    Butler Matrix Based Six-port Passive Junction

    In this work we propose the utilization of the Butler matrix as a six-port I/Q demodulator. The inherent symmetry of its topology allows to overcome the intrinsic amplitude and phase imbalances of the traditional approaches making it more suitable for the design of high performance six-port networks. To demonstrate the validity of the proposal a Butler matrix has been designed covering the complete UWB band and its results are compared with the traditional approaches

    An Ultracompact GRIN-Lens-Based Spot Size Converter using Subwavelength Grating Metamaterials

    AbstractGraded‐index materials offer virtually complete control over light propagation in integrated photonic chips but can be challenging to implement. Here, an anisotropic graded‐index metamaterial, synthesized with fully etched silicon subwavelength structures, is proposed. Based on this material, a spot size converter that expands the transverse electric (TE) mode field profile from a 0.5 µm wide silicon wire waveguide to a 15 µm wide waveguide within a length of only 14 µm is designed. Measured insertion losses are below 1 dB in an unprecedented 130 nm bandwidth, limited by the measurement setup, with full 3D finite‐difference time‐domain (FDTD) simulations predicting a bandwidth in excess of 300 nm. Furthermore, the device is well suited to feed fiber‐to‐chip grating couplers, while requiring a footprint ten times smaller than conventional adiabatic tapers

    Sub-wavelength structures are enabling the design of devices based in dielectric waveguides with unprecedented performance in both the near-infrared and mid-infrared wavelength regions. These devices include fiber-to-chip grating couplers with sub-decibel efficiency, waveguide couplers with bandwidths of several hundred nanometers, and low loss suspended waveguides. Here we will report our progress in the electromagnetic modelling and simulation of sub-wavelength structures, providing at the same time an intuitive vision of their fundamental optical properties. Furthermore, we will address design strategies for several integrated optical devices based on these structures, and present the latest experimental results for structures operating both at near and mid-infrared wavelengths.Universidad de Málaga.     Bricked and evanescently-coupled topologies: expanding the portfolio of subwavelength metamaterial silicon photonic devices

    We present two novel topologies of subwavelength grating (SWG) waveguides: the bricked-SWG and the evanescently-coupled-SWG. The bricked topology enables accurate control of waveguide anisotropy while maintaining the index and dispersion engineering advantage intrinsic to SWG waveguides. The evanescently-coupled-SWG allows unprecedented control of the strength of the modal perturbation in waveguide Bragg gratings and nanophotonic antennas. Both topologies leverage a Manhattan-like pattern, with pixel sizes compatible with deep-uv lithography. Our recent results will be discussed, focusing on polarization-independent multimode interference couplers for the O and C bands and a millimeter-long narrow-beam steerable optical antenna array with angular divergence of only 1.8º×0.2º.Universidad de Málaga.     Silicon high performance devices using subwavelength structures

This requires a wide variety of integrated silicon devices, including fiber-to-chip couplers, polarization splitters and waveguide couplers, operating both in the near-infrared and the mid-infrared wavelength range. However, the reduced set of materials available in this platform can often limit the performance of these devices. Subwavelength structures enable the synthesis of optical metamaterials, with properties than can be tuned to enhance device performance, by using fully etched silicon structures with a periodicity smaller than the wavelength of light. Here we review the basic operating principles of these structures, discuss how to efficiently model them, and report on the latest advances in this rapidly growing field.Universidad de Málaga.     RICORS2040 : The need for collaborative research in chronic kidney disease

    Chronic kidney disease (CKD) is a silent and poorly known killer. The current concept of CKD is relatively young and uptake by the public, physicians and health authorities is not widespread. Physicians still confuse CKD with chronic kidney insufficiency or failure. For the wider public and health authorities, CKD evokes kidney replacement therapy (KRT). In Spain, the prevalence of KRT is 0.13%. Thus health authorities may consider CKD a non-issue: very few persons eventually need KRT and, for those in whom kidneys fail, the problem is 'solved' by dialysis or kidney transplantation. However, KRT is the tip of the iceberg in the burden of CKD. The main burden of CKD is accelerated ageing and premature death. The cut-off points for kidney function and kidney damage indexes that define CKD also mark an increased risk for all-cause premature death. CKD is the most prevalent risk factor for lethal coronavirus disease 2019 (COVID-19) and the factor that most increases the risk of death in COVID-19, after old age. Men and women undergoing KRT still have an annual mortality that is 10- to 100-fold higher than similar-age peers, and life expectancy is shortened by ~40 years for young persons on dialysis and by 15 years for young persons with a functioning kidney graft. CKD is expected to become the fifth greatest global cause of death by 2040 and the second greatest cause of death in Spain before the end of the century, a time when one in four Spaniards will have CKD. However, by 2022, CKD will become the only top-15 global predicted cause of death that is not supported by a dedicated well-funded Centres for Biomedical Research (CIBER) network structure in Spain. Realizing the underestimation of the CKD burden of disease by health authorities, the Decade of the Kidney initiative for 2020-2030 was launched by the American Association of Kidney Patients and the European Kidney Health Alliance. Leading Spanish kidney researchers grouped in the kidney collaborative research network Red de Investigación Renal have now applied for the Redes de Investigación Cooperativa Orientadas a Resultados en Salud (RICORS) call for collaborative research in Spain with the support of the Spanish Society of Nephrology, Federación Nacional de Asociaciones para la Lucha Contra las Enfermedades del Riñón and ONT: RICORS2040 aims to prevent the dire predictions for the global 2040 burden of CKD from becoming true

    Goodbye Hartmann trial: a prospective, international, multicenter, observational study on the current use of a surgical procedure developed a century ago

    Background: Literature suggests colonic resection and primary anastomosis (RPA) instead of Hartmann's procedure (HP) for the treatment of left-sided colonic emergencies. We aim to evaluate the surgical options globally used to treat patients with acute left-sided colonic emergencies and the factors that leading to the choice of treatment, comparing HP and RPA. Methods: This is a prospective, international, multicenter, observational study registered on ClinicalTrials.gov. A total 1215 patients with left-sided colonic emergencies who required surgery were included from 204 centers during the period of March 1, 2020, to May 31, 2020. with a 1-year follow-up. Results: 564 patients (43.1%) were females. The mean age was 65.9 ± 15.6 years. HP was performed in 697 (57.3%) patients and RPA in 384 (31.6%) cases. Complicated acute diverticulitis was the most common cause of left-sided colonic emergencies (40.2%), followed by colorectal malignancy (36.6%). Severe complications (Clavien-Dindo ≥ 3b) were higher in the HP group (P < 0.001). 30-day mortality was higher in HP patients (13.7%), especially in case of bowel perforation and diffused peritonitis. 1-year follow-up showed no differences on ostomy reversal rate between HP and RPA. (P = 0.127). A backward likelihood logistic regression model showed that RPA was preferred in younger patients, having low ASA score (≤ 3), in case of large bowel obstruction, absence of colonic ischemia, longer time from admission to surgery, operating early at the day working hours, by a surgeon who performed more than 50 colorectal resections. Conclusions: After 100 years since the first Hartmann's procedure, HP remains the most common treatment for left-sided colorectal emergencies. Treatment's choice depends on patient characteristics, the time of surgery and the experience of the surgeon. RPA should be considered as the gold standard for surgery, with HP being an exception

    Global disparities in surgeons’ workloads, academic engagement and rest periods: the on-calL shIft fOr geNEral SurgeonS (LIONESS) study

    : The workload of general surgeons is multifaceted, encompassing not only surgical procedures but also a myriad of other responsibilities. From April to May 2023, we conducted a CHERRIES-compliant internet-based survey analyzing clinical practice, academic engagement, and post-on-call rest. The questionnaire featured six sections with 35 questions. Statistical analysis used Chi-square tests, ANOVA, and logistic regression (SPSS® v. 28). The survey received a total of 1.046 responses (65.4%). Over 78.0% of responders came from Europe, 65.1% came from a general surgery unit; 92.8% of European and 87.5% of North American respondents were involved in research, compared to 71.7% in Africa. Europe led in publishing research studies (6.6 ± 8.6 yearly). Teaching involvement was high in North America (100%) and Africa (91.7%). Surgeons reported an average of 6.7 ± 4.9 on-call shifts per month, with European and North American surgeons experiencing 6.5 ± 4.9 and 7.8 ± 4.1 on-calls monthly, respectively. African surgeons had the highest on-call frequency (8.7 ± 6.1). Post-on-call, only 35.1% of respondents received a day off. Europeans were most likely (40%) to have a day off, while African surgeons were least likely (6.7%). On the adjusted multivariable analysis HDI (Human Development Index) (aOR 1.993) hospital capacity > 400 beds (aOR 2.423), working in a specialty surgery unit (aOR 2.087), and making the on-call in-house (aOR 5.446), significantly predicted the likelihood of having a day off after an on-call shift.
