    Modulation of galactic protons in the heliosphere during the unusual solar minimum of 2006 to 2009

    The last solar minimum activity period, and the consequent minimum modulation conditions for cosmic rays, was unusual. The highest levels of galactic protons were recorded at Earth in late 2009 in contrast to expectations. Proton spectra observed for 2006 to 2009 from the PAMELA cosmic ray detector on-board the Resurs-DK1 satellite are presented together with the solutions of a comprehensive numerical model for the solar modulation of cosmic rays. The model is used to determine what mechanisms were mainly responsible for the modulation of protons during this period, and why the observed spectrum for 2009 was the highest ever recorded. From mid-2006 until December 2009 we find that the spectra became significantly softer because increasingly more low energy protons had reached Earth. To simulate this effect, the rigidity dependence of the diffusion coefficients had to decrease significantly below ~3 GeV. The modulation minimum period of 2009 can thus be described as relatively more "diffusion dominated" than previous solar minima. However, we illustrate that drifts still had played a significant role but that the observable modulation effects were not as well correlated with the waviness of the heliospheric current sheet as before. Protons still experienced global gradient and curvature drifts as the heliospheric magnetic field had decreased significantly until the end of 2009, in contrast to the moderate decreases observed during previous minimum periods. We conclude that all modulation processes contributed to the observed increases in the proton spectra for this period, exhibiting an intriguing interplay of these major mechanisms

    We use the dynamical gluon configurations provided by the MILC collaboration in a study of the charmonium spectrum and psi leptonic width.     Geant4 Muon Digitization in the ATHENA Framework

    The aim of this note is to describe the Muon Digitization software packages, completely re-written to run in the Athena framework and to interface with the Geant4 Muon Spectrometer simulation. The Muon Digitization is the simulation of the Raw Data Objects (RDO), or the electronic output, of the Muon Spectrometer. It consists of two steps: in the first step, the output of the detector simulation, the Muon Hits, is converted to Muon Digits, namely intermediate objects that can be fed into the reconstruction. In the second step, the Muon Digits are converted into RDO, the transient representation of raw data byte stream. We describe the detailed implementation of the first step of the Muon Digitization, where the detector simulation output is âワdigitizedâ into Muon Digits. We describe the fundamentals of the Muon Digitization algorithms, outlining their global structure and the infrastructure for the simulation of piled-up events.     Effect of Low Molecular Weight Heparins (LMWHs) on antiphospholipid Antibodies (aPL)-mediated inhibition of endometrial angiogenesis

    Antiphospholipid syndrome (APS) is an autoimmune disorder characterized by vascular thrombosis and/or pregnancy morbidity in the presence of circulating antiphospholipid antibodies (aPL). Different pathogenic mechanisms for aPL-mediated pregnancy failure have been proposed. In particular a direct effect of aPL on both maternal and fetal side of the placental tissue has been reported, since their reactivity with \u3b22-glycoprotein I (\u3b22GPI) makes them adhere to trophoblast and human endometrial endothelial cell (HEEC) membranes. \u3b22GPI can be recognized by aPL that, once bound, interfere with both trophoblast functions and with the HEEC differentiation.APS patients can be successfully treated with Low Molecular Weight Heparin (LMWH). Recent reports suggest that LMWH acts through mechanisms alternative to its well known anticoagulant effect, because of its ability to bind \u3b22GPI. In our previous studies, we showed that LMWH is able to reduce the aPL binding to trophoblasts and restore cell invasiveness and differentiation. So far, however, no study has described its effects on endometrial angiogenesis.The aim of our research was to evaluate whether two LMWHs, tinzaparin and enoxaparin, have an effect on the aPL-inhibited endometrial angiogenesis. This prompted us to investigate: (i) in vitro HEEC angiogenesis through a Matrigel assay; (ii) VEGF secretion by ELISA; (iii) matrix metalloproteinase-2 (MMP-2) activity by gelatin zymography; (iv) Nuclear Factor-\u3baB (NF-\u3baB) DNA binding activity by colorimetric assay; (v) STAT-3 activation by a sandwich-ELISA kit. Furthermore, using an in vivo murine model we investigated the LMWHs effects on angiogenesis.We demonstrated that the addition of LMWHs prevents aPL-inhibited HEEC angiogenesis, both in vitro and in vivo, and is able to restore the aPL inhibited NF-\u3baB and/or STAT-3 activity, the VEGF secretion and the MMPs activity.The demonstration of a beneficial role for LMWHs on the aPL-inhibited HEEC angiogenesis might provide additional mechanisms whereby this treatment protects early pregnancy in AP

    Placentation is an immunological compromise where maternal immune system cells and trophoblastic cells interact to reach an equilibrium condition. Although the cross talk between the two systems is complex and not completely understood, Human Leukocyte Antigen G (HLA-G), expressed on trophoblastic cell surfaces, seems to be one of the main molecules involved in the modulation of both local and systemic maternal immune response. The prevalence of recurrent pregnancy loss (RPL), probably underestimated, is 5% of all women who achieve pregnancy, and about 40–60% percent of RPL cases are unexplained. There is an immunological analogy between allograft rejection and miscarriage, and the purpose of this review is to describe how the HLA-G pathway alterations are involved in disrupting the immunologic balance and in increasing the risk of recurrent pregnancy loss

    Infodemiological patterns in searching medication errors: Relationship with risk management and shift work

    INTRODUCTION: Western world health care systems have been trying to improve their efficiency and effectiveness in order to respond properly to population aging and non-communicable diseases epidemic. Treatment of the elderly population is becoming complex due to the high number of prescribed drugs because of multimorbidity. Errors in drugs administration in different health care related settings are an actual important issue due to different causes. Aim of this observational study is to measure the online interest in seeking medication errors information related to risk management and shift work. MATERIALS AND METHODS: We investigated Google Trends® for popular search relating to medication errors, risk management and shift work. Relative search volumes (RSVs) were evaluated from 2008 to 2018. A comparison between RSV curves related to medication errors, risk management and shift work was carried out. Then, we compared the world to Italian search. RESULTS: RSVs were persistently higher for risk management than for medication errors (mean RSVs 069 vs. 48%) and RSVs were stably higher for medication errors than shift work (mean RSVs 48 vs. 22%). In Italy, RSVs were much lower compared to the rest of the world, and RSVs for medication errors during the study period were negligible. Mean RSVs for risk management and shift work were 3 and 25%, respectively. RSVs related to medication errors and clinical risk management were correlated (r=0.520, p<0.0001). CONCLUSIONS: Google Trends® search query volumes related to medication errors, risk management and shift work are different. RSVs for risk management are higher, and they are correlated with medication errors. Also, shift work search appears to be lower. These results should be interpreted in order to correctly evaluate how to decrease the number of medication errors in different health care related setting

    Improving sleep quality in cancer patients. a literature review on non-pharmacologic interventions

    Objective: Disrupted sleep-in cancer patients is due to several factors. Pre-existing sleep disorders, mental status in cancer, or side effects of cancer treatment are all potential predictors of sleep disturbances in this vulnerable population. This review aims to evaluate and synthesise non-pharmaco-logic interventions for improving sleep quality and the associated benefits in cancer patients. Materials and Methods: A literature review was performed according to PRISMA guidelines. PubMed, Google Scholar, MEDLINE, CINAHL, and Embase databases were interrogated, selected the article with cancer, neoplasm, oncology, sleep-wake disorders, sleep disturbance, sleep problem, strategies, treat-ment, and intervention such as keywords. No time and geographic restrictions but paediatrics, children, and interventions unrelated to sleep improvement in cancer were excluded. Results: Overall, twenty-nine articles were included in the review after selection. The included studies analysed different types of cancer, like breast, lung, and prostate cancer and acute leukaemia. Most of the data gathered from the relevant research suggest that nonpharmacologic interventions significantly improved cancer patients’ sleep quality. Conclusions: Cancer survival rates are increasing; researchers and healthcare professionals should aim not just at survival but also to allow cancer patients just comfortably to live, considering the best quality of life possible. Nurses have a crucial role in the care of this patient population. Nursing implies an intimate relationship with patients and educating the patient to improve their mental and physical condition through non-pharmacological approaches, which should be considered a specific competence
