    Extremely rare case of a rupture of the left sinus of Valsalva aneurysm into the main pulmonary artery

    We describe a case of ruptured aneurysm of the left coronary sinus of Valsalva diagnosed by transthoracic two-dimensional echocardiography. A very few cases with ruptured left sinus of Valsalva have been reported. We are reporting a rupture of the left sinus of Valsalva into the main pulmonary artery in a 40-year-old male diagnosed by transthoracic echocardiography

    Formulation and Characterization of Aceclofenac Mouth Dissolving Tablet by QbD

    The purpose of this study was to develop fast dissolving tablets of Aceclofenac using different concentration of super disintegrants. Fast dissolving tablets of Aceclofenac were prepared by dry granulation technique using croscarmellose sodium together with avicel ph as superdisintegrants. The porous granules were then compressed in to tablets by direct compression technique. These tablets were evaluated for drug content, weight variation, friability, hardness, wetting time and dispersion time. All the formulations showed low weight variation with dispersion time less than 90 seconds and rapid in vitro dissolution. The drug content of all the formulations was within the acceptable limits. The optimized formulation showed good release profile i.e. maximum drug being released at all-time intervals compared to other trial batches. It was concluded that fast dissolving tablets with improved dissolution could be prepared by dry granulation method of tablet.     Machines can seldom replace dentists in rightly handling the patients with optimistic human insight, considerations, creative planning and the monitoring of psychological acceptance and comfort experienced by any patient with the rehabilitation done. Intelligent computer related armamentarium with software can still help dental practitioners detect typical medical and dental signs and classify them according to certain rules more effectively. Based on image analysis algorithms, CAD systems can be used to look for signs of any tooth pathology that can be spotted in dental X-ray or cone beam computed tomography (CBCT) images. Applying computer vision algorithms to high-resolution CBCT slices helps to a great extent in diagnosing periapical lesions like granulomas, cysts, etc., and can help creating 3-D model of a root canal that reflects its shape with sufficient precision facilitating an optimum endodontic treatment planning. Hence, computer vision systems are already able to speed up the diagnostic process and provide a valuable second opinion in doubtful cases. This can lead a dentist and the patient thoroughly experience an optimistic acceptance and satisfaction of the treatment done

    The Role of Palliative Care Consultation in Withdrawal of Life-Sustaining Treatment among ICU Patients Receiving Veno-Venous Extracorporeal Membrane Oxygenation (VV-ECMO): A Retrospective Case-Control Study

    Background: Extracorporeal membrane oxygenation (ECMO) has extended the survivability of critically ill patients beyond their unsupported prognosis and has widened the timeframe for making an informed decision about the goal of care. However, an extended time window for survival does not necessarily translate into a better outcome and the sustaining treatment is ultimately withdrawn in many patients. Emerging evidence has implicated the determining role of palliative care consult (PCC) in direction of the care that critically ill patients receive. Objective: To evaluate the impact of PCC in withdrawal of life-sustaining treatment (WOLST) among critically ill patients, who were placed on venovenous ECMO (VV-ECMO) at the intensive care unit (ICU) of a tertiary care hospital. Methods: In a retrospective observational study, electronic medical records of 750 patients admitted to the ICU of our hospital between January 1, 2015, and October 31, 2021, were reviewed. Data was collected for patients on VV-ECMO, for whom WOLST was withdrawn during the ICU stay. Clinical characteristics and the underlying reasons for WOLST were compared between those who received PCC (PCC group) and those who did not (non-PCC group). Results: A total of 95 patients were included in our analysis, 63 in the PCC group and 32 in the non-PCC group. The average age of the study population was 48.8 ± 12.6 years, and 64.2% were male. There was no statistically significant difference between the two groups in terms of demographics or clinical characteristics at the time of ICU admission. The average duration of ICU stay and VV-ECMO were 14.1 ± 19.9 days and 9.4 ± 16.6 days, respectively. The number of PCC visits was correlated with the length of ICU stay. The average duration of ICU stay (40.3 ± 33.2 days vs 27.8 ± 19.3 days, P = .05) and ECMO treatment (31.9 ± 27 days vs 18.6 ± 16.1 days, P = .01) were significantly longer in patients receiving PCC than those not receiving PCC. However, the frequency of life sustaining measures or the underlying reasons for WOLST did not significantly differ between the two groups (P \u3e .05). Conclusion: Among ICU patients requiring ECMO support, longer duration of ICU stay and treatment with a higher number of life-sustaining measures seemed to be correlated with the number of PCC visits. The underlying reasons for WOLST seem not to be affected by PCC

    Constraints on the χ_(c1) versus χ_(c2) polarizations in proton-proton collisions at √s = 8 TeV

    The polarizations of promptly produced χ_(c1) and χ_(c2) mesons are studied using data collected by the CMS experiment at the LHC, in proton-proton collisions at √s=8  TeV. The χ_c states are reconstructed via their radiative decays χ_c → J/ψγ, with the photons being measured through conversions to e⁺e⁻, which allows the two states to be well resolved. The polarizations are measured in the helicity frame, through the analysis of the χ_(c2) to χ_(c1) yield ratio as a function of the polar or azimuthal angle of the positive muon emitted in the J/ψ → μ⁺μ⁻ decay, in three bins of J/ψ transverse momentum. While no differences are seen between the two states in terms of azimuthal decay angle distributions, they are observed to have significantly different polar anisotropies. The measurement favors a scenario where at least one of the two states is strongly polarized along the helicity quantization axis, in agreement with nonrelativistic quantum chromodynamics predictions.
