    Health Disparities Between Appalachian and Non-Appalachian Counties in Virginia USA

    The examination of health disparities among people within Appalachian counties compared to people living in other counties is needed to find ways to strategically target improvements in community health in the United States of America (USA). Methods: A telephone survey of a random sample of adults living in households within communities of all counties of the state of Virginia (VA) in the USA was conducted. Findings: Health status was poorer among those in communities within Appalachian counties in VA and health insurance did not make a difference. Health perception was significantly worse in residents within communities in Appalachian counties compared to non-Appalachian community residents (30.5 vs. 17.4% rated their health status as poor/fair), and was worse even among those with no chronic diseases. Within communities in Appalachian counties, black residents report significantly better health perception than do white residents. Conclusion: Residents living in communities in Appalachian counties in VA are not receiving adequate health care, even among those with health insurance. More research with a larger ethnic minority sample is needed to investigate the racial/ethnic disparities in self-reported health and health care utilization within communities

    Long-Term Alterations of Cytokines and Growth Factors Expression in Irradiated Tissues and Relation with Histological Severity Scoring

    PURPOSE: Beside its efficacy in cancer treatment, radiotherapy induces degeneration of healthy tissues within the irradiated area. The aim of this study was to analyze the variations of proinflammatory (IL-1α, IL-2, IL-6, TNF-α, IFN-γ), profibrotic (TGF-β1), proangiogneic (VEGF) and stem cell mobilizing (GM-CSF) cytokines and growth factors in an animal model of radiation-induced tissue degeneration. MATERIALS AND METHODS: 24 rats were irradiated unilaterally on the hindlimb at a monodose of 30 Gy. Six weeks (n=8), 6 months (n=8) and 1 year (n=8) after irradiation the mediators expression in skin and muscle were analyzed using Western blot and the Bio-Plex® protein array (BPA) technology. Additional histological severity for fibrosis, inflammation, vascularity and cellularity alterations scoring was defined from histology and immnunohistochemistry analyses. RESULTS: A significant increase of histological severity scoring was found in irradiated tissue. Skin tissues were more radio-sensitive than muscle. A high level of TGF-β1 expression was found throughout the study and a significant relation was evidenced between TGF-β1 expression and fibrosis scoring. Irradiated tissue showed a chronic inflammation (IL-2 and TNF-α significantly increased). Moreover a persistent expression of GM-CSF and VEGF was found in all irradiated tissues. The vascular score was related to TGF-β1 expression and the cellular alterations score was significantly related with the level of IL-2, VEGF and GM-CSF. CONCLUSION: The results achieved in the present study underline the complexity and multiplicity of radio-induced alterations of cytokine network. It offers many perspectives of development, for the comprehension of the mechanisms of late injuries or for the histological and molecular evaluation of the mode of action and the efficacy of rehabilitation techniques

    Effect of humidity and temperature on conidial germination and appressorium development of two Philippine isolates of the mango anthracnose pathogen Colletotrichum gloeosporioides

    A comparison of rates of germination and appressorium formation by an isolate of Colletotrichum gloeosporioides on mango leaves, fruit surfaces and cellophane membranes showed that behaviour was broadly similar on all three substrates. Frequency of appressorium formation was slightly higher on cellophane membranes, and both hyaline and melanized appressoria were formed. Only melanized appressoria were formed on mango surfaces. In vitro experiments on membranes showed comparative differences in physiological behaviour with temperature for two Philippine isolates of C. gloeosporioides. The most stimulatory temperature for production of appressoria differed in isolates I-2 and I-4 (25 and 20 degrees C, respectively). At 30 degrees C more appressoria became melanized than at lower temperatures, but the frequency of formation of penetration pegs was highest at 25 degrees C. Conidia of C. gloeosporioides germinated on cellophane membranes at relative humidities as low as 95%, but the percentage of conidia germinating and forming appressoria increased as the RH approached 100%. Approximately 18% of conidia of C. gloeosporioides I-2 held at 62 and 86% RH for 4 weeks retained viability, and some were capable of forming appressoria when placed at 100% RH. These results have implications for epidemiological models for disease control

    Social Innovation in Public Organisations: The Perspectives of Managers

    The world today is not only becoming increasingly competitive but this competition is also increasingly intense and, for organisations, innovation emerges as one solution to the problems they face. Thus, innovation has attracted increased interest as the means to acquire and develop skills to address and/or inspire organisations in a globally competitive and economically uncertain environment. Furthermore, in such a competitive world, not even the public sector escapes the need to evolve towards providing better services and satisfying its citizens. However, just what is the need to evaluate social innovation in public organisations? Is there the drive to compete and/or market share to conquer? Profits to achieve? The public sector has many points in common with the private sector with the goals in both sectors involving the raising of efficiency and quality levels and better satisfaction for customers. Thus, this research aims at studying social innovation in public organisations and understanding how social innovation can better the lives of citizens from the perspective of managers. As the data collection method, we applied a questionnaire based on the constructs and scales already validated by Vigoda-Gadot (Public Administration 86(2):307–329, 2008) before then analysing the results through recourse to PLS. The results identify how responsiveness and ethics and morals are important factors in the perceptions of innovation as perceived by citizens. In turn, the findings also demonstrate the importance of this capacity for innovation to the image of the public sector as well as the confidence and satisfaction of its citizens.info:eu-repo/semantics/publishedVersio
