    Mesenchymal stem cells - a promising therapy for Acute Respiratory Distress Syndrome

    Acute Respiratory Distress Syndrome (ARDS) constitutes a spectrum of severe acute respiratory failure in response to a variety of inciting stimuli that is the leading cause of death and disability in the critically ill. Despite decades of research, there are no therapies for ARDS, and management remains supportive. A growing understanding of the complexity of the pathophysiology of ARDS, coupled with advances in stem cell biology, has lead to a renewed interest in the therapeutic potential of mesenchymal stem cells for ARDS. Recent evidence suggests that mesenchymal stem cells can modulate the immune response to reduce injury and also increase resistance to infection, while also facilitating regeneration and repair of the injured lung. This unique combination of effects has generated considerable excitement. We review the biological characteristics of mesenchymal stem cells that underlie their therapeutic potential for ARDS. We also summarise existing pre-clinical evidence, evaluate the potential and pitfalls of using mesenchymal stem cells for treatment, and examine the likely future directions for mesenchymal stem cells in ARDS

    Carbon dioxide is a waste product of aerobic cellular respiration in all aerobic life forms. PaCO2 represents the balance between the carbon dioxide produced and that eliminated. Hypocapnia remains a common - and generally underappreciated - component of many disease states, including early asthma, high-altitude pulmonary edema, and acute lung injury. Induction of hypocapnia remains a common, if controversial, practice in both adults and children with acute brain injury. In contrast, hypercapnia has traditionally been avoided in order to keep parameters normal. More recently, advances in our understanding of the role of excessive tidal volume has prompted clinicians to use ventilation strategies that result in hypercapnia. Consequently, hypercapnia has become increasingly prevalent in the critically ill patient. Hypercapnia may play a beneficial role in the pathogenesis of inflammation and tissue injury, but may hinder the host response to sepsis and reduce repair. In contrast, hypocapnia may be a pathogenic entity in the setting of critical illness. The present paper reviews the current clinical status of low and high PaCO2 in the critically ill patient, discusses the insights gained to date from studies of carbon dioxide, identifies key concerns regarding hypocapnia and hypercapnia, and considers the potential clinical implications for the management of patients with acute lung injury

    Effect of Harvest Timing and Soil Moisture Content on Compaction, Growth and Harvest Yield in a Miscanthus Cropping System

    peer-reviewedHarvesting Miscanthus × giganteus (J.M. Greef & Deuter ex Hodkinson & Renvoize) after shoot emergence is known to reduce yields in subsequent seasons. This research was conducted in Miscanthus to assess the effects on crop response and soil compaction of annually repeated traffic, applied both before new growth in the rhizomes (early harvest) and after shoot emergence (late harvest), at two different soil moisture contents. While an annual early harvest, yields more than a late harvest, because damage to new shoots is avoided, soil compaction may be increased following repeated harvests. Five treatments were tested: (a) An untrafficked control, (b) early-traffic on soil with typical soil moisture content (SMC) (early-normal), (c) early-traffic on soil with elevated SMC (early-elevated), (d) late-traffic on soil with typical SMC (late-normal) and (e) late-traffic on soil with elevated SMC (late-wet). The experiment was conducted on a Gleysol in Co. Dublin, Ireland during 2010 and 2011. Crop response effects were assessed by measuring stem numbers, stem height, trafficked zone biomass yield (November) and overall stem yield (January). Compaction effects were assessed by measuring penetration resistance, bulk density and water infiltration rate. Trafficked zone biomass yield in the early-dry and early-wet treatments was, respectively, 18% and 23% lower than in the control, but was, respectively, 39% and 31% higher than in the late-dry treatment. Overall, stem yield was significantly lower in the late-normal and late-wet treatments (10.4 and 10.1 tdm ha−1 respectively) when compared with the control (12.4 tdm ha−1), but no significant difference was recorded in overall stem yield between both early-traffic treatments and the control. Penetration resistance values were significantly higher in all trafficked treatments when compared with the control at depths of 0.15 m (≥54–61%) and 0.30 m (≥27–57%) and were significantly higher in 2011 when compared with 2010 at depths of 0.15 and 0.30 m. Baler system traffic in Miscanthus significantly reduced yields and significantly increased compaction annually. Miscanthus harvested early, on a dry soil, yielded 1.1 tdm ha−1 more than when harvested late on a dry soil. The yield advantage increased to 1.3 tdm ha−1 when early harvesting on a soil with 40–43% moisture content was compared with late harvesting on a wetter soil (51–52% moisture content). In this study, the magnitude of yield losses from compaction or other causes in early harvests was substantially lower than the yield losses, which resulted from shoot damage in late harvests. It is likely in similar climates that the results of this study would also apply to other perennial crops growing in similar soil type

    INTRODUCTION: Annually in the UK, 20 000 children become very ill or injured and need specialist care within a paediatric intensive care unit (PICU). Most children survive. However, some children and their families may experience problems after they have left the PICU including physical, functional and/or emotional problems. It is unknown which children and families experience such problems, when these occur or what causes them. The aim of this mixed-method longitudinal cohort study is to understand the physical, functional, emotional and social impact of children surviving PICU (aged: 1 month-17 years), their parents and siblings, during the first year after a PICU admission. METHODS AND ANALYSIS: A quantitative study involving 300 child survivors of PICU; 300 parents; and 150-300 siblings will collect data (using self-completion questionnaires) at baseline, PICU discharge, 1, 3, 6 and 12 months post-PICU discharge. Questionnaires will comprise validated and reliable instruments. Demographic data, PICU admission and treatment data, health-related quality of life, functional status, strengths and difficulties behaviour and post-traumatic stress symptoms will be collected from the child. Parent and sibling data will be collected on the impact of paediatric health conditions on the family's functioning capabilities, levels of anxiety and social impact of the child's PICU admission. Data will be analysed using descriptive and inferential statistics. Concurrently, an embedded qualitative study involving semistructured interviews with 24 enrolled families at 3 months and 9 months post-PICU discharge will be undertaken. Framework analysis will be used to analyse the qualitative data. ETHICS AND DISSEMINATION: The study has received ethical approval from the National Health Services Research Ethics Committee (Ref: 19/WM/0290) and full governance clearance. This will be the first UK study to comprehensively investigate physical, functional, emotional and social consequences of PICU survival in the first-year postdischarge.Clinical Trials Registration Number: ISRCTN28072812 [Pre-results]

    Network Hawkes Process Models for Exploring Latent Hierarchy in Social Animal Interactions

    Group-based social dominance hierarchies are of essential interest in animal behavior research. Studies often record aggressive interactions observed over time, and models that can capture such dynamic hierarchy are therefore crucial. Traditional ranking methods summarize interactions across time, using only aggregate counts. Instead, we take advantage of the interaction timestamps, proposing a series of network point process models with latent ranks. We carefully design these models to incorporate important characteristics of animal interaction data, including the winner effect, bursting and pair-flip phenomena. Through iteratively constructing and evaluating these models we arrive at the final cohort Markov-Modulated Hawkes process (C-MMHP), which best characterizes all aforementioned patterns observed in interaction data. We compare all models using simulated and real data. Using statistically developed diagnostic perspectives, we demonstrate that the C-MMHP model outperforms other methods, capturing relevant latent ranking structures that lead to meaningful predictions for real data

    Increased telomere attrition following renal transplantation: impact of anti-metabolite therapy

    Background: The uremic milieu exposes chronic kidney disease (CKD) patients to premature ageing processes. The impact of renal replacement therapy (dialysis and renal transplantation [RTx]) or immunosuppressive treatment regimens on ageing biomarkers has scarcely been studied. Methods: In this study telomere length in whole blood cells was measured in 49 dialysis patients and 47 RTx patients close to therapy initiation and again after 12 months. Forty-three non-CKD patients were included as controls. Results: Non-CKD patients had significantly (P <= 0.01) longer telomeres than CKD patients. Telomere attrition after 12 months was significantly greater in RTx patients compared to dialysis patients (P = 0.008). RTx patients receiving mycophenolate mofetil (MMF) had a greater (P = 0.007) degree of telomere attrition compared to those treated with azathioprine. After 12 months, folate was significantly higher in RTx patients than in dialysis patients (P < 0.0001), whereas the opposite was true for homocysteine (P < 0.0001). The azathioprine group had lower levels of folate after 12 months than the MMF group (P = 0.003). Conclusions: The associations between immunosuppressive therapy, telomere attrition, and changes in folate indicate a link between methyl donor potential, immunosuppressive drugs, and biological ageing. The hypothesis that the increased telomere attrition, observed in the MMF group after RTx, is driven by the immunosuppressive treatment, deserves further attention
