    A new type of matrix, termed permutative, is defined and motivated herein. The focus is upon identifying circumstances under which square permutative matrices are rank deficient. Two distinct ways, along with variants upon them are given. These are a special kind of grouping of rows and a type of partition in which the blocks are again permutative. Other, results are given, along with some questions and conjectures

    Continuous renal replacement therapy for two neonates with hyperammonemia

    Objectives: This study aims to assess the feasibility of using hemofiltration for ammonia clearance in low body weight infants with an inborn error of metabolism. Design: A study of two cases. Setting: Quaternary pediatric hospital (Saint Louis Children's Hospital) NICU and PICU. Patients: Infants <6 months of age with an ICD-9 diagnosis of 270.6 (hyperammonemia). Interventions: Continuous renal replacement therapy (CRRT). Measurements and Main Results: We measure serum ammonia levels over time and the rate of ammonia clearance over time. Continuous renal replacement therapy was more effective than scavenger therapy alone (Ammonul™) for rapid removal of ammonia in low weight infants (as low as 2.5 kg). Conclusions: Continuous renal replacement therapy is technically feasible in low weight infants with severe hyperammonemia secondary to an inborn error of metabolism

    Does continuity of care impact decision making in the next birth after a caesarean section (VBAC)? A randomised controlled trial

    BACKGROUND: Caesarean section (CS) has short and long-term health effects for both the woman and her baby. One of the greatest contributors to the CS rate is elective repeat CS. Vaginal birth after caesarean (VBAC) is an option for many women; despite this the proportion of women attempting VBAC remains low. Potentially the relationship that women have with their healthcare professional may have a major influence on the uptake of VBAC. Models of service delivery, which enable an individual approach to care, may make a difference to the uptake of VBAC. Midwifery continuity of care could be an effective model to encourage and support women to choose VBAC. METHODS/DESIGN: A randomised, controlled trial will be undertaken. Eligible pregnant women, whose most recent previous birth was by lower-segment CS, will be randomly allocated 1:1 to an intervention group or control group. The intervention provides midwifery continuity of care to women through pregnancy, labour, birth and early postnatal care. The control group will receive standard hospital care from different midwives through pregnancy, labour, birth and early postnatal care. Both groups will receive an obstetric consultation during pregnancy and at any other time if required. Clinical care will follow the same guidelines in both groups. DISCUSSION: This study will determine whether midwifery continuity of care influences the decision to attempt a VBAC and impacts on mode of birth, maternal experiences with care and the health of the neonate. Outcomes from this study might influence the way maternity care is provided to this group of women and thus impact on the CS rate. This information will provide high level evidence to policy makers, health service managers and practitioners who are working towards addressing the increased rate of CS.     Microbial-Induced Heterogeneity in the Acoustic Properties of Porous Media

    It is not known how biofilms affect seismic wave propagation in porous media. Such knowledge is critical for assessing the utility of seismic techniques for imaging biofilm development and their effects in field settings. Acoustic wave data were acquired over a two-dimensional region of a microbial-stimulated sand column and an unstimulated sand column. The acoustic signals from the unstimulated column were relatively uniform over the 2D scan region. The data from the microbial-stimulated column exhibited a high degree of spatial heterogeneity in the acoustic wave amplitude, with some regions exhibiting significant increases in attenuation while others exhibited decreases. Environmental scanning electron microscopy showed differences in the structure of the biofilm between regions of increased and decreased acoustic wave amplitude. We conclude from these observations that variations in microbial growth and biofilm structure cause heterogeneity in the elastic properties of porous media with implications for the validation of bioclogging models

    Micronutrient Dietary Intake in Latina Pregnant Adolescents and Its Association with Level of Depression, Stress, and Social Support

    Adolescent pregnant women are at greater risk for nutritional deficits, stress, and depression than their adult counterparts, and these risk factors for adverse pregnancy outcomes are likely interrelated. This study evaluated the prevalence of nutritional deficits in pregnant teenagers and assessed the associations among micronutrient dietary intake, stress, and depression. One hundred and eight pregnant Latina adolescents completed an Automated Self-Administered 24-hour dietary recall (ASA24) in the 2nd trimester. Stress was measured using the Perceived Stress Scale and the Prenatal Distress Questionnaire. Depressive symptoms were evaluated with the Reynolds Adolescent Depression Scale. Social support satisfaction was measured using the Social Support Questionnaire. More than 50% of pregnant teenagers had an inadequate intake (excluding dietary supplement) of folate, vitamin A, vitamin E, iron, zinc, calcium, magnesium, and phosphorous. Additionally, >20% of participants had an inadequate intake of thiamin, riboflavin, niacin, vitamin B6, vitamin B12, vitamin C, copper, and selenium. Prenatal supplement inclusion improved dietary intake for most micronutrients except for calcium, magnesium, and phosphorous, (>50% below the Estimated Average Requirement (EAR)) and for copper and selenium (>20% below the EAR). Higher depressive symptoms were associated with higher energy, carbohydrates, and fats, and lower magnesium intake. Higher social support satisfaction was positively associated with dietary intake of thiamin, riboflavin, niacin, vitamin B6, folate, vitamin B12, vitamin C, vitamin E, iron, and zinc. The findings suggest that mood and dietary factors are associated and should be considered together for health interventions during adolescent pregnancy for the young woman and her future child

    Madagascar's fire regimes challenge global assumptions about landscape degradation

    Narratives of landscape degradation are often linked to unsustainable fire use by local communities. Madagascar is a case in point: the island is considered globally exceptional, with its remarkable endemic biodiversity viewed as threatened by unsustainable anthropogenic fire. Yet, fire regimes on Madagascar have not been empirically characterised or globally contextualised. Here, we contribute a comparative approach to determining relationships between regional fire regimes and global patterns and trends, applied to Madagascar using MODIS remote sensing data (2003–2019). Rather than a global exception, we show that Madagascar's fire regimes are similar to 88% of tropical burned area with shared climate and vegetation characteristics, and can be considered a microcosm of most tropical fire regimes. From 2003–2019, landscape-scale fire declined across tropical grassy biomes (17%–44% excluding Madagascar), and on Madagascar at a relatively fast rate (36%–46%). Thus, high tree loss anomalies on the island (1.25–4.77× the tropical average) were not explained by any general expansion of landscape-scale fire in grassy biomes. Rather, tree loss anomalies centred in forests, and could not be explained by landscape-scale fire escaping from savannas into forests. Unexpectedly, the highest tree loss anomalies on Madagascar (4.77×) occurred in environments without landscape-scale fire, where the role of small-scale fires (&lt;21 h [0.21 km2]) is unknown. While landscape-scale fire declined across tropical grassy biomes, trends in tropical forests reflected important differences among regions, indicating a need to better understand regional variation in the anthropogenic drivers of forest loss and fire risk. Our new understanding of Madagascar's fire regimes offers two lessons with global implications: first, landscape-scale fire is declining across tropical grassy biomes and does not explain high tree loss anomalies on Madagascar. Second, landscape-scale fire is not uniformly associated with tropical forest loss, indicating a need for socio-ecological context in framing new narratives of fire and ecosystem degradation

    Caribbean Sea Soundscapes: Monitoring Humpback Whales, Biological Sounds, Geological Events, and Anthropogenic Impacts of Vessel Noise

    Assessing marine soundscapes provides an understanding of the biological, geological and anthropogenic composition of a habitat, including species diversity, community composition, and human impacts. For this study, nine acoustic recorders were deployed between December 2016 and June 2017 off six Caribbean islands in several Marine Parks: the Dominican Republic (DR), St. Martin (SM), Guadeloupe east and west (GE, GW), Martinique (MA), Aruba (AR), and Bonaire (BO). Humpback whale song was recorded at five sites on four islands (DR, SM, GE, GW, and MA) and occurred on 49–93% of recording days. Song appeared first at the DR site and began 4–6 weeks later at GE, GW, and MA. No song was heard in AR and BO, the southernmost islands. A 2-week period was examined for the hourly presence of vessel noise and the number and duration of ship passages. Hourly vessel presence ranged from low (20% – DR, 30% – SM), medium (52% – MA, 54% – BO, 77% – GE) to near continuous (99% – GW; 100% – AR). Diurnal patterns were observed at BO, GE, and MA with few to no vessels present during night time hours, possibly reflecting the activity of recreational craft and fishing vessels. At the DR and GW sites, vessel traffic was ubiquitous for most of the day, likely reflecting heavy cruise ship and container ship presence. Soundscapes were diverse across islands with persistent fish choruses, sporadic sperm whale (Physeter macrocephalus) and dolphin (Delphinidae) presence at BO, minke whales (Balaenoptera acutorostrata) from late December to late February at MA and an earthquake recorded across all sites. These analyses provide an important first step in characterizing the health and species richness in Caribbean marine parks and demonstrate a surprising high anthropogenic foot print. Vessel traffic in particular contributes adversely to marine soundscapes, masking marine mammal sounds, potentially changing typical animal behavior and raising the risk of ship strike
