    Factors associated with first- versus second generation long-acting antipsychotics prescribed un-der ordinary clinical practice in Italy.

    Background For many years, long-acting intramuscular (LAI) antipsychotics have been prescribed predominantly to chronic and severe patients, as a last resort when other treatments failed. Recently, a broader and earlier use of LAIs, particularly second-generation LAIs, has been emphasized. To date, few studies attempted to frame how this change in prescribing took place in real-world practice. Therefore, this study aimed to describe the clinical features of patients prescribed with LAIs, and to explore possible prescribing differences between first- and second-generations LAIs under ordinary clinical practice in Italy. Methods The STAR Network \u201cDepot\u201d Study is an observational, longitudinal, multicenter study involving 35 centers in Italy. In the cross-sectional phase, patients prescribed with LAIs were consecutively recruited and assessed over a period of 12 months. Descriptive statistics and multivariable logistic regression analyses were employed. Results Of the 451 recruited patients, 61% were males. The level of social and working functioning was heterogeneous, as was the severity of disease. Seventy-two per cent of the patients had a diagnosis of the schizophrenia spectrum. Seventy per cent were prescribed with second-generation antipsychotic (SGA) LAIs (mostly paliperidone, aripiprazole and risperidone). Compared to first-generation antipsychotic (FGA) LAIs, patients prescribed with SGA LAIs were more often younger; employed; with a diagnosis of the schizophrenia spectrum or bipolar disorder; with higher levels of affective symptoms; with fewer LAI prescriptions in the past. Discussion LAIs' prescribing practices appear to be more flexible as compared to the past, although this change is mostly restricted to SGA LAI

    Effects of Once-Weekly Exenatide on Cardiovascular Outcomes in Type 2 Diabetes.

    Abstract BACKGROUND: The cardiovascular effects of adding once-weekly treatment with exenatide to usual care in patients with type 2 diabetes are unknown. METHODS: We randomly assigned patients with type 2 diabetes, with or without previous cardiovascular disease, to receive subcutaneous injections of extended-release exenatide at a dose of 2 mg or matching placebo once weekly. The primary composite outcome was the first occurrence of death from cardiovascular causes, nonfatal myocardial infarction, or nonfatal stroke. The coprimary hypotheses were that exenatide, administered once weekly, would be noninferior to placebo with respect to safety and superior to placebo with respect to efficacy. RESULTS: In all, 14,752 patients (of whom 10,782 [73.1%] had previous cardiovascular disease) were followed for a median of 3.2 years (interquartile range, 2.2 to 4.4). A primary composite outcome event occurred in 839 of 7356 patients (11.4%; 3.7 events per 100 person-years) in the exenatide group and in 905 of 7396 patients (12.2%; 4.0 events per 100 person-years) in the placebo group (hazard ratio, 0.91; 95% confidence interval [CI], 0.83 to 1.00), with the intention-to-treat analysis indicating that exenatide, administered once weekly, was noninferior to placebo with respect to safety (P<0.001 for noninferiority) but was not superior to placebo with respect to efficacy (P=0.06 for superiority). The rates of death from cardiovascular causes, fatal or nonfatal myocardial infarction, fatal or nonfatal stroke, hospitalization for heart failure, and hospitalization for acute coronary syndrome, and the incidence of acute pancreatitis, pancreatic cancer, medullary thyroid carcinoma, and serious adverse events did not differ significantly between the two groups. CONCLUSIONS: Among patients with type 2 diabetes with or without previous cardiovascular disease, the incidence of major adverse cardiovascular events did not differ significantly between patients who received exenatide and those who received placebo.     Practical repercussions of a paradox in curved beams

    This paper is devoted to the discussion of practical applications of a recent paradox in curved beams, according to which it is possible to simultaneously remove material and reduce the bending stresses. This paradox is compared to a similar paradox valid for rectilinear beams. The beam cross section zones from which material may advantageously be removed are clarified with the aid of Gateaux differentiation. A simplification that eases the application of the weight optimization procedure is commented with reference to crane hooks. Possible extensions to the optimization of small and big ends of a connecting rod are discussed

    DC Shipboard Microgrid Modeling for Fuel Cell Integration Study

    The integration of fuel cell technology in shipboard is essential for real fuel and pollution reduction in the marine sector. This integration can be pushed in the adoption of a dc shipboard power system (SPS). One of the impediments to this integration is the necessity to couple fuel cells (FCs) and battery energy storage systems (BESSs) and control them with a proper power management system (PMS), in order to sustain high load variations due to electric propulsion. In the literature, all these aspects are dealt independently and mostly assuming ideal conditions. The proposed control strategies are derived from the droop and secondary frequency regulation of the ac power system. This paper starts with a brief description on the characteristics of dc SPS along with fuel cell and battery modeling. Subsequently, this paper describes the proposed control strategy. Finally, this paper shows the behavior and principle of the proposed integration and analyzes the performance on a real measurement test case of a ferry. This paper is concluded by identifying the future research for the development of a PMS control strategy for critical marine missions
