A Multi-Criteria Methodology for Measuring the Resilience of Transportation Assets and Prioritizing Security Investments
Transportation project prioritization uses performance measures that are related to the transportation asset, its operations, and its environment. However, in the state of practice, evaluation does not consider directly the likelihood of natural or man-made threats, the infrastructure resilience, or the consequences of the infrastructure damage in the event that the threat occurs. Thus, during the prioritization of investments, assets of low security do not receive the due attention they deserve. In defining security as the lack of risk of damage from threats due to inherent structure or functional resilience, this paper is based on the premise that the inclusion of security considerations in prioritization introduces a much needed element of robustness in investment prioritization However, the inclusion of investment security impacts leads to an increase in the number of performance measures for the investment evaluation. This paper presents a methodology to quantify the overall security level for an asset in terms of the environmental threats it faces, its resilience or vulnerability to damage, and the consequences of the infrastructure damage. The overall framework consists of the traditional steps in risk management, and this paper\u27s specific contribution is in the part of the framework that measures the risk. This paper applies the methodology to a given set of assets by measuring the risk (security) of each asset and prioritizing security investments across multiple assets using multiple criteria analysis

Genomic characterization and detection of potential therapeutic targets for peritoneal mesothelioma in current practice
Peritoneal mesothelioma (PeM) is an aggressive tumor with limited treatment options. The current study aimed to evaluate the value of next generation sequencing (NGS) of PeM samples in current practice. Foundation Medicine F1CDx NGS was performed on 20 tumor samples. This platform assesses 360 commonly somatically mutated genes in solid tumors and provides a genomic signature. Based on the detected mutations, potentially effective targeted therapies were identified. NGS was successful in 19 cases. Tumor mutational burden (TMB) was low in 10 cases, and 11 cases were microsatellite stable. In the other cases, TMB and microsatellite status could not be determined. BRCA1 associated protein 1 (BAP1) mutations were found in 32% of cases, cyclin dependent kinase inhibitor 2A/B (CDKN2A/B) and neurofibromin 2 (NF2) mutations in 16%, and ataxia-telangiectasia mutated serine/threonine kinase (ATM) in 11%. Based on mutations in the latter two genes, potential targeted therapies are available for approximately a quarter of cases (i.e., protein kinase inhibitors for three NF2 mutated tumors, and polyADP-ribose polymerase inhibitors for two ATM mutated tumors). Extensive NGS analysis of PeM samples resulted in the identification of potentially effective targeted therapies for about one in four patients. Although these therapies are currently not available for patients with PeM, ongoing developments might result in new treatment options in the future.</p

Post-diagnosis dietary factors and survival after invasive breast cancer.
Little is known about the effects of diet after breast cancer diagnosis on survival. We prospectively examined the relation between post-diagnosis dietary factors and breast cancer and all-cause survival in women with a history of invasive breast cancer diagnosed between 1987 and 1999 (at ages 20-79 years). Diet after breast cancer diagnosis was measured using a 126-item food frequency questionnaire. Among 4,441 women without a history of breast cancer recurrence prior to completing the questionnaire, 137 subsequently died from breast cancer within 7 years of enrollment. Hazard ratios (HR) and 95% confidence intervals (CI) were estimated for intake of macronutrients as well as selected micronutrients and food groups from Cox proportional hazards regression models. After adjustment for factors at diagnosis (age, state of residence, menopausal status, smoking, breast cancer stage, alcohol, history of hormone replacement therapy), interval between diagnosis and diet assessment, and at follow-up (energy intake, breast cancer treatment, body mass index, and physical activity), women in the highest compared to lowest quintile of intake of saturated fat and trans fat had a significantly higher risk of dying from any cause (HR = 1.41, 95% CI = 1.06-1.87, P trend = 0.03) for saturated fat; (HR = 1.78, 95% CI = 1.35-2.32, P trend = 0.01) for trans fat intake. Associations were similar, though did not achieve statistical significance, for breast cancer survival. This study suggests that lower intake of saturated and trans fat in the post-diagnosis diet is associated with improved survival after breast cancer diagnosis
