    A COMPARATIVE EVALUATION ON THE EFFECT OF ZINC-PROBIOTIC AND PROBIOTIC THERAPY IN PAEDIATRIC ACUTE DIARRHOEA AND THE IMPACT OF COUNSELLING OF MOTHERS

    Objective: To compare the efficacy of zinc-probiotic combination therapy and probiotic therapy alone in the treatment of acute paediatric diarrhoea and to assess the knowledge, attitude and practice (KAP) of the mothers of affected children and the impact of counselling on diarrhoea management.Methods: A prospective observational comparative study was conducted from October 2014 to March 2015 in the paediatric department of Cosmopolitan Hospitals Pvt. Ltd., Thiruvananthapuram, Kerala, South India. The study was carried out on 150 patients between the ages of 3 mo to 12 y. The selected patients were divided into 2 groups of 75 members each. Initially, demographic data, details of socioeconomic status, severity, duration and frequency of diarrhoea and the presence of other associated symptoms and KAP of mothers before and after counselling was collected by using specially designed proforma. Group 1 patients received zinc and probiotic and Group 2 received probiotic only by oral administration. All children in both groups received sufficient quantity of oral rehydration solution (ORS). The requirement of other medications and adverse effects were also monitored.Results: The study revealed that, in group 1, 54.6% of patients were males and in group 2, 58.6% were females. The mean age of affected children was 5.14±3.53. In both groups, the majority of patients were from outpatient (OP) departments. In group 1, majority of patients, 49.3% were resting in upper lower economic class followed by 30.7% were lower middle class. In the case of group 2, 34.6% patients were from lower middle and 28% were from upper middle class. Other medications such as antiemetic and antipyretic were administered to 127and 110 patients respectively. Antisecretory and antibiotic were administered to 31 and 26 patients respectively. Totally 8 patients were affected with adverse drug reactions such as rashes and swelling of lips. In both groups severity of diarrhoea was high before treatment and it was changed after effective treatment with drugs. Comparing with group 2, the duration and severity of diarrhoea and other associated symptoms in group 1 patients were significantly reduced after treatment. Before counseling, the majority of mothers had very poor knowledge about the diarrhoeal disease and its management. Evaluation after counselling showed a significant improvement.Conclusion: Low socioeconomic life is a risk factor for diarrhoea. A combination of zinc and probiotic therapy is more effective than probiotic therapy alone in the treatment of acute diarrhoea and vomiting in children. The emergence and severity of the diarrhoeal disease can be reduced by effective clinical pharmacist interventions, and a great emphasis is needed in the counselling and education of mothers about this disease and its management.Keywords: Paediatric diarrhoea, Zinc, Probiotic, KAP, Counselling and educatio

    Synthesis of Nm-PHB (nanomelanin-polyhydroxy butyrate) nanocomposite film and its protective effect against biofilm-forming multi drug resistant Staphylococcus aureus

    Melanin is a dark brown ubiquitous photosynthetic pigment which have many varied and ever expanding applications in fabrication of radio-protective materials, food packaging, cosmetics and in medicine. In this study, melanin production in a Pseudomonas sp. which was isolated from the marine sponge Tetyrina citirna was optimized employing one-factor at a time experiments and characterized for chemical nature and stability. Following sonication nucleated nanomelanin (Nm) particles were formed and evaluated for antibacterial and antioxidant properties. Nanocomposite film was fabricated using combinations (% w/v) of polyhydroxy butyrate-nanomelanin (PHB: Nm) blended with 1% glycerol. The Nm was found to be spherical in shape with a diameter of 100-140 nm and showed strong antimicrobial activity against both Gram positive and Gram negative bacteria. The Nm-PHB nanocomposite film was homogeneous, smooth, without any cracks, and flexible. XRD and DSC data indicated that the film was crystalline in nature, and was thermostable up to 281.87 degrees C. This study represents the first report on the synthesis of Nm and fabrication of Nm-PHB nanocomposite film which show strong protective effect against multidrug resistant Staphyloccoccus aureus. Thus this Nm-PHB nanocomposite film may find utility as packaging material for food products by protecting the food products from oxidation and bacterial contamination

    Scaffolding School Pupils’ Scientific Argumentation with Evidence-Based Dialogue Maps

    This chapter reports pilot work investigating the potential of Evidence-based Dialogue Mapping to scaffold young teenagers’ scientific argumentation. Our research objective is to better understand pupils’ usage of dialogue maps created in Compendium to write scientific ex-planations. The participants were 20 pupils, 12-13 years old, in a summer science course for “gifted and talented” children in the UK. Through qualitative analysis of three case studies, we investigate the value of dialogue mapping as a mediating tool in the scientific reasoning process during a set of learning activities. These activities were published in an online learning envi-ronment to foster collaborative learning. Pupils mapped their discussions in pairs, shared maps via the online forum and in plenary discussions, and wrote essays based on their dialogue maps. This study draws on these multiple data sources: pupils’ maps in Compendium, writings in science and reflective comments about the uses of mapping for writing. Our analysis highlights the diversity of ways, both successful and unsuccessful, in which dialogue mapping was used by these young teenagers

    We found that 1,5-anhydroglucitol—a marker of glucose excursions—was not independently associated with subclinical cardiac damage, nor with vascular outcomes, in the ADVANCE Trial. High-sensitivity cardiac troponin T and N-terminal pro-b-type natriuretic peptide provided better prognostic information regarding vascular risk in diabetes than 1,5-anhydroglucitol

    In the interactions between particles of material with slightly different electronic levels, unusually large shifts in the pair potential can result from photoexcitation, and on subsequent electronic excitation transfer. To elicit these phenomena, it is necessary to understand the fundamental differences between a variety of optical properties deriving from dispersion interactions, and processes such as resonance energy transfer that occur under laser irradiance. This helps dispel some confusion in the recent literature. By developing and interpreting the theory at a deeper level, one can anticipate that in suitable systems, light absorption and energy transfer will be accompanied by significant displacements in interparticle separation, leading to nanoscale mechanical motion

    Social deprivation and exposure to health promotion. A study of the distribution of health promotion resources to schools in England

It is also known that area deprivation is associated with lower impact health promotion. It is less well known, however, whether deprived areas are less responsive to health promotion, or whether they are less exposed. Using data from a national, school-based campaign to promote vaccination against the human papilloma virus (HPV), the relationship between area deprivation and exposure was examined. METHODS: Taking advantage of a health promotion campaign to provide information to schools about HPV vaccination, a cross sectional study was conducted to examine the relationship between area level, social deprivation, and take-up of (i.e., exposure to) available health promotion material. The sample was 4,750 schools across England, including government maintained and independent schools. The relationship between area deprivation and exposure was examined using bi- and multivariate logistic regression. RESULTS: It was found that schools in the least deprived quintile had 1.32 times the odds of requesting health promotion materials than schools in the most deprived areas (p = .01). This effect was independent of the school size, the type of school, and the geographic region. Conclusion The relationship between area deprivation and the impact of health promotion may be due, at least in part, to differential levels of exposure. The study was limited in scope, pointing to the need for more research, but also points to potentially important policy implications

    The impact of diabetes on multiple avoidable admissions: a cross-sectional study

    Background Multiple admissions for ambulatory care sensitive conditions (ACSC) are responsible for an important proportion of health care expenditures. Diabetes is one of the conditions consensually classified as an ACSC being considered a major public health concern. The aim of this study was to analyse the impact of diabetes on the occurrence of multiple admissions for ACSC. Methods We analysed inpatient data of all public Portuguese NHS hospitals from 2013 to 2015 on multiple admissions for ACSC among adults aged 18 or older. Multiple ACSC users were identified if they had two or more admissions for any ACSC during the period of analysis. Two logistic regression models were computed. A baseline model where a logistic regression was performed to assess the association between multiple admissions and the presence of diabetes, adjusting for age and sex. A full model to test if diabetes had no constant association with multiple admissions by any ACSC across age groups. Results Among 301,334 ACSC admissions, 144,209 (47.9%) were classified as multiple admissions and from those, 59,436 had diabetes diagnosis, which corresponded to 23,692 patients. Patients with diabetes were 1.49 times (p < 0,001) more likely to be admitted multiple times for any ACSC than patients without diabetes. Younger adults with diabetes (18–39 years old) were more likely to become multiple users. Conclusion Diabetes increases the risk of multiple admissions for ACSC, especially in younger adults. Diabetes presence is associated with a higher resource utilization, which highlights the need for the implementation of adequate management of chronic diseases policies.NOVASaudeinfo:eu-repo/semantics/publishedVersio
