Nasal lysine aspirin challenge in the diagnosis of aspirin - exacerbated respiratory disease
Background
Aspirin-exacerbated respiratory disease is under-diagnosed and therefore effective and inexpensive therapy with aspirin desensitization is rarely performed.
Methods
We present an audit of 150 patients with difficult to treat nasal polyposis, 132 of whom also had asthma, 131 of whom underwent challenge with the only soluble form of aspirin, lysine aspirin (LAS), to confirm or exclude the diagnosis of aspirin-exacerbated respiratory disease (AERD).
Results
One hundred patients proved positive on nasal challenge, 31 who were negative went onto oral LAS challenge and a further 14 gave positive results, leaving 17 who were negative to a dose equivalent to over 375 mg of aspirin. Nineteen were not challenged because of contraindications.
With the exception of one patient who developed facial angioedema and two patients with > 20% drop in FEV1 (following nasal plus oral challenge) no other severe adverse events occurred. No hospitalization was required for these three patients.
Nasal inspiratory peak flow monitoring was less sensitive to obstruction caused by aspirin than was acoustic rhinometry – which should be employed when aspirin challenge is an outpatient procedure.
Conclusions
Provided patients are carefully chosen and monitored LAS challenge is suitable for ENT day case practice where respiratory physician help with asthma is available and should reduce the under-diagnosis of this condition
Large-scale and significant expression from pseudogenes in Sodalis glossinidius – a facultative bacterial endosymbiont
The majority of bacterial genomes have high coding efficiencies, but there are some genomes of intracellular bacteria that have low gene density. The genome of the endosymbiont Sodalis glossinidius contains almost 50 % pseudogenes containing mutations that putatively silence them at the genomic level. We have applied multiple ‘omic’ strategies, combining Illumina and Pacific Biosciences Single-Molecule Real-Time DNA sequencing and annotation, stranded RNA sequencing and proteome analysis to better understand the transcriptional and translational landscape of Sodalis pseudogenes, and potential mechanisms for their control. Between 53 and 74 % of the Sodalis transcriptome remains active in cell-free culture. The mean sense transcription from coding domain sequences (CDSs) is four times greater than that from pseudogenes. Comparative genomic analysis of six Illumina-sequenced Sodalis isolates from different host Glossina species shows pseudogenes make up ~40 % of the 2729 genes in the core genome, suggesting that they are stable and/or that Sodalis is a recent introduction across the genus Glossina as a facultative symbiont. These data shed further light on the importance of transcriptional and translational control in deciphering host–microbe interactions. The combination of genomics, transcriptomics and proteomics gives a multidimensional perspective for studying prokaryotic genomes with a view to elucidating evolutionary adaptation to novel environmental niches
The Practice and Potential of Renewable Energy Localisation: Results from a UK Field Trial

generation is one of the key challenges of the energy transition. We describe a UK field trial in 48 homes of an approach to this problem aimed at directly matching local supply and demand. This combined a community-based business model with social engagement and demand response technology employing both thermal and electrical energy storage. A proportion of these homes (14) were equipped with rooftop photovoltaics (PV) amounting to a total of 45 kWp; the business model enabled the remaining 34 homes to consume the electricity exported from the PV-equipped dwellings at a favourably low tariff in the context of a time-of-day tariff scheme. We report on the useful
financial return achieved by all participants, their overall experience of the trial, and the proportion of local generation consumed locally. The energy storage devices were controlled, with user oversight, to respond automatically to signals indicating the availability of low cost electricity either from the photovoltaics or the time of day grid tariff. A substantial response was observed in the resulting demand profile from these controls, less so from demand scheduling methods which required regular user configuration. Finally results are reported from a follow-up fully commercial implementation of the concept showing the viability of the business model. We conclude that the sustainability of the transition to renewable energy can be strengthened with a community-oriented approach as demonstrated in the trial that supports users through technological change and improves return on investment by matching local generation and consumption
Neuroimaging in Parkinson's disease dementia: Connecting the dots
Dementia is a common and devastating symptom of Parkinson’s disease but the anatomical substrate remains unclear. Some evidence points towards hippocampal involvement but neuroimaging abnormalities have been reported throughout the brain and are largely inconsistent across studies. Here, we test whether these disparate neuroimaging findings for Parkinson’s disease dementia localize to a common brain network. We used a literature search to identify studies reporting neuroimaging correlates of Parkinson’s dementia (11 studies, 385 patients). We restricted our search to studies of brain atrophy and hypometabolism that compared Parkinson’s patients with dementia to those without cognitive involvement. We used a standard coordinate-based activation likelihood estimation meta-analysis to assess for consistency in the neuroimaging findings. We then used a new approach, coordinate-based network mapping, to test whether neuroimaging findings localized to a common brain network. This approach uses resting-state functional connectivity from a large cohort of normative subjects (n = 1000) to identify the network of regions connected to a reported neuroimaging coordinate. Activation likelihood estimation meta-analysis failed to identify any brain regions consistently associated with Parkinson’s dementia, showing major heterogeneity across studies. In contrast, coordinate-based network mapping found that these heterogeneous neuroimaging findings localized to a specific brain network centred on the hippocampus. Next, we tested whether this network showed symptom specificity and stage specificity by performing two further analyses. We tested symptom specificity by examining studies of Parkinson’s hallucinations (9 studies, 402 patients) that are frequently co-morbid with Parkinson’s dementia. We tested for stage specificity by using studies of mild cognitive impairment in Parkinson’s disease (15 studies, 844 patients). Coordinate-based network mapping revealed that correlates of visual hallucinations fell within a network centred on bilateral lateral geniculate nucleus and correlates of mild cognitive impairment in Parkinson’s disease fell within a network centred on posterior default mode network. In both cases, the identified networks were distinct from the hippocampal network of Parkinson’s dementia. Our results link heterogeneous neuroimaging findings in Parkinson’s dementia to a common network centred on the hippocampus. This finding was symptom and stage-specific, with implications for understanding Parkinson’s dementia and heterogeneity of neuroimaging findings in general
A classification scheme for annotating speech acts in a business email corpus
This paper reports on the process of manual annotation of speech acts in a corpus
of business emails, in the context of the PROBE project (PRagmatics of
Business English). The project aims to bring together corpus, computational,
and theoretical linguistics by drawing on the insights made available by the
annotated corpus. The corpus data sheds light on the linguistic and discourse
structures of speech act use in business email communication. This enhanced
linguistic description can be compared to theoretical linguistic representations
of speech act categories to assess how well traditional distinctions relate to
real-world, naturally occurring data. From a computational perspective, the
annotated data is required for the development of an automated speech act tagging
tool. Central to this research is the creation of a high quality, manually
annotated speech act corpus, using an easily interpretable classification
scheme. We discuss the scheme chosen for the project and the training guidelines
given to the annotators, and describe the main challenges identified by the
annotators
Characterization of Pro-Inflammatory Flagellin Proteins Produced by Lactobacillus ruminis and Related Motile Lactobacilli
peer-reviewedLactobacillus ruminis is one of at least twelve motile but poorly characterized species found in the genus Lactobacillus. Of these, only L. ruminis has been isolated from mammals, and this species may be considered as an autochthonous member of the gastrointestinal microbiota of humans, pigs and cows. Nine L. ruminis strains were investigated here to elucidate the biochemistry and genetics of Lactobacillus motility. Six strains isolated from humans were non-motile while three bovine isolates were motile. A complete set of flagellum biogenesis genes was annotated in the sequenced genomes of two strains, ATCC25644 (human isolate) and ATCC27782 (bovine isolate), but only the latter strain produced flagella. Comparison of the L. ruminis and L. mali DSM20444T motility loci showed that their genetic content and gene-order were broadly similar, although the L. mali motility locus was interrupted by an 11.8 Kb region encoding rhamnose utilization genes that is absent from the L. ruminis motility locus. Phylogenetic analysis of 39 motile bacteria indicated that Lactobacillus motility genes were most closely related to those of motile carnobacteria and enterococci. Transcriptome analysis revealed that motility genes were transcribed at a significantly higher level in motile L. ruminis ATCC27782 than in non-motile ATCC25644. Flagellin proteins were isolated from L. ruminis ATCC27782 and from three other Lactobacillus species, while recombinant flagellin of aflagellate L. ruminis ATCC25644 was expressed and purified from E. coli. These native and recombinant Lactobacillus flagellins, and also flagellate L. ruminis cells, triggered interleukin-8 production in cultured human intestinal epithelial cells in a manner suppressed by short interfering RNA directed against Toll-Like Receptor 5. This study provides genetic, transcriptomic, phylogenetic and immunological insights into the trait of flagellum-mediated motility in the lactobacilli.This work was supported by a Principal Investigator Award (07/IN.1/B1780) from Science Foundation Ireland to PWOT. Making Legacy Thermal Storage Heating fit for the Smart Grid

The Publisher's final version can be found by following the DOI link.Thermal storage heaters, charged using overnight off-peak electricity, have been used for domestic space heating in the UK and other countries since the 1980s. However, they have always been difficult for consumers to manage efficiently and, with the advent of a high proportion of renewables in the electricity generation mix, the time of day when they are charged needs to be more flexible. There is also a need to reduce peaks in the demand profile to allow distribution networks to support new sources of demand such as electric vehicles. We describe a trial of a smart control system that was retrofitted to a group of six dwellings with this form of heating, with the objectives of providing more convenient and efficient control for the users while varying the times at which charging is performed, to flatten the profile of demand and make use of locally-generated renewable electricity. The trial also employs a commercially-realistic combination of a static time-of-day tariff with a real time tariff dependent on local generation, to provide consumers with the opportunity and incentive to reduce their costs by varying times of use of appliances. Results from operation over the 2015-16 heating season indicate that the objectives are largely achieved. It is estimated that on an annualised and weather-adjusted basis most of the users have consumed less electricity than before intervention and their costs are less on the trial tariffs. Critical factors for success of this form of system are identified, particularly the need to facilitate hands-on control of heating by thrifty users and the importance of an effective and sustained user engagement programme when introducing the technology, to ensure users gain confidence through a readily-accessible source of support and advice
