Heart rate turbulence - an overview of methods and applications
Heart rate turbulence (HRT) is a relatively new but already well-established non-invasive
ECG risk predictor. It has been confirmed as a strong predictor of mortality in large populations
of post-infarction patients treated with both classical and modern methods. There have been
contradictory results regarding its predictive value in other high-risk populations (idiopathic
dilated cardiomyopathy, hypertrophic cardiomyopathy and low ejection fraction cohorts). It is
important that HRT seems to be particularly useful in identifying high-risk patients with
preserved left ventricular function, the group of patients not covered by current indications for
implantable cardioverter defibrillators. Further studies are needed to establish the need for
correlation with heart rate, number of ventricular premature beats, heart rate variability
parameters or clinical covariates in the process of risk stratification by means of HRT
Prognostic value of Holter monitoring in congestive heart failure
Congestive heart failure (CHF) is an increasingly widespread, costly and deadly disease,
frequently named as epidemics of the 21 century. Despite advancement in modern treatment,
mortality rate in CHF patients remains high. Therefore, risk stratification in patients with
CHF remains one of the major challenges of contemporary cardiology. Electrocardiographic
parameters based on ambulatory Holter monitoring have been documented to be independent
risk predictors of total mortality and progression of heart failure. Recent years brought an
increased interest in evaluation of dynamic Holter-derived ECG markers reflecting changes in
heart rate and ventricular repolarization behavior. It is widely accepted that structural changes
reflecting myocardial substrate are better identified by means of imaging techniques, Holter
monitoring on the other hand provides complementary information on myocardial vulnerability
and autonomic nervous system. Therefore, combining the electrocardiographic stratification
with assessment of myocardial substrate may provide the complex insight into interplay between
factors contributing to death.
The present article reviews the literature data on the prognostic role of various Holter-based
ECG parameters, with special emphasis to dynamic ECG risk markers - heart rate
variability, heart rate turbulence, repolarization dynamics and variability - in predicting
mortality, as well as different modes of death in patients with CH
Late diagnosis of congenital cardiovascular defect
Coarctation of the aorta (CoA) is a common congenital anomaly that is usually treated in
infancy or childhood. Adult patients with coarctation have a high incidence of associated
cardiac disorders, including valve diseases, atrial fibrillation and ischemic heart disease.
Most patients with uncorrected CoA die before reaching the age of 50 from complications such
as myocardial infarction, intracranial hemorrhage, congestive heart failure (HF), infective
endocarditis or aortic dissection.
We report the case of a 65 year-old woman admitted to hospital with symptoms of heart failure
NYHA class IV. She had been treated for several years for refractory arterial hypertension and
concomitant stenocardia (II CCS). The symptoms of HF had been increasing over several
months. Outpatient echocardiography examination revealed significant, increasing mitral
and tricuspid valve regurgitation with progressive left ventricular dysfunction. The patient
was referred for surgical repair of the mitral and tricuspid valves. In-hospital echocardiography
and angiography revealed descending aorta discontinuity at the level of the aortic isthmus.
This congenital disease revealed during hospitalization was determined to be the underlying
cause of all the symptoms the patient presented. Due to the clinical status of the patient, she was
discharged from surgical procedures and put on medication. (Cardiol J 2012; 19, 2: 201–203
Turbulencja rytmu serca - przegląd metod i zastosowanie
Ocena turbulencji rytmu serca (HRT) jest nową, uznaną już nieinwazyjną metodą stratyfikacji
ryzyka za pomocą EKG. HRT jest silnym czynnikiem predykcyjnym śmiertelności, którego
wartość prognostyczną potwierdzono w dużych populacjach pacjentów po zawale serca, leczonych
nowymi oraz klasycznymi metodami. Istnieją sprzeczne wyniki dotyczące wartości predykcyjnej
HRT w innych populacjach pacjentów wysokiego ryzyka (z idiopatyczną kardiomiopatią
rozstrzeniową, kardiomiopatią przerostową i w grupie chorych z niską frakcją wyrzutową).
Ocena HRT wydaje się szczególnie przydatna w przewidywaniu wysokiego ryzyka zgonu
wśród pacjentów z zachowaną funkcją skurczową lewej komory - grupy nieobjętej w dotychczasowych
wytycznych wskazaniami do implantacji kardiowertera-defibrylatora. Potrzebne są
dalsze badania w celu oceny związku parametrów HRT z częstością akcji serca, liczbą przedwczesnych
pobudzeń komorowych, parametrami zmienności rytmu serca i parametrami klinicznymi

Wstęp: Elektrody endokawitarne wciąż pozostają "piętą achillesową" współczesnych układów do stałej stymulacji serca. W porównaniu z ogromnym postępem, jaki osiągnięto w konstrukcji stymulatorów, rozwój w technologii elektrod przebiega dużo wolniej. Jednym z parametrów, który budzi kontrowersje od prawie 40 lat jest polarność elektrody.
Materiał i metody: Dokonano retrospektywnej analizy zapisów holterowskich u 260 pacjentów z implantowanym jednojamowym komorowym stymulatorem serca (VVI). Grupa I (UP) - 130 pacjentów z implantowanymi stymulatorami VVI z unipolarnymi elektrodami - Biotronik TIR 60-UP, wszczepionymi w latach 1993–1995. Grupa II (BP) - 130 pacjentów z implantowanymi stymulatorami serca VVI z elektrodami bipolarnymi - Biotronik TIR 60-BP, wszczepionymi w latach 1998–2000.
Oceniano zaburzenia wyczuwania oraz zaburzenia stymulacji. U wszystkich pacjentów wykonano badanie holterowskie w okresie 3–24 miesięcy, średnio 17,1 miesiąca po implantacji.
Wyniki: W grupie I (pacjenci z elektrodami unipolarnymi) - najczęściej obserwowanym zaburzeniem były incydenty oversensing - hamowania miopotencjałami (18 pacjentów). Kolejne zaburzenie to epizody undersensing - zaobserwowane u 6 pacjentów. Zaburzenia stymulacji wystąpiły u 4 chorych. Tylko u 2 osób z grupy II ujawniono nieprawidłowości w funkcjonowaniu układu stymulującego. Były to epizody undersensing u 1 pacjenta i oversensing u 1 chorego. U żadnej osoby z grupy II nie stwierdzono zaburzeń stymulacji.
Wnioski: Zastosowanie elektrod dwubiegunowych w istotny sposób zmniejszyło liczbę zaburzeń sterowania zarówno o typie under-, jak i oversensing u pacjentów z implantowanymi stymulatorami VVI. Wydaje się, że postęp w technologii produkcji elektrod wewnątrzsercowych przyczynia się do redukcji ryzyka incydentów zaburzeń stymulacji u pacjentów ze stymulatorami VVI. The usefulness of 24 hour Holter monitoring in asymptomatic pacemaker patients in early post-implantation period
Background: Twenty four hour Holter monitoring (HM) in an early post-implantation
period in asymptomatic patients is considered as class IIb according to the ACC/AHA guidelines.
It seems that post-implantation assessment extended by 24 hour HM in these patients
might shorten hospitalization and increase safety of these patients. This aspect has not been
widely discussed so far. The aim of our study was to evaluate pacing and sensing disturbances
in asymptomatic patients with proper parameters of single and double chamber pacemakers.
Methods: Studied group included 236 patients implanted with Biotronik Actros S (single
chamber) (group I - 130 patients) and Biotronik Actros D or Axios D (group II - 106 patients)
pacemakers. In all the patients 24 hour HM was performed 1-6 days after implantation (mean 3.4)
in order to assess all pacing and sensing disturbances.
Results: Sensing disturbances were found in 2 patients from group I and 22 patients from
group II (the most frequent pacemaker failure was atrial undersensing followed by ventricular
oversensing-T wave stering). In 1 patient from group I atrial failure to pace was observed. In
whole group pacing/sensing disturbances were found in 23% of patients, nevertheless they did
not provoke any hemodynamic consequences.
Conclusions: In an early post-implantation period pacemaker disturbances occur in 23% of
asymptomatic patients being more frequent in patients with dual chamber pacemaker. Atrial
undersensing and ventricular oversensing are the most common disturbances, nevertheless
having no hemodynamic consequences they are not life-threatening. Detection of these episodes
in an early post-implantation period allows for early change in pacemakers’ parameters and
thus decreasing risk of rehospitalization. We confirmed the low usefulness of HM in patients

Inappropriate sinus tachycardia — ailment or disease?
We present a case of a 36-year-old female patient with a long history of persistent palpitations, limited exercise tolerance, and pre-syncope. ECG recordings revealed long lasting episodes of inappropriate sinus tachycardia (IST). IST is characterised by persistent cardiac arrhythmias with sinus P waves in the ECG, and an exaggerated response to a minimal physical effort or stress. Using 24-h Holter definition the average heart rate in patients with IST should be > 90 bpm or daytime heart rate > 100 bpm. The syndrome is associated neither with structural heart disease nor with any reversible causes of sinus tachycardia (i.e. fever, anemia, infection, hyperthyroidism, drug abuse). Therefore the diagnosis is generally one of exclusion. Beta-blockers are most frequently used as first line therapy. Other potentially effective drugs are nondihydropyridine calcium antagonists or ivabradine.
