Predicting recovery following stroke: deep learning, multimodal data and feature selection using explainable AI
Machine learning offers great potential for automated prediction of
post-stroke symptoms and their response to rehabilitation. Major challenges for
this endeavour include the very high dimensionality of neuroimaging data, the
relatively small size of the datasets available for learning, and how to
effectively combine neuroimaging and tabular data (e.g. demographic information
and clinical characteristics). This paper evaluates several solutions based on
two strategies. The first is to use 2D images that summarise MRI scans. The
second is to select key features that improve classification accuracy.
Additionally, we introduce the novel approach of training a convolutional
neural network (CNN) on images that combine regions-of-interest extracted from
MRIs, with symbolic representations of tabular data. We evaluate a series of
CNN architectures (both 2D and a 3D) that are trained on different
representations of MRI and tabular data, to predict whether a composite measure
of post-stroke spoken picture description ability is in the aphasic or
non-aphasic range. MRI and tabular data were acquired from 758 English speaking
stroke survivors who participated in the PLORAS study. The classification
accuracy for a baseline logistic regression was 0.678 for lesion size alone,
rising to 0.757 and 0.813 when initial symptom severity and recovery time were
successively added. The highest classification accuracy 0.854 was observed when
8 regions-of-interest was extracted from each MRI scan and combined with lesion
size, initial severity and recovery time in a 2D Residual Neural Network.Our
findings demonstrate how imaging and tabular data can be combined for high
post-stroke classification accuracy, even when the dataset is small in machine
learning terms. We conclude by proposing how the current models could be
improved to achieve even higher levels of accuracy using images from hospital
scanners
Parallaxes of southern extremely cool objects III : 118 L and T dwarfs
We present new results from the Parallaxes of Southern Extremely Cool dwarfs program to measure parallaxes, proper motions and multiepoch photometry of L and early T dwarfs. The observations were made on 108 nights over the course of 8 yr using the Wide Field Imager on the ESO 2.2m telescope. We present 118 new parallaxes of L and T dwarfs of which 52 have no published values and 24 of the 66 published values are preliminary estimates from this program. The parallax precision varies from 1.0 to 15.5mas with a median of 3.8mas. We find evidence for two objects with long term photometric variation and 24 new moving group candidates. We cross-match our sample to published photometric catalogues and find standard magnitudes in up to 16 pass-bands from which we build spectral energy distributions and H-R diagrams. This allows us to confirm the theoretically anticipated minimum in radius between stars and brown dwarfs across the hydrogen burning minimum mass. We find the minimum occurs between L2 and L6 and verify the predicted steep dependence of radius in the hydrogen burning regime and the gentle rise into the degenerate brown dwarf regime. We find a relatively young age of ~2 Gyr from the kinematics of our sample.Peer reviewedFinal Accepted Versio
MDL28170, a Calpain Inhibitor, Affects Trypanosoma cruzi Metacyclogenesis, Ultrastructure and Attachment to Rhodnius prolixus Midgut
BACKGROUND: Trypanosoma cruzi is the etiological agent of Chagas' disease. During the parasite life cycle, many molecules are involved in the differentiation process and infectivity. Peptidases are relevant for crucial steps of T. cruzi life cycle; as such, it is conceivable that they may participate in the metacyclogenesis and interaction with the invertebrate host. METHODOLOGY/PRINCIPAL FINDINGS: In this paper, we have investigated the effect of the calpain inhibitor MDL28170 on the attachment of T. cruzi epimastigotes to the luminal midgut surface of Rhodnius prolixus, as well as on the metacyclogenesis process and ultrastructure. MDL28170 treatment was capable of significantly reducing the number of bound epimastigotes to the luminal surface midgut of the insect. Once the cross-reactivity of the anti-Dm-calpain was assessed, it was possible to block calpain molecules by the antibody, leading to a significant reduction in the capacity of adhesion to the insect guts by T. cruzi. However, the antibodies were unable to interfere in metacyclogenesis, which was impaired by the calpain inhibitor presenting a significant reduction in the number of metacyclic trypomastigotes. The calpain inhibitor also promoted a direct effect against bloodstream trypomastigotes. Ultrastructural analysis of epimastigotes treated with the calpain inhibitor revealed disorganization in the reservosomes, Golgi and plasma membrane disruption. CONCLUSIONS/SIGNIFICANCE: The presence of calpain and calpain-like molecules in a wide range of organisms suggests that these proteins could be necessary for basic cellular functions. Herein, we demonstrated the effects of MDL28170 in crucial steps of the T. cruzi life cycle, such as attachment to the insect midgut and metacyclogenesis, as well as in parasite viability and morphology. Together with our previous findings, these results help to shed some light on the functions of T. cruzi calpains. Considering the potential roles of these molecules on the interaction with both invertebrate and vertebrate hosts, it is interesting to improve knowledge on these molecules in T. cruzi
Uptake and Accumulation of Oxidized Low-Density Lipoprotein during Mycobacterium tuberculosis Infection in Guinea Pigs
The typical host response to infection of humans and some animals by M. tuberculosis is the accumulation of reactive oxygen species generating inflammatory cells into discrete granulomas, which frequently develop central caseous necrosis. In previous studies we showed that infection of immunologically naĂŻve guinea pigs with M. tuberculosis leads to localized and systemic oxidative stress that results in a significant depletion of serum total antioxidant capacity and the accumulation of malondialdehyde, a bi-product of lipid peroxidation. Here we show that in addition, the generation of excessive reactive oxygen species in vivo resulted in the accumulation of oxidized low density lipoproteins (OxLDL) in pulmonary and extrapulmonary granulomas, serum and lung macrophages collected by bronchoalveolar lavage. Macrophages from immunologically naĂŻve guinea pigs infected with M. tuberculosis also had increased surface expression of the type 1 scavenger receptors CD36 and LOX1, which facilitate the uptake of oxidized host macromolecules including OxLDL. Vaccination of guinea pigs with Bacillus Calmette Guerin (BCG) prior to aerosol challenge reduced the bacterial burden as well as the intracellular accumulation of OxLDL and the expression of macrophage CD36 and LOX1. In vitro loading of guinea pig lung macrophages with OxLDL resulted in enhanced replication of bacilli compared to macrophages loaded with non-oxidized LDL. Overall, this study provides additional evidence of oxidative stress in M. tuberculosis infected guinea pigs and the potential role OxLDL laden macrophages have in supporting intracellular bacilli survival and persistence
Mayo Clinic consensus report on membranous nephropathy: proposal for a novel classification
Membranous nephropathy (MN) is a pattern of injury caused by autoantibodies binding to specific target antigens, with accumulation of immune complexes along the subepithelial region of glomerular basement membranes. The past 20 years have brought revolutionary advances in the understanding of MN, particularly via the discovery of novel target antigens and their respective autoantibodies. These discoveries have challenged the traditional classification of MN into primary and secondary forms. At least 14 target antigens have been identified, accounting for 80%-90% of cases of MN. Many of the forms of MN associated with these novel MN target antigens have distinctive clinical and pathologic phenotypes. The Mayo Clinic consensus report on MN proposes a 2-step classification of MN. The first step, when possible, is identification of the target antigen, based on a multistep algorithm and using a combination of serology, staining of the kidney biopsy tissue by immunofluorescence or immunohistochemistry, and/or mass spectrometry methodology. The second step is the search for a potential underlying disease or associated condition, which is particularly relevant when knowledge of the target antigen is available to direct it. The meeting acknowledges that the resources and equipment required to perform the proposed testing may not be generally available. However, the meeting consensus was that the time has come to adopt an antigen-based classification of MN because this approach will allow for accurate and specific MN diagnosis, with significant implications for patient management and targeted treatment
Quality indicators for the care and outcomes of adults with atrial fibrillation
Aims To develop quality indicators (QIs) that may be used to evaluate the quality of care and outcomes for adults with atrial fibrillation (AF). Methods and results We followed the ESC methodology for QI development. This methodology involved (i) the identification of the domains of AF care for the diagnosis and management of AF (by a group of experts including members of the ESC Clinical Practice Guidelines Task Force for AF); (ii) the construction of candidate QIs (including a systematic review of the literature); and (iii) the selection of the final set of QIs (using a modified Delphi method). Six domains of care for the diagnosis and management of AF were identified: (i) Patient assessment (baseline and follow-up), (ii) Anticoagulation therapy, (iii) Rate control strategy, (iv) Rhythm control strategy, (v) Risk factor management, and (vi) Outcomes measures, including patient-reported outcome measures (PROMs). In total, 17 main and 17 secondary QIs, which covered all six domains of care for the diagnosis and management of AF, were selected. The outcome domain included measures on the consequences and treatment of AF, as well as PROMs. Conclusion This document defines six domains of AF care (patient assessment, anticoagulation, rate control, rhythm control, risk factor management, and outcomes), and provides 17 main and 17 secondary QIs for the diagnosis and management of AF. It is anticipated that implementation of these QIs will improve the quality of AF care
Mathematical Modeling of Epicardial RF Ablation of Atrial Tissue with Overlying Epicardial Fat
The efficacy of treating atrial fibrillation by RF ablation on the epicardial surface is currently under question due to the presence of epicardial adipose tissue interposed between the ablation electrode and target site (atrial wall). The problem is probably caused by the electrical conductivity of the fat (0.02 S/m) being lower than that of the atrial tissue (0.4-0.6 S/m). Since our objective is to improve epicardial RF ablation techniques, we planned a study based on a two-dimensional mathematical model including an active electrode, a fragment of epicardial fat over a fragment of atrial tissue, and a section of atrium with circulating blood. Different procedures for applying RF power were studied, such as varying the frequency, using a cooled instead of a dry electrode, and different modes of controlling RF power (constant current, temperature and voltage) for different values of epicardial fat thickness. In general, the results showed that the epicardial fat layer seriously impedes the passage of RF current, thus reducing the effectiveness of atrial wall RF ablation
