Differential effects of dietary supplements on metabolomic profile of smokers versus non-smokers.
BackgroundCigarette smoking is well-known to associate with accelerated skin aging as well as cardiovascular disease and lung cancer, in large part due to oxidative stress. Because metabolites are downstream of genetic variation, as well as transcriptional changes and post-translational modifications of proteins, they are the most proximal reporters of disease states or reversal of disease states.MethodsIn this study, we explore the potential effects of commonly available oral supplements (containing antioxidants, vitamins and omega-3 fatty acids) on the metabolomes of smokers (n = 11) compared to non-smokers (n = 17). At baseline and after 12 weeks of supplementation, metabolomic analysis was performed on serum by liquid and gas chromatography with mass spectroscopy (LC-MS and GC-MS). Furthermore, clinical parameters of skin aging, including cutometry as assessed by three dermatologist raters blinded to subjects' age and smoking status, were measured.ResultsLong-chain fatty acids, including palmitate and oleate, decreased in smokers by 0.76-fold (P = 0.0045) and 0.72-fold (P = 0.0112), respectively. These changes were not observed in non-smokers. Furthermore, age and smoking status showed increased glow (P = 0.004) and a decrease in fine wrinkling (P = 0.038). Cutometry showed an increase in skin elasticity in smokers (P = 0.049) but not in non-smokers. Complexion analysis software (VISIA) revealed decreases in the number of ultraviolet spots (P = 0.031), and cutometry showed increased elasticity (P = 0.05) in smokers but not non-smokers.ConclusionsAdditional future work may shed light on the specific mechanisms by which long-chain fatty acids can lead to increased glow, improved elasticity measures and decreased fine wrinkling in smokers' skin. Our study provides a novel, medicine-focused application of available metabolomic technology to identify changes in sera of human subjects with oxidative stress, and suggests that oral supplementation (in particular, commonly available antioxidants, vitamins and omega-3 fatty acids) affects these individuals in a way that is unique (compared to non-smokers) on a broad level

How much exposure to English is necessary for a bilingual toddler to perform like a monolingual peer in language tests?
Background
Bilingual children are under-referred due to an ostensible expectation that they lag behind their monolingual peers in their English acquisition. The recommendations of the Royal College of Speech and Language Therapists (RCSLT) state that bilingual children should be assessed in both the languages known by the children. However, despite these recommendations, a majority of speech and language professionals report that they assess bilingual children only in English as bilingual children come from a wide array of language backgrounds and standardized language measures are not available for the majority of these. Moreover, even when such measures do exist, they are not tailored for bilingual children.
Aims
It was asked whether a cut-off exists in the proportion of exposure to English at which one should expect a bilingual toddler to perform as well as a monolingual on a test standardized for monolingual English-speaking children.
Methods & Procedures
Thirty-five bilingual 2;6-year-olds exposed to British English plus an additional language and 36 British monolingual toddlers were assessed on the auditory component of the Preschool Language Scale, British Picture Vocabulary Scale and an object-naming measure. All parents completed the Oxford Communicative Development Inventory (Oxford CDI) and an exposure questionnaire that assessed the proportion of English in the language input. Where the CDI existed in the bilingual's additional language, these data were also collected.
Outcomes & Results
Hierarchical regression analyses found the proportion of exposure to English to be the main predictor of the performance of bilingual toddlers. Bilingual toddlers who received 60% exposure to English or more performed like their monolingual peers on all measures. K-means cluster analyses and Levene variance tests confirmed the estimated English exposure cut-off at 60% for all language measures. Finally, for one additional language for which we had multiple participants, additional language CDI production scores were significantly inversely related to the amount of exposure to English.
Conclusions & Implications
Typically developing 2;6-year-olds who are bilingual in English and an additional language and who hear English 60% of the time or more, perform equivalently to their typically developing monolingual peers
Lung cancer biomarker testing : perspective from Europe
A questionnaire on biomarker testing previously used in central European countries was extended and distributed in Western and Central European countries to the pathologists participating at the Pulmonary Pathology Society meeting 26-28 June 2019 in Dubrovnik, Croatia. Each country was represented by one responder. For recent biomarkers the availability and reimbursement of diagnoses of molecular alterations in non-small cell lung carcinoma varies widely between different, also western European, countries. Reimbursement of such assessments varies widely between unavailability and payments by the health care system or even pharmaceutical companies. The support for testing from alternative sources, such as the pharmaceutical industry, is no doubt partly compensating for the lack of public health system support, but it is not a viable or long-term solution. Ideally, a structured access to testing and reimbursement should be the aim in order to provide patients with appropriate therapeutic options. As biomarker enabled therapies deliver a 50% better probability of outcome success, improved and unbiased reimbursement remains a major challenge for the future.Peer reviewe
USArray Imaging of Continental Crust in the Conterminous United States
The thickness and bulk composition of continental crust provide important constraints on the evolution and dynamics of continents. Crustal mineralogy and thickness both may influence gravity anomalies, topographic elevation, and lithospheric strength, but prior to the inception of EarthScopeâs USArray, seismic measurements of crustal thickness and properties useful for inferring lithology are sparse. Here we improve upon a previously published methodology for joint inversion of Bouguer gravity anomalies and seismic receiver functions by using parameter space stacking of cross correlations of modeled synthetic and observed receiver functions instead of standard H-Îș amplitude stacking. The new method is applied to estimation of thickness and bulk seismic velocity ratio, vP/vS, of continental crust in the conterminous United States using USArray and other broadband network data. Crustal thickness variations are reasonably consistent with those found in other studies and show interesting relationships to the history of North American continental formation. Seismic velocity ratios derived in this study are more robust than in other analyses and hint at large-scale variations in composition of continental crust. To interpret the results, we model the pressure-/temperature-dependent thermodynamics of mineral formation for various crustal chemistries, with and without volatile constituents. Our results suggest that hydration lowers bulk crustal vP/vS and density and releases heat in the shallow crust but absorbs heat in the lowermost crust (where plagioclase breaks down to pyroxene and garnet resulting in higher seismic velocity). Hence, vP/vS variations may provide a useful proxy for hydration state in the crust

Intermediate-severity wind disturbance in mature temperate forests: legacy structure, carbon storage, and stand dynamics
Wind is one of the most important natural disturbances influencing forest structure, ecosystem function, and successional processes worldwide. This study quantifies the stand-scale effects of intermediate-severity windstorms (i.e., blowdowns) on (1) live and dead legacy structure, (2) aboveground carbon storage, and (3) tree regeneration and associated stand dynamics at four mature, mixed hardwoodâconifer forest sites in the northeastern United States. We compare wind-affected forests to adjacent reference conditions (i.e., undisturbed portions of the same stands) 0â8 yr post-blowdown using parametric (ANOVA) and nonparametric (NMS ordination) analyses. We supplement inventory plots and downed coarse woody detritus (DCWD) transects with hemispherical photography to capture spatial variation in the light environment. Although recent blowdowns transferred a substantial proportion of live overstory trees to DCWD, residual live tree basal area was high (19â59% of reference areas). On average, the initial post-blowdown ratio of DCWD carbon to standing live tree carbon was 2.72 in blowdown stands and 0.18 in reference stands, indicating a large carbon transfer from live to dead pools. Despite these dramatic changes, structural complexity remained high in blowdown areas, as indicated by the size and species distributions of overstory trees, abundance of sound and rotten downed wood, spatial patterns of light availability, and variability of understory vegetation. Furthermore, tree species composition was similar between blowdown and reference areas at each site, with generally shade-tolerant species dominating across multiple canopy strata. Community response to intermediate-severity blowdown at these sites suggests a dynamic in which disturbance maintains late-successional species composition rather than providing a regeneration opportunity for shade-intolerant, pioneer species. Our findings suggest that intermediate-severity wind disturbances can contribute to stand-scale structural complexity as well as development toward late-successional species composition, at least when shade-tolerant regeneration is present pre-blowdown. Advance regeneration thus enhances structural and compositional resilience to this type of disturbance. A prospective study of chemotherapy-induced febrile neutropenia in the South West London Cancer Network. Interpretation of study results in light of NCAG/NCEPOD findings
BACKGROUND: Chemotherapy-induced febrile neutropenia is a medical emergency complicating the treatment of many cancer patients. It is associated with considerable morbidity and mortality, as well as impacting on healthcare resources. METHODS: A prospective study of all cases of chemotherapy-induced febrile neutropenia in the South West London Cancer Network was conducted over a 4-month period. Factors including demographics, treatment history, management of febrile neutropenia and outcome were recorded.
