    Role of modelling in improving nutrient efficiency in cropping systems

    The applicability of models in addressing resource management issues in agriculture has been widely promoted by the research community, yet examples of real impacts of such modelling efforts on current farming practices are rare. Nevertheless, simulation models can compliment traditional field experimentation in researching alternative management options. The first objective of this paper is, therefore, to provide four case study examples of where models were used to help research issues relating to improved nutrient efficiency in low-input cropping systems. The first two cases addressed strategies of augmenting traditional farming practices with small applications of chemical fertilizer (N and P). The latter two cases explicitly addressed the question of what plant genetic traits can be beneficial in low-nutrient farming systems. In each of these case studies, the APSIM (Agricultural Production Systems Simulator) systems model was used to simulate the impacts of alternative crop management systems. The question of whether simulation models can assist the research community in contributing to purposeful change in farming practice is also addressed. Recent experiences in Australia are reported where simulation models have contributed to practice change by farmers. Finally, current initiatives aimed at testing whether models can also contribute to improving the nutrient efficiency of smallholder farmers in the SAT are discussed

    Evaluation of water productivity, stover feed quality and farmers’ preferences on sweet sorghum cultivar types in the semi-arid regions of Zimbabwe

    Twenty sweet sorghum cultivars that included 17 improved cultivars (experimental grain, forage, dual and India released varieties) from India and 3 landraces from southern Africa were evaluated for their use as an alternative food and fodder crop for crop-livestock farmers. The trials were conducted during 2007/08 season in semi-arid conditions at Matopos Research Station, Zimbabwe. Three methods of assessment were applied to help identify suitable cultivars: grain and stover water productivity (WP), stover feed quality traits and farmers’ assessment of cultivars in the field. Grain and stover WP ranged from 0.6 to 2.7 kg m-3 and 1.2 to 4.0 kg m-3 respectively. We observed significant differences in cultivar groups on plant height, time to maturity, harvest index, grain WP, nitrogen uptake, nitrogen harvest index and stover metabolizable energy and digestibility (P <0.001), and sugar (Brix %) and stover WP (P <0.05). In the improved grain and dual type cultivars, grain yield increased by 118% compared to landraces and by 69% over the forage type while in the India released variety type cultivars grain yield increased by 86% compared to landrace yields and by 44% over the forage cultivars with an increase in stover yield. The landrace type was superior to all sweet sorghum types on feed quality traits (metabolizable energy and digestibility). The farmers’ assessment demonstrated the need to combine qualitative and quantitative screening methods. The farmers’ combined analysis showed that forage and grain yield are important parameters to the farmers following crop-livestock production systems. Results of the three methods showed that the dual type SP1411 was the preferred cultivar. Future breeding activities should therefore be directed towards the tradeoff trade off between grain yield potential and stover feed quality in the quest for developing a wider range of dual purpose cultivars

    Farmers, advisers and researchers learning together better management of crops and croplands

    Summary. Farmers in the northeastern sub-tropics of Australia must cope with very high climatic variability in order to succeed in crop production. Their capacity for innovation was tapped by means of an on-farm research project that brought farmers, advisers and researchers together on the Darling Downs and in central Queensland. The researchers added value to the farmers&apos; own experiments on fertility and water use efficiency by soil and weather monitoring at specific sites and then using a simulation model of cropping systems to extend findings to a wider context of climate and soil. The advisers extended knowledge aquired from this experience via local farmer networks and have undertaken training in the use of simulation to support farmers&apos; management decisions. The experience described opens up possibilities for developing new, cost-effective ways for devising and testing improved farm management

    Effect of Conservation Agriculture on Maize Yield in the Semi-Arid Areas of Zimbabwe

    Globally, a range of agronomic factors have been reported to have an impact on the performance of conservation agriculture (CA) and often determine its performance in relation to conventional agriculture (CONV). To assess this performance in Zimbabwe, 48 CA experiments were conducted by the International Crops Research Institute for the Semi-Arid Tropics in the semi-arid areas of southern Zimbabwe from 2004 to 2010, to calculate the weighted mean difference (WMD) through meta-analytical methods. The two CA practices, planting basins (Basins) and ripper tillage (Ripper), were compared with CONV. It was hypothesised that CA results improved yield compared with CONV and that the effect of CA practices on yield is affected by soil type, rainfall amount and distribution and selected management practices, which included rates of inorganic fertilisers and manures and mulching. Basins were superior to CONV in 59% of the experiments and the overall effect was significant (p < 0.001). The effect of Ripper was non-significant. The hypothesis that CA practices result in improved maize grain yield over CONV was accepted for Basins. The WMD for experiments conducted on sandy soils was 0.365 t ha−1 for Basins and 0.184 t ha−1 for Ripper, and in both cases was significant (p < 0.05). For clay soils, only the WMD for Basins was significant. A higher rainfall regime (500–830 mm) resulted in a lower WMD for Basins (0.095 t ha−1) and Ripper (0.105 t ha−1) compared with 0.151 t ha−1 for Basins and 0.110 t ha−1 for Ripper under lower rainfall (320–500 mm). The overall effect of Basins under the higher rainfall regime was not significant. There was better yield performance for Basins when the rainfall was well distributed; the reverse was noted for the Ripper. The application of 10–30 kg ha−1 of N (micro-dose range) resulted in a higher WMD for Basins than zero N application. Without N application, the WMD of Basins was not significant. For zero manure application in Basins, the WMD was 0.043 t ha−1 compared with 0.159 t ha−1 when manure was applied. The application of mulch depressed the WMD in Basins by 44% and Ripper by 89%.     Trial protocol OPPTIMUM : does progesterone prophylaxis for the prevention of preterm labour improve outcome?

    Background Preterm birth is a global problem, with a prevalence of 8 to 12% depending on location. Several large trials and systematic reviews have shown progestogens to be effective in preventing or delaying preterm birth in selected high risk women with a singleton pregnancy (including those with a short cervix or previous preterm birth). Although an improvement in short term neonatal outcomes has been shown in some trials these have not consistently been confirmed in meta-analyses. Additionally data on longer term outcomes is limited to a single trial where no difference in outcomes was demonstrated at four years of age of the child, despite those in the “progesterone” group having a lower incidence of preterm birth. Methods/Design The OPPTIMUM study is a double blind randomized placebo controlled trial to determine whether progesterone prophylaxis to prevent preterm birth has long term neonatal or infant benefit. Specifically it will study whether, in women with singleton pregnancy and at high risk of preterm labour, prophylactic vaginal natural progesterone, 200 mg daily from 22 – 34 weeks gestation, compared to placebo, improves obstetric outcome by lengthening pregnancy thus reducing the incidence of preterm delivery (before 34 weeks), improves neonatal outcome by reducing a composite of death and major morbidity, and leads to improved childhood cognitive and neurosensory outcomes at two years of age. Recruitment began in 2009 and is scheduled to close in Spring 2013. As of May 2012, over 800 women had been randomized in 60 sites. Discussion OPPTIMUM will provide further evidence on the effectiveness of vaginal progesterone for prevention of preterm birth and improvement of neonatal outcomes in selected groups of women with singleton pregnancy at high risk of preterm birth. Additionally it will determine whether any reduction in the incidence of preterm birth is accompanied by improved childhood outcome

    Conceptualizing pathways linking women's empowerment and prematurity in developing countries.

    BackgroundGlobally, prematurity is the leading cause of death in children under the age of 5. Many efforts have focused on clinical approaches to improve the survival of premature babies. There is a need, however, to explore psychosocial, sociocultural, economic, and other factors as potential mechanisms to reduce the burden of prematurity. Women's empowerment may be a catalyst for moving the needle in this direction. The goal of this paper is to examine links between women's empowerment and prematurity in developing settings. We propose a conceptual model that shows pathways by which women's empowerment can affect prematurity and review and summarize the literature supporting the relationships we posit. We also suggest future directions for research on women's empowerment and prematurity.MethodsThe key words we used for empowerment in the search were "empowerment," "women's status," "autonomy," and "decision-making," and for prematurity we used "preterm," "premature," and "prematurity." We did not use date, language, and regional restrictions. The search was done in PubMed, Population Information Online (POPLINE), and Web of Science. We selected intervening factors-factors that could potentially mediate the relationship between empowerment and prematurity-based on reviews of the risk factors and interventions to address prematurity and the determinants of those factors.ResultsThere is limited evidence supporting a direct link between women's empowerment and prematurity. However, there is evidence linking several dimensions of empowerment to factors known to be associated with prematurity and outcomes for premature babies. Our review of the literature shows that women's empowerment may reduce prematurity by (1) preventing early marriage and promoting family planning, which will delay age at first pregnancy and increase interpregnancy intervals; (2) improving women's nutritional status; (3) reducing domestic violence and other stressors to improve psychological health; and (4) improving access to and receipt of recommended health services during pregnancy and delivery to help prevent prematurity and improve survival of premature babies.ConclusionsWomen's empowerment is an important distal factor that affects prematurity through several intervening factors. Improving women's empowerment will help prevent prematurity and improve survival of preterm babies. Research to empirically show the links between women's empowerment and prematurity is however needed

    Patient-reported outcomes in the ProtecT randomized trial of clinically localized prostate cancer treatments: Study design, and baseline urinary, bowel and sexual function and quality of life

    Objectives: To present the baseline patient-reported outcome measures (PROMs) in the Prostate Testing for Cancer and Treatment (ProtecT) randomized trial comparing active monitoring, radical prostatectomy and external-beam conformal radiotherapy for localized prostate cancer and to compare results with other populations. Materials and Methods: A total of 1643 randomized men, aged 50-69 years and diagnosed with clinically localized disease identified by prostate-specific antigen (PSA) testing, in nine UK cities in the period 1999-2009 were included. Validated PROMs for disease-specific (urinary, bowel and sexual function) and condition-specific impact on quality of life (Expanded Prostate Index Composite [EPIC], 2005 onwards; International Consultation on Incontinence Questionnaire-Urinary Incontinence [ICIQ-UI], 2001 onwards; the International Continence Society short-form male survey [ICSmaleSF]; anxiety and depression (Hospital Anxiety and Depression Scale [HADS]), generic mental and physical health (12-item short-form health survey [SF-12]; EuroQol quality-of-life survey, the EQ-5D-3L) were assessed at prostate biopsy clinics before randomization. Descriptive statistics are presented by treatment allocation and by men's age at biopsy and PSA testing time points for selected measures. Results: A total of 1438 participants completed biopsy questionnaires (88%) and 77-88% of these were analysed for individual PROMs. Fewer than 1% of participants were using pads daily (5/754). Storage lower urinary tract symptoms were frequent (e.g. nocturia 22%, 312/1423). Bowel symptoms were rare, except for loose stools (16%, 118/754). One third of participants reported erectile dysfunction (241/735) and for 16% (118/731) this was a moderate or large problem. Depression was infrequent (80/1399, 6%) but 20% of participants (278/1403) reported anxiety. Sexual function and bother were markedly worse in older men (65-70 years), whilst urinary bother and physical health were somewhat worse than in younger men (49-54 years, all P < 0.001). Bowel health, urinary function and depression were unaltered by age, whilst mental health and anxiety were better in older men (P < 0.001). Only minor differences existed in mental or physical health, anxiety and depression between PSA testing and biopsy assessments. Conclusion: The ProtecT trial baseline PROMs response rates were high. Symptom frequencies and generic quality of life were similar to those observed in populations screened for prostate cancer and control subjects without cancer

    Progress along developmental tracks for electronic health records implementation in the United States

    The development and implementation of electronic health records (EHR) have occurred slowly in the United States. To date, these approaches have, for the most part, followed four developmental tracks: (a) Enhancement of immunization registries and linkage with other health records to produce Child Health Profiles (CHP), (b) Regional Health Information Organization (RHIO) demonstration projects to link together patient medical records, (c) Insurance company projects linked to ICD-9 codes and patient records for cost-benefit assessments, and (d) Consortia of EHR developers collaborating to model systems requirements and standards for data linkage. Until recently, these separate efforts have been conducted in the very silos that they had intended to eliminate, and there is still considerable debate concerning health professionals access to as well as commitment to using EHR if these systems are provided. This paper will describe these four developmental tracks, patient rights and the legal environment for EHR, international comparisons, and future projections for EHR expansion across health networks in the United States
