    From capability to concept : fusion of systems analysis techniques for derivation of future soldier systems

    The intent of this thesis is to define a set of processes for use within UK Government dismounted soldier systems research that will provide stakeholders with auditable and traceable information to understand gaps in military capability and justify future procurement decisions. The need for this approach is linked to organisational shifts within the UK Ministry of Defence, and more specifically Government research with the move towards procurement of capability rather than equipment. In conjunction with reducing defence budgets and increased scrutiny, there is a need to prioritise spending to those areas that will provide the most significant enhancement to operational effectiveness. The proposed process suite provides underpinning data to support Government decisions, from definition of military need through to concept design and prioritisation of future research activities. The approach is grounded in the field of systems thinking and systems engineering providing the logical and systematic constructs required for highly complex systems where the human is a central focus. A novel fusion of existing systems tools and techniques enables both subjective data from domain experts and objective data in the form of operational analysis and field trials to be utilised for analysis across the five NATO capability domains, with output defining the relative importance of survivability, sustainability, mobility, lethality and C4I in the context of operational and strategic level military goals as well as wider challenges represented by the doctrinal defence lines of development. Future developments should include alignment with developing pan-MoD initiatives in the form of MODAF, if required by the customer organisation.     Perspectives of Portuguese People with Physical Disabilities Regarding Their Sexual Health: A Focus Group Study

    The World Health Organization has considered sexual health as a major dimension of global health and a sexual right. However, the sexual health of people with physical disabilities is still poorly addressed by health and social care professionals, and it is very stigmatized by society. This study aimed to assess the perspectives of Portuguese people living with physical disabilities regarding issues affecting their sexual health. Nine women and 17 men with different physical disabilities participated in the study. Participants were recruited from a professional rehabilitation facility located in the North of Portugal and were assigned to four groups in one-hour sessions. Three main categories emerged from the content analysis: (1) meanings and beliefs regarding sexuality; (2) experiences of sexuality; (3) necessary changes. Despite the positive social changes towards sexuality, participants expressed that their sexual rights are still unfulfilled, as they live in a context that perpetuates their dependency. They pointed out low self-esteem, prejudice and social isolation, poor architectural accessibility and scarcity of financial support as some of the barriers to their lives and their sexual health. Finally, participants identified the main needs regarding their sexual health, such as: access to specialized information; training for health professionals. This study gives voice to people with physical disabilities and sheds light into both individual and contextual factors affecting their sexual health.     The 'Choice and Autonomy Framework' : implications for occupational therapy practice

    Introduction This paper presents findings from a PhD study exploring autonomy of adults with physical disability. The plethora of descriptions of autonomy in psychological, occupational therapy and rehabilitation literature (e.g. Ryan and Deci 2000, Rogers 1982, Cardol et al 2002) detracts from the centrality of autonomy and results in difficulty incorporating it into occupational therapy practice. This paper presents a framework providing an integrated, clinically useful approach to autonomy. Methods Sixteen people were recruited, based on age, gender, impairment and living circumstances (community/residential settings). All have significant physical disability, use a wheelchair and require personal assistance for some/all self-care activities. Qualitative methods were used for data collection, including life-history narrative, diary information and extensive interview. An integrated method of analysis was used, including content analysis and bracketing. Results The ‘Choice and Autonomy Framework’ consists of five strands, including: • the meaning of autonomy • whether or not autonomy is a goal or value • the experience of autonomy • personality factors that impact autonomy • environmental features that enhance or negate autonomy. This paper will describe each strand, as derived from the research. The results suggest that, contrary to common wisdom (Hmel and Pincus 2002), autonomy is not necessarily a universal goal for people with physical disability; an understanding of the person’s own perspective will enhance person-centred practice and enable therapists to further recognise individuality of clients. It will argue that the concept of autonomy needs to be further understood and incorporated into occupational therapy practice
