    Role and Place of Health Care Managers in Managing Nurse Turnover

    INTRODUCTION: An indisputable fact is that the role of the supervisor in managing the turnover is complex and multifaceted. The health care managers need to combine both leadership and management skills to motivate and prevent nurses from leaving.AIM: The aim of this paper is to examine the relationship between level of satisfaction with supervisors and attitudes towards nurse turnover.MATERIALS AND METHODS: For the purpose of the empirical research on the attitudes to changing the workplace, a direct individual anonymous survey was held during the months of February to April 2014. The opinion of nurses and health care managers from university hospitals and hospitals in the North region was studied. It amounted to a total of 11 specialized wards and clinics, characterized by intensive workflow.RESULTS AND DISCUSSION: The analysis of the empirical research shows that managers who provide an opportunity for autonomy at work, delegate authorities and provide the necessary support in difficult moments retain staff at the workplace. At the same time, the lack of support from the supervisor has a strong impact on their decision to leave. Similar to the current results, authors from similar studies also show that leaders who support and encourage participation not only in routine decisions but also discuss with the staff relevant to the organization management decisions, succeed to retain staff in the organization.CONCLUSION: The statistically significant results from the correlations are indicators that effective management creates a positive working environment, increases satisfaction, and provides an opportunity to attract and retain highly qualified personnel. This outlines the need for a change in the role, responsibilities and functions of today`s leaders

    Factors affecting nurses` intention to emigrate from the country in the context of the satisfaction with employment situation

    Introduction: Healthcare professionals` high turnover in our country is an especially topical issue within modern conditions. The reasons behind this ongoing process are largely due to nurses` emigration. Thus, the main task of healthcare managers is to focus their efforts on identifying the factors affecting the attitude of nurses to emigrate abroad. Materials and methods: For the purpose of an empirical sociological study, in February - April 2014 a direct individual anonymous survey on health professionals` motivation to emigrate abroad was conducted. The opinions of 413 nurses from a university and hospitals for active treatment in Northeastern Bulgaria were included in the survey. A total of 11 types of departments and clinics, characterized by high intensity workflow, were studied.Deliverables: The results of this analysis indicate significant emigration potential among surveyed nurses. Those who are willing to leave the country are highly educated younger nurses, whose point of view is influenced by personal professional experience. The intention to practice the profession abroad is significantly motivated by various aspects of work satisfaction.Discussion: Similar to our results, a number of authors in their studies report data revealing the intention to work abroad of predominantly young professionals seeking higher payment and better perspectives for career development.Conclusion: The results demonstrate significant emigration potential among the nurses participating in the survey. Among the individual factors, those which clearly affect the intention to emigrate are professional experience, age and level of educational degree. Work satisfaction compared to experience and age has stronger influence on nurses` attitudes to seek alternative opportunities to work outside the country

    The management of open and honest communication - a factor influencing nurse turnover

    INTRODUCTION: Effective management of turnover depends both on leadership skills and the competence to manage it. An important prerequisite for efficient operation is open and honest communication between managers and staff. Management support includes important skills for adequate communication approaches - open and loyal relationships. The purpose of this paper is to examine the impact of the management options for available and open communication on the turnover of nurses.MATERIALS AND METHODS: For the purpose of the study an anonymous survey was conducted from Febru­ary to April 2014. The study was on the opinion of nurses and experts from university hospitals and hospitals including 11 types of wards and clinics, characterized by intensive workflow. The data were processed statisti­cally by SRSS v. 20.0 for Windows.RESULTS: Evidence suggests that an important and necessary condition for effective operation is open and honest communication between managers and subordinates. A significant proportion of nurses who would not choose the same hospital said that their leaders are „almost never` or „never` open and honest in their com­munication with them (60.70%). Among the influencers about the choice of hospital is the presence of a loyal and accessible relationship with the managers (53.60%) (p<0.001). Over 60% of respondents who intend to leave within a year, respond with „almost never` (30.40%) and „never` (30.40%) on whether they have seen openness and loyalty in managers.CONCLUSION: The survey data show that for the effective management of turnover an important and neces­sary condition is open and honest communication between managers and subordinates

    „Синдром на извънземната ръка` е феномен, при който едната ръка не е под контрол на ума. Лицето губи контрол върху ръката си. Етиологията включва неврохирургия, тумор, аневризми и рядко инсулт. В повечето случаи класическите признаци за „извънземна ръка` произлизат от увреждане на средния фронтален кортекс, което може да е придружено с увреждането на корпус калозум. Невроизображенията и патологичните изследвания показват, че фронталният лоб и corpus callosum са най-честите анатомични лезии, отговорни за синдрома на извънземната ръка. Няма лек за този синдром. Въпреки това симптомите могат да бъдат намалени и управлявани до известна степен, като се държи извънземната ръка заета, като например й се даде обект, който да държи.Alien hand syndrome is a phenomenon in which one hand is not under the control of the brain. The person loses control of the hand. The etiology includes neurosurgery, tumor, aneurysms, and rarely stroke. In most cases, classic alien-hand signs derive from damage to the medial frontal cortex, accompanying damage to the corpus callosum. Neuroimaging and pathological research show that the frontal lobe (in the frontal variant) and corpus callosum (in the callosal variant) are the most common anatomical lesions responsible for the alien hand syndrome.There is no cure for alien hand syndrome. However, the symptoms can be reduced and managed to some degree by keeping the alien hand occupied and involved in a task, for example by giving it an object to hold in its grasp

В резултат на това нарушение костите стават чупливи и придобиват неправилна форма. Най-често биват засегнати костите на таза, черепа, гръбначния стълб и краката. Клиничната картина варира от безсимптомно протичане до тежки скелетни деформации. По-голямата част от пациентите са асимптомни и често заболяването се открива с рентгенография или компютърна томография (СТ) на костите по друг повод. БП засяга по-често мъжете. При повечето пациенти с активна БП или с риск за фрактури се препоръчва терапия с бисфосфонати. Ако нямате оплаквания в началото, може да не се нуждаете от лечение. As a result of this disorder, bones become brittle and irregular in shape. The bones of the pelvis, the skull, the spine and the legs are most frequently affected. The majority of patients are asymptomatic and often the disease is detected by X-ray or computed tomography (CT) of bones on another occasion. Paget‘s disease (PD) affects men more often. Bisphosphonate therapy is recommended in most patients with active PD or at risk of fracture. If the patient has no complaints at the beginning, he may not need treatment. More advanced cases require medical treatment

The aim of the study is to investigate the opinion of nursing leaders and health care sectors of intensive work and offer practical approaches to limit turnover. The most of managers are convinced that adequate material compensation is the basis on which must be built motivational incentive for staff retention (65.80%).     Тетралогия на Фало е комбинация от 4 аномалии на сърдечно-съдовата система: междукамерен дефект; „яздеща` аорта; обструкция в изходния тракт на дясна камера (клапна, подклапна или комбинирана); компенсаторна деснокамерна хипертрофия. Около 3.5% от всички новородени с вродени сърдечни малформации са с тетралогия на Фало. Най-характерният симптом при това заболяване е цианозата (посиня-ването), обикновено пристъпно, при физическо усилие, хранене, висока температура на околната среда). Диагнозата се поставя на базата на симптомите, ЕКГ, ЕхоКГ, сърдечна катетеризация, по-рядко КАТ или ЯМР. Най-общо оперативните корекции се делят на палиативни и радикални.Tetralogy of Fallot (TOF) is a combination of 4 cardiovascular anomalies: a ventricular septal defect, a hole between the two ventricles; pulmonary stenosis, narrowing of the exit from the right ventricle; right ventricular hypertrophy, enlargement of the right ventricle; an overriding aorta, which allows blood from both ventricles to enter the aorta. About 3.5% of all newborns with congenital heart malformations are with TOF. The most common symptom of this disease is cyanosis, usually with bouts in physical effort, with food intake, in high temperatures.     Awareness of people and priority about the choice of bathing zones at the Black Sea seaside of Varna region

    Introduction: Providing information about the quality of the seawater in bathing zones is in accordance with the European and Bulgarian legislation. Testing and controlling seawater purity is extremely important for preventing illnesses.Aim: The aim of this article is to study the awareness of people about the quality of seawater in the Varna region; to study the factors that define people`s choice of bathing zones and evaluate it according to the classification of European Commission.Results: The development and improvement of European and national standards of treating seawater have significant importance for the constant bathing water quality improvement through the years. There are the following priorities in people`s choice of bathing zones: `good infrastructure`, `easy access` - 48.3% followed by bathing water quality - 43.3% of participants. Better educated people choose more often according to the water quality - 53% of participants with higher education compared to 31.5% of people with secondary education, p<0.05 at χ2 5,616.The information submitted according to European and Bulgarian legislation is not sufficient - 91.7% of the participants need more information about the quality of seawater in a bathing zone they would visit and only 8.3% do not need any. It has been noticed that 27.7%, or 1/4, of participants have pointed out that they would not visit a `poor` quality zone, while in fact they normally do visit such type of zones.Conclusion: The existing standard for bathing water quality monitoring is well organized and absolutely in accordance with international and European trends in the field. The participants in the survey do not know the health risks of bathing in polluted water. Better educated people consider water quality a factor defining their choice of bathing places

    Hemodialysis Patient`s Awareness about Kidney Transplantation

    Kidney transplantation is the most frequent type of transplantation worldwide and within the country. It is the only effective treatment for patients diagnosed with chronic renal failure. Currently there are approximately 3000 patients in Bulgaria on hemodialysis. According to the latest data of the Executive Agency for Transplantation (EAT), 977 persons are waiting for a new kidney. The main reason for the low number of enlisted patients for renal transplantation is mostly due to the lack of information about their options.AIM: The purpose of this study is to explore the views of the 50 patients on hemodialysis, their awareness of transplantation possibilities as a factor for improving the quality of life and increase the level of interest about the procedure.MATERIALS AND METHODS: A total of 50 patients undergoing hemodialysis treatment at the St. Marina University Hospital in Varna have been interviewed using a questionnaire method and an individual anonymous survey. The survey was conducted in July 2016.RESULTS AND CONCLUSIONS: The importance of the problem was confirmed by the fact that almost half of the respondents assessed their awareness about transplantations as low. After recognizing the need to raise their awareness of the problem, significantly higher percentage (83%) of the patients stated their motivation to learn about new developments in the field of transplantation through specialized trainings on the subject. The survey data reflects the views of the respondents who increased their knowledge on organ transplantation and realized the possibilities of improving the quality of their life
