Reattendance in the Swiss mammography screening pilot programme
[Abstract] OBJECTIVES: To explore the patterns and determinants of reattendance among initial attendees at an organised breast cancer screening programme. SETTING: Swiss mammography screening pilot programme, based on first-round enrollment in 1993-1995. METHODS: Data on socio-demographic features, health preventive behaviour and history, prior screening round and referring physicians' attributes were collected at initial attendance. First-round attendees (n=4162) were followed prospectively through next screening round. Determinants of reattendance were identified by multivariate logistic regression. RESULTS: Reattendance rate was high (80%). Women most likely to reattend were urban, Swiss residents with a true-negative first-round screening result. The more intensive the initial recruitment efforts, the lesser were the odds of reattendance. Mammography screening prior to screening enrolment increased reattendance. Having a gynaecologist, a female and a younger doctor as a referring physician was positively associated with reattendance. CONCLUSION: Mammography screening attendance is not only influenced by women-related factors but also by structural factors, and thus requires a global approach. For strategies aimed at improving performances of organised cancer screening programmes, understanding and quantification of determinants of (re)attendance are key elements to consider. [Authors]]]>

<![CDATA[Contact time (t <sub>c</sub> ) relies upon the accuracy of foot-strike and toe-off events, for which ground reaction force (GRF) is the gold standard. However, force plates are not always available, e.g., when running on a noninstrumented treadmill. In this situation, a kinematic algorithm (KA) - an algorithm based on motion capture data - might be used if it performs equally for all foot-strike angles across speeds. The purpose of this study was to propose a novel KA, using a combination of heel and toe kinematics (three markers per foot), to detect foot-strike and toe-off and compare it to GRF at different speeds and across foot-strike angles. One hundred runners ran at 9 km/h, 11 km/h, and 13 km/h. Force data and whole-body kinematic data were acquired by an instrumented treadmill and optoelectronic system. Foot-strike and toe-off showed small systematic biases between GRF and KA at all speeds (≤5 ms), except toe-off at 11 km/h (no bias). The root mean square error (RMSE) was ≤9 ms and was mostly constant across foot-strike angles for toe-off (7.4 ms) but not for foot-strike (4.1-11.1 ms). Small systematic biases (≤8 ms) and significant differences (P ≤ 0.01) were reported for t <sub>c</sub> at all speeds, and the RMSE was ≤14 ms (≤5%). The RMSE for t <sub>c</sub> increased with increasing foot-strike angle (3.5-5.4%). Nonetheless, this novel KA computed smaller errors than existing methods for foot-strike, toe-off, and t <sub>c</sub> . Therefore, this study supports the use of this novel KA to accurately estimate foot-strike, toe-off, and t <sub>c</sub> from kinematic data obtained during noninstrumented treadmill running independent of the foot-strike angle
Profile of women not attending in the Swiss Mammography Screening Pilot Programme
[Abstract] The Swiss Mammography Screening Pilot Programme was conducted between 1993 and 1998 in three areas of the French-speaking canton of Vaud. Socio-demographic characteristics were available for the whole of the target population and were used to identify determinants of initial (in the first round) and repeated (in both first and second rounds) non-attendance by means of logistic regression analyses. Initial non-attendance was higher among non-Swiss, single or divorced/separated women, and increased with distance between the residence and the screening centre. Being single and aged 65-70, not living in the vicinity of the screening centre and replying to decline the invitation to the initial screening session significantly increased the odds of persisting in non-attendance for screening within the programme. Factors such as age, nationality, marital status and screening centre differed in their influence on initial and repeated non-attendance, and the type of reply to the invitation was strongly predictive of non-attendance in the next round. <![CDATA[Thirteen patients with mild mitral stenosis and 21 asymptomatic patients after commissurotomy were studied by echocardiography in order to assess the rate of progression of mitral stenosis and the incidence of restenosis after successful open mitral commissurotomy. In the group with mitral stenosis there was a decrease of the diastolic closing velocity (E-F slope) from 35.7 to 29.5 mm./sec. (p less than 0.0005) over a period of 37 months. In 23% of the patients the stenosis increased significantly (p less than 0.0005) by echocardiographic parameters. Forty-eight months after commissurotomy we noted a significant over-all slowing of the diastolic closing velocity (from 52.6 to 44.8 mm./sec., p less than 0.0005) and a decrease of the mitral valve closure index DE/MAIC (from 1.7 to 1.5, p less than 0.0025). Five of 21 patients (24%) showed a change in one or both of these parameters which was greater than 2 standard deviations of the mean change. Based on echocardiographic criteria, we conclude that patients with mild mitral stenosis and asymptomatic patients following successful commissurotomy need only be checked approximately every 3 years

<![CDATA[Left rostral dorsal premotor cortex (rPMd) and supramarginal gyrus (SMG) have been implicated in the dynamic control of actions. In 12 right-handed healthy individuals, we applied 30 min of low-frequency (1 Hz) repetitive transcranial magnetic stimulation (rTMS) over left rPMd to investigate the involvement of left rPMd and SMG in the rapid adjustment of actions guided by visuospatial cues. After rTMS, subjects underwent functional magnetic resonance imaging while making spatially congruent button presses with the right or left index finger in response to a left- or right-sided target. Subjects were asked to covertly prepare motor responses as indicated by a directional cue presented 1 s before the target. On 20% of trials, the cue was invalid, requiring subjects to readjust their motor plan according to the target location. Compared with sham rTMS, real rTMS increased the number of correct responses in invalidly cued trials. After real rTMS, task-related activity of the stimulated left rPMd showed increased task-related coupling with activity in ipsilateral SMG and the adjacent anterior intraparietal area (AIP). Individuals who showed a stronger increase in left-hemispheric premotor-parietal connectivity also made fewer errors on invalidly cued trials after rTMS. The results suggest that rTMS over left rPMd improved the ability to dynamically adjust visuospatial response mapping by strengthening left-hemispheric connectivity between rPMd and the SMG-AIP region. These results support the notion that left rPMd and SMG-AIP contribute toward dynamic control of actions and demonstrate that low-frequency rTMS can enhance functional coupling between task-relevant brain regions and improve some aspects of motor performance
Results from the Swiss mammography screening pilot programme
The first Swiss mammography screening pilot programme operated between 1993 and 1998. Approximately 15000 women aged 50-69 years and residing in western Switzerland (canton of Vaud) were offered a biennial screening. Quality standard recommendations for screening were met for most performance indicators. Some 4.6%/2.3% (prevalent/incident round) of participants were referred for further assessment, and 84.7%/75.6% of them turned out not to have cancer. Specificity was high (96.1%/98.2%) and the cancer detection rate amounted to 7.0/1000 and 5.9/1000 in the prevalent and incident rounds, respectively. Some 30%/26% of cancers in screened women were interval cancers, which were at a more advanced stage than screen-detected tumours. Screening performances improved with time and age. The objectives of feasibility and acceptability of an organised mammography screening programme in the liberal Swiss healthcare system, where routine opportunistic screening existed, were achieved and contributed to the implementation of screening programmes in two additional Swiss cantons. <![CDATA[An efficient screening strategy for the identification of potentially interesting low-abundance antifungal natural products in crude extracts that combines both a sensitive bioautography assay and high performance liquid chromatography (HPLC) microfractionation was developed. This method relies on high performance thin layer chromatography (HPTLC) bioautography with a hypersusceptible engineered strain of Candida albicans (DSY2621) for bioactivity detection, followed by the evaluation of wild type strains in standard microdilution antifungal assays. Active extracts were microfractionated by HPLC in 96-well plates, and the fractions were subsequently submitted to the bioassay. This procedure enabled precise localisation of the antifungal compounds directly in the HPLC chromatograms of the crude extracts. HPLC-PDA-mass spectrometry (MS) data obtained in parallel to the HPLC antifungal profiles provided a first chemical screening about the bioactive constituents. Transposition of the HPLC analytical conditions to medium-pressure liquid chromatography (MPLC) allowed the efficient isolation of the active constituents in mg amounts for structure confirmation and more extensive characterisation of their biological activities. The antifungal properties of the isolated natural products were evaluated by their minimum inhibitory concentration (MIC) in a dilution assay against both wild type and engineered strains of C. albicans. The biological activity of the most promising agents was further evaluated in vitro by electron microscopy and in vivo in a Galleria mellonella model of C. albicans infection. The overall procedure represents a rational and comprehensive means of evaluating antifungal activity from various perspectives for the selection of initial hits that can be explored in more in-depth mode-of-action studies. This strategy is illustrated by the identification and bioactivity evaluation of a series of antifungal compounds from the methanolic extract of a Rubiaceae plant, Morinda tomentosa, which was used as a model in these studies
Participation de la population au dépistage du cancer du sein : déterminants et enjeux
Cet article présente le résumé de l'atelier consacré au dépistage du cancer du sein organisé dans le cadre de la 70e Assemblée annuelle de la Société suisse de médecine interne qui s'est tenue à Genève du 25 au 27 avril 2002. L'objectif de cet atelier était d'identifier, sur la base des programmes pilotes conduits en Suisse ainsi que dans le Bas-Rhin (France), les modalités d'organisation les mieux adaptées au contexte sanitaire et culturel local afin d'optimiser la participation de la population aux programmes de dépistage du cancer susceptibles d'être mis en oeuvre dans les prochaines années. En effet, une participation élevée constitue, avec l'exigence de qualité, la condition nécessaire pour que ces programmes soient efficaces
Conflits et polémiques dans l’épistolaire
La lettre, lieu d’échange et de dialogue, traditionnellement définie depuis Cicéron comme une « conversation entre amis séparés », peut également offrir un espace privilégié à l’expression des antagonismes - malentendus, différends, âpres controverses. Croiser la notion d’épistolaire avec celle de conflit et de polémique amène alors à repenser avec une acuité particulière la signification de la lettre. Un premier aspect important concerne l’attitude de l’épistolier, son degré d’implication et l’image de soi qu’il cherche à construire (ethos) : si en cas d’affrontement direct la violence verbale est généralement évitée, quand le destinataire est seulement pris à témoin la lettre donne volontiers cours au langage des passions (indignation, moqueries), aux procédés de dévalorisation de l’adversaire ou tout au moins à l’ironie. Mais c’est aussi la fonction et la portée de la lettre qui est en jeu, qu’il s’agisse de clarifier sa pensée devant l’autre, de diffuser un débat, d’exacerber les tensions ou au contraire d’œuvrer à l’apaisement. Telles sont les questions examinées à travers les trente contributions de ce volume, qui traitent de conflits personnels entre correspondants, de luttes politiques, de controverses religieuses et théologiques, de confrontations d’idées (philosophie et rhétorique), d’affrontements autour de la médecine, et enfin de rivalités entre érudits et de polémiques littéraires
Conseiller, diriger par lettre
La lettre, discours adressé à une personne ou à un ensemble de personnes, est souvent le lieu de suggestions, d'avertissements ou d'exhortations, quand l'épistolier écrit pour influer sur la pensée et le comportement de son correspondant, ou même assume le rôle de directeur de conscience. Cette fonction de l'écriture épistolaire - conseiller, diriger, prescrire - invite à examiner plus précisément la relation au destinataire, la figure d'autorité que construit la lettre pour inspirer confiance, les moyens déployés pour convaincre, l'adaptation à telle circonstance particulière, l'articulation entre perspective théorique et cas pratique. La lettre de conseil déborde d'autant mieux le cadre argumentatif de la délibération qu'elle sert parfois à conforter une opinion acquise (parénèse), recourt aux maximes et préceptes, aux exemples qui stimulent l'imitation, aux métaphores qui visent la sensibilité, avec une plasticité irréductible aux schémas rhétoriques. Tels sont les aspects étudiés à travers ces trente et une contributions qui, de l'Antiquité gréco-romaine au xxe siècle, explorent la signification et les enjeux du conseil par voie épistolaire, qu'il s'agisse d'avis politiques, de direction philosophique et spirituelle, d'éducation religieuse, de directives de spécialistes (philologie, médecine) ou de conseils personnels
La présence de l’histoire dans l’épistolaire
Dans les lettres que nous a léguées l’Antiquité gréco-romaine comme dans les correspondances plus récentes, l’histoire est présente sous différents aspects, liés à l’ambiguïté du mot « histoire », qui désigne la connaissance du passé humain aussi bien que ce passé lui-même. La présence de l’histoire dans l’épistolaire est donc celle des historiens et du genre historique, mais aussi celle des événements : événements du passé, qui constituent une source inépuisable d’exempla ; événements contemporains, dont l’épistolier se fait, volontairement ou non, le témoin. Cette présence est étudiée ici, selon des approches diverses, mais souvent convergentes, chez plus de vingt auteurs, surtout grecs et latins, avec des prolongements médiévaux, renaissants et modernes. Mieux que tout autre document historique, les lettres ne favorisent-elles pas cette « rencontre fraternelle avec des hommes », que Marc Bloch situait au cœur du « métier d’historien »
