Meeting the challenge of health system transformation in European countries
\ua9 The Author(s) 2022. Transforming health systems is a complex, messy business with no quick or simple solutions. Countries struggle to make it happen with policy failure often the result. Reporting on a World Health Organization Europe project aimed at understanding how health system transformation can succeed, the paper draws on three European country case studies to offer insights and lessons for policymakers elsewhere engaged in similar efforts. Critical to success in implementation is the adoption of a receptive context for change. Building on the policy capacity literature, it emphasizes the importance of environmental pressure, the quality and coherence of policy, leadership style, supportive organizational culture, and managerial-health profession relations
Globally discordant Isocrinida (Crinoidea) migration confirms asynchronous Marine Mesozoic Revolution
The Marine Mesozoic Revolution (MMR, starting ~200 million years ago) changed the ecological structure of sea floor communities due to increased predation pressure. It was
thought to have caused the migration of less mobile invertebrates, such as stalked isocrinid crinoids, into deeper marine environments by the end of the Mesozoic. Combined MR direct thrombus imaging and non-contrast magnetic resonance venography reveal the evolution of deep vein thrombosis: a feasibility study.
OBJECTIVES: Lower limb deep venous thrombosis (DVT) is a common condition with high morbidity and mortality. The aim of the study was to investigate the temporal evolution of the acute thrombus by magnetic resonance imaging (MRI) and its relationship to venous recanalization in patients with recurrent DVTs. METHODS: Thirteen patients with newly diagnosed lower limb DVTs underwent MRI with non-contrast MR venography (NC-MRV) and MR direct thrombus imaging (MR-DTI), an inversion-recovery water-selective fast gradient-echo acquisition. Imaging was performed within 7Â days of the acute thrombotic event, then at 3 and 6Â months. RESULTS: By 3Â months from the thrombotic event a third of the thrombi had resolved and by 6Â months about half of the cases had resolved on the basis of vein recanalisation using NC-MRV. On the initial MR-DTI acute thrombus was clearly depicted by hyperintense signal, while the remaining thrombi were predominantly low signal at 3 and 6Â months. Some residual thrombi contained small and fragmented persisting hyperintense areas at 3Â months, clearing almost completely by 6Â months. CONCLUSIONS: Our study suggests that synergistic venous assessment with combined NC-MRV and MR-DTI is able to distinguish acute venous thrombosis from the established (old) or evolving DVT detected by ultrasound. High Energy Gamma-Ray Emission From Blazars: EGRET Observations
We will present a summary of the observations of blazars by the Energetic
Gamma Ray Experiment Telescope (EGRET) on the Compton Gamma Ray Observatory
(CGRO). EGRET has detected high energy gamma-ray emission at energies greater
than 100 MeV from more that 50 blazars. These sources show inferred isotropic
luminosities as large as ergs s. One of the most
remarkable characteristics of the EGRET observations is that the gamma-ray
luminosity often dominates the bolometric power of the blazar. A few of the
blazars are seen to exhibit variability on very short time-scales of one day or
less. The combination of high luminosities and time variations seen in the
gamma-ray data indicate that gamma-rays are an important component of the
relativistic jet thought to characterize blazars. Currently most models for
blazars involve a beaming scenario. In leptonic models, where electrons are the
primary accelerated particles, gamma-ray emission is believed to be due to
inverse Compton scattering of low energy photons, although opinions differ as
to the source of the soft photons. Hardronic models involve secondary
production or photomeson production followed by pair cascades, and predict

Expression of a beta-adrenergic receptor kinase 1 inhibitor prevents the development of myocardial failure in gene-targeted mice.
Heart failure is accompanied by severely impaired beta-adrenergic receptor (betaAR) function, which includes loss of betaAR density and functional uncoupling of remaining receptors. An important mechanism for the rapid desensitization of betaAR function is agonist-stimulated receptor phosphorylation by the betaAR kinase (betaARK1), an enzyme known to be elevated in failing human heart tissue. To investigate whether alterations in betaAR function contribute to the development of myocardial failure, transgenic mice with cardiac-restricted overexpression of either a peptide inhibitor of betaARK1 or the beta2AR were mated into a genetic model of murine heart failure (MLP-/-). In vivo cardiac function was assessed by echocardiography and cardiac catheterization. Both MLP-/- and MLP-/-/beta2AR mice had enlarged left ventricular (LV) chambers with significantly reduced fractional shortening and mean velocity of circumferential fiber shortening. In contrast, MLP-/-/betaARKct mice had normal LV chamber size and function. Basal LV contractility in the MLP-/-/betaARKct mice, as measured by LV dP/dtmax, was increased significantly compared with the MLP-/- mice but less than controls. Importantly, heightened betaAR desensitization in the MLP-/- mice, measured in vivo (responsiveness to isoproterenol) and in vitro (isoproterenol-stimulated membrane adenylyl cyclase activity), was completely reversed with overexpression of the betaARK1 inhibitor. We report here the striking finding that overexpression of this inhibitor prevents the development of cardiomyopathy in this murine model of heart failure. These findings implicate abnormal betaAR-G protein coupling in the pathogenesis of the failing heart and point the way toward development of agents to inhibit betaARK1 as a novel mode of therapy
My joint pain, a web-based resource, effects on education and quality of care at 24 months
Objective: To evaluate the effects of the updated version of an evidence-based osteoarthritis (OA) resource and consumer hub, 'My Joint Pain' website, on health education and quality of care over 12âmonths.Methods: Using a classic quasi-experimental design, participants with symptomatic hip or knee OA were recruited across Australia to evaluate the 'My Joint Pain' website, compared to a control group of non-users from 12 to 24âmonths. Outcome measures included the Health Education Impact Questionnaire (HEIQ) and the OA Quality Indicator (OAQI) questionnaire. The changes from 12 to 24âmonths in the HEIQ were evaluated using a generalised linear model. The differences between users and non-users in the OAQI were evaluated using a chi-square test.Results: A total of 277 eligible participants with symptomatic hip or knee OA were recruited at baseline, and 122 participants completed the 24-month surveys (users: nâ=â35, non-users: nâ=â87). There was no significant difference between users and non-users for the HEIQ scores at 24âmonths after adjustments for age, sex and body mass index (BMI). Users had higher emotional distress scores than non-users in univariable analysis. When compared with non-users in the OAQI, users showed favourable changes in receiving information about "self-management" and "acetaminophen" and "non-steroidal anti-inflammatory drugs (NSAIDs)" from 12 to 24âmonths.Conclusion: The evaluation of the updated 'My Joint Pain' website didn't find significant improvements in terms of health education, but it may help delivering useful information about self-management and appropriate use of pharmacological treatments. More strategies are needed to facilitate the uptake of evidence-based self-management and education online resources for OA consumers
Needles in the Haystack: Identifying Individuals Present in Pooled Genomic Data
Recent publications have described and applied a novel metric that quantifies the
genetic distance of an individual with respect to two population samples, and
have suggested that the metric makes it possible to infer the presence of an
individual of known genotype in a sample for which only the marginal allele
frequencies are known. However, the assumptions, limitations, and utility of
this metric remained incompletely characterized. Here we present empirical tests
of the method using publicly accessible genotypes, as well as analytical
investigations of the method's strengths and limitations. The results
reveal that the null distribution is sensitive to the underlying assumptions,
making it difficult to accurately calibrate thresholds for classifying an
individual as a member of the population samples. As a result, the
false-positive rates obtained in practice are considerably higher than
previously believed. However, despite the metric's inadequacies for
identifying the presence of an individual in a sample, our results suggest
potential avenues for future research on tuning this method to problems of
ancestry inference or disease prediction. By revealing both the strengths and
limitations of the proposed method, we hope to elucidate situations in which
this distance metric may be used in an appropriate manner. We also discuss the
implications of our findings in forensics applications and in the protection of
GWAS participant privacy
