotherapy, hydrotherapy, moisturizers, and certain skin care products can help pro-mote healthy microcirculation at the somatic level. In the scientific literature, the focus is on hypoxia rather than tissue oxygenation. This article proposes a paradigm shift and emphasizes the homeostatic importance of microcirculation and tissue oxygenation in pathophysiological and therapeutic circumstances. The systematic review of the data from the last 2 years (2021-2022) and the meta-analysis performed on tissue oxygenation will contribute to the practical approach to the pathology circumscribed to tissue oxygenation

    Periodontitis and Heart Disease: Current Perspectives on the Associative Relationships and Preventive Impact

    Due to the important advancement and the accumulation of new evidence on the periodontitis-cardiovascular disease (CVD) relationship as well as the major medical, economic and social burden caused by both diseases this chapter aims to review existing epidemiological and pathogenetic links related to this topic. Also, this chapter aims to highlight the impact of the periodontitis-CVD relationships on clinical practice and on the preventive approaches targeting to decrease the impact of periodontitis on CVD. Periodontitis is an infectious disease eliciting local and general inflammation, which leads to periodontal destruction and systemic involvement. Several pathways could explain the link between periodontitis and CVD such as bacteraemia, chronic persistent systemic inflammation and oxidative stress. The first step in the treatment of periodontitis addresses the elimination of microbial components, which lead to a decrease in local and systemic inflammation. Periodontal therapy seems to positively impact CVD. Specialists should inform patients with CVD on the negative impact of periodontitis on their systemic status and refer patients to the periodontist for an extensive examination as routine management of CVD. Some possible risks of periodontal therapy should be considered in patients undergoing antithrombotic medication

    Surgical management of retroperitoneal hydatid cysts - results on a group of six patients

    Objectives. Hydatid disease is a potentially lethal parasitic condition caused by the larvae of Echinococcus Granulosus; it is found in humans and more commonly in domestic and wild animals in endemic areas. This pathology is frequently encountered in the liver, the hydatid cyst with retroperitoneal location representing a rarely encountered form of evolution. Materials and Methods. In the present paper, we present a study carried out on 6 patients hospitalized in the Surgery Department of the Craiova County Emergency Clinical Hospital. These patients were diagnosed between 2002-2022 with retroperitoneal hydatid cyst or with multiple cysts (including retroperitoneal), being operated and monitored postoperatively at a distance. Results. The diagnosis was generally established by clinical examination, laboratory investigations and CT imaging, while the MRI evaluation was performed only in two of the cases, in order to provide additional data necessary for the surgeon. Conclusions. The difficulty of surgical interventions was determined by the size and local/multilocular extension of the disease; in the case of one patient, a double approach, abdominal and thigh, was necessary
