La prise en charge conventionnelle de l’obésité
morbide, définie par un IMC > 40 kg/m2, ou > 35 kg/m2
avec co-morbidités, est inefficace. Seule la chirurgie bariatrique
paraît capable de maintenir la perte de poids et de
diminuer la mortalité à long terme. Parmi les traitements
endoscopiques, le ballon intragastrique (BG), qu’il s’agisse
du Bioenterics Intragastric Balloon ou du Ballon Héliosphère,
est la seule méthode actuellement répandue. Selon
l’ANAES, les indications du BG sont constituées par l’obésité
morbide. Ces indications sont superposables Ă celles
de la chirurgie bariatrique pour laquelle, cependant, il est
souligné que l’obésité doit être stable (ou s’aggraver)
depuis 5 ans avant d’envisager une intervention. Le BG ne
peut être utilisé que temporairement (6 mois) soit comme
traitement, soit dans la préparation à la chirurgie, bariatrique
ou autre, de malades atteints d’obésité morbide ou
d’obésité extrême (« super-obèses » avec IMC > 50 ou
60 kg/m2)
ENDOSCOPIC Nd YAG LASER : PALLIATIVE THERAPY FOR ESOPHAGO-CARDIAL CANCER. Concerning 30 patients treated between July 1984 and January 1987
26 hommes et 4 femmes, âgés de 34 à 91 ans (moyenne : 69) ont été traités : 26 cancers de l'oesophage (16 malpighiens ; 8 cylindriques ; 1 indifférencié ; 1 carcinosarcome) ; 4 du cardia (3 adénocarcinomes ; 1 linite plastique). Les cancers de l'oesophage siégeaient aux tiers supérieur : 4, moyen : 7, inférieur : 9, supérieur et moyen : 1, moyen et inférieur : 4, sur tout l'oesophage : 1. La hauteur des lésions allait de 1,5 à 16 cm (moyenne : 7). 11 malades avaient une sténose infranchissable. 3 malades avaient une récidive post-opératoire. 3 avaient une tumeur inextirpable à l'intervention. Les 24 autres présentaient des contre-indications opératoires. La radiothérapie a été récusée pour contre-indications chez 17 malades. 13 ont eu une radiothérapie : 10 avant, 3 après le laser. Les indications du laser étaient : une hématémèse (1 cas), la destruction d'un petit cancer de 1,5 cm (1 cas), la dysphagie (28 cas). Les résultats immédiats ont été : une hémorragie arrêtée ; un petit cancer détruit ; dysphagie améliorée : 22/28 ; tumeur rendue franchissable : 7/11. 2 complications sont survenues : un pneumopéritoine régressif ; un pneumopéricarde fatal. A distance, les résultats ont été : 19 malades décédés 8 à 460 jours (moyenne : 105) après le début du traitement laser ; 11 vivants 14 à 637 jours (moyenne : 239) après le début du traitement, avec une très nette amélioration symptomatique.26 men and 4 women, aged between 34 and 91 years (mean : 69) were treated : 26 cancers of the oesophagus (16 squamous cell carcinoma ; 8 adenocarcinoma ; 1 undifferentiated carcinoma ; 1 carcinosarcoma) ; 4 of the gastric cardia (3 adenocarcinoma ; 1 linitis plastica). The cancers of the oesophagus occupied the upper third in 4 cases, middle : 7, lower : 9, upper and middle : 1, middle and lower : 4, the whole oesophagus : 1. Lesion length ranged from 1.5 to 16 cm (mean : 7). 11 patients had impassable stenosis. 3 patients had post-operative relapses. 3 had tumors which on operation proved inextricable. The other 24 displayed operative contra-indications. In 17 patients radiotherapy was ruled out because of contra-indications. 13 underwent radiotherapy : 10 before and 3 after laser treatment. Symptoms justifying laser therapy were : hematemesis (1 case) ; destruction of one small 1.5 cm cancer (1 case) ; dysphagia (28 cases). The immediate results were : stoppage of a hemorrhage ; a small cancer was destroyed ; dysphagia was improved in 22 cases out of 28 ; the tumor was rendered passable in 7 cases out of 11. 2 complications occured a regressive pneumoperitoneum and a fatal case of pneumopericardium. The long-term results were as follows : 19 patients died between 8 and 460 days (mean : 105) after laser treatment began ; 11 were still alive between 14 and 637 days (mean : 239) after treatment began, with a clear improvement in their symptoms

20 hommes et 6 femmes, âgés de 57 à 88 ans (moyenne : 77) ont été traités. Les cancers siègeaient au niveau du rectum : 7, d'une anastomose colorectale : 3, d'un moignon rectal : 2, de la charnière recto-sigmoïdienne : 9, du sigmoïde : 6 ; il existait une double localisation, rectale et sigmoïdienne, dans 1 cas. La hauteur des lésions allait de 2 à 8 cm (moyenne : 5). 13 malades sur 24 avaient une sténose infranchissable. 18 présentaient des contre-indications opératoires. 8 avaient une récidive post-opératoire. Les indications du laser étaient : cancer découvert fortuitement : 2, syndrome rectal : 17, subocclusion : 6, occlusion 1. Les résultats immédiats ont été : destruction du cancer asymptomatique (2/2) ; diminution ou disparition du syndrome rectal (12/17) ; levée de la subocclusion (6/6) ; levée de l'occlusion (1/1) ; sténose rendue franchissable (8/13). 3 complications sont survenues : une hémorragie régressive ; une sténose cicatricielle traitée par dilatation ; un abcès périrectal. A distance : 11 malades décédés 48 à 281 jours (moyenne : 132) apris le début du traitement (1 malade est décédé d'un accident vasculaire cérébral) ; 15 vivants 13 à 484 jours (moyenne : 210) après le début du traitement.20 men and 6 women, aged between 57 and 88 years (mean : 77) were treated. 7 of the cancers were in the rectum, 3 on a colorectal anastomosis, 2 on Hartmann stumps, 9 on the rectosigmoid juction, 6 on the sigmoid (1 case of double localization, both rectal and sigmoid). Lesion length ranged from 2 to 8 cm (mean : 5). 13 patients out of 24 had impassable stenosis. 18 patients displayed operative contra-indications . 8 other patients had post-operative relapses. The symptoms justifying laser therapy were : cancer discovered during systematic check-ups : 2 ; rectal syndrom : 17 ; subocclusion : 6 ; occlusion : 1. The immediate results were : destruction of asymptomatic cancer (2/2) ; decrease or desappearance of rectal syndrom (12/17) ; removal of the subocclusion (6/6) ; removal of the occlusion (1/1) ; stenosis rendered passable (8/13). 3 complications occured : regressive hemorrhage, cicatricial stenosis treated by dilatation and a pararectal abcess. The long-term results were as follows : 11 patients died 48 to 281 days (mean : 132) after laser therapy began (1 of these patients died from a vascular brain attack) ; 15 were still alive between 13 and 484 days (mean : 210) after therapy began
