Factors Associated with Revision Surgery after Internal Fixation of Hip Fractures
Background: Femoral neck fractures are associated with high rates of revision surgery after management with internal fixation. Using data from the Fixation using Alternative Implants for the Treatment of Hip fractures (FAITH) trial evaluating methods of internal fixation in patients with femoral neck fractures, we investigated associations between baseline and surgical factors and the need for revision surgery to promote healing, relieve pain, treat infection or improve function over 24 months postsurgery. Additionally, we investigated factors associated with (1) hardware removal and (2) implant exchange from cancellous screws (CS) or sliding hip screw (SHS) to total hip arthroplasty, hemiarthroplasty, or another internal fixation device. Methods: We identified 15 potential factors a priori that may be associated with revision surgery, 7 with hardware removal, and 14 with implant exchange. We used multivariable Cox proportional hazards analyses in our investigation. On résout théoriquement et numériquement le problème des démarrages d'écoulements de fluides à comportement netwtonien ou pseudoplastique. Les gradients de pression à l'origine des écoulements sont soit constants soit fonction du temps. Une méthode expérimentale d'étude est aussi proposée. Les résultats présentés montrent l'influence de divers paramètres rhéologiques, hydrodynamiques ou géométriques sur la durée de tels régimes transitoires d'écoulement
Ecoulements pulsés de fluides inélastiques en conduite tronconique ou déformable
The aim of the present work consists in the study of the pulsating flows of inelastic fluids in non rigid or slowly varying axisymmetric tubes. Four methods of investigation were employed : • an analytical method, • a perturbation method, • a numerical method, • an experimental method based on laser anemometry applied to aqueous solutions of high polymers. The relative influences of rheological, geometrical and hydrodynamical parameters have been analysed and compared. This study can be considered as a step in the modelling of flow in blood vessels.L'objet du présent travail consiste à mettre en évidence, dans un écoulement non permanent, certains effets caractéristiques dus au comportement inélastique du fluide, à la géométrie tronconique ou à la déformabilité de la conduite. Les déterminations de répartition de pression et de profils de vitesses sont faites à l'aide de quatre méthodes : une méthode de perturbation, une méthode analytique, une méthode numérique aux différences finies et une méthode expérimentale mettant en œuvre la vélocimétrie à laser. Les résultats obtenus peuvent permettre une meilleure connaissance de certains aspects d'écoulements sanguins

The effects of rheological, hydrodynamic and geometrical parameters are examined by solving the Navier-Stokes equations by using different mathematical techniques with regard to start-up flows. An experimental method is therefore proposed. The flows are subjected to a constant pressure gradient or to an unsteady one. The fluids studied exhibit either newtonian or pseudoplastic behaviour.
