    The Hepatologist in the Haematologist Field: a Rare Cause of Non-Cirrhotic Portal Hypertension

    An 81 year-old gentleman was admitted to the emergency department of our hospital due to hematemesis and melena, and endoscopic evidence of portal hypertension. Due to presence of ascites and esophageal varices he was referred to gastroenterology care. His history included systemic mastocytosis, and biochemistry showed marked cholestasis with normal liver function tests. Clinical work-up was not consistent with liver disease-related portal hypertension, but rather hepatic involvement of systemic mastocytosis and the development of non-cirrhotic portal hypertension. This clinical case highlights that although systemic mastocytosis is uncommon, the subsequent development of liver involvement and non-cirrhotic portal hypertension are complications of the disease that need always be considere

    Analgesia and/or anaesthesia during piglet castration – part II: practicability of farm protocols, resource efficiency and economic implications

    Pain alleviation associated with castration of piglets is a debated welfare issue. This study compares practical aspects, resource efficiency and economic implications of two protocols involving both analgesia and anaesthesia compared to a control group: conventional castration without pain relief (CTRL); joint administration of azaperone and meloxicam (AZA-MEL), i.m.; procaine (PROC-MEL), s.c., and meloxicam, i.m. A total number of 356 male piglets (56 L), was involved. Labour, mortality during the lactation period and costs for procedures were analysed. The total amount of labour required for each single male piglet and the risk of recording at least one dead piglet during lactation in litters were significantly higher in AZA-MEL and PROC-MEL groups than in CTRL group (labour: 02:04 and 02:04 vs. 01:18 min, respectively, p <.001; mortality risk: (RR = 1.48; CI 95% = 1.02 12 2.16; p =.029). The cost estimated for the castration of each male piglet in CRTL group was 0.32 \u20ac, whereas was 3.14 \u20ac for AZA-MEL group and 3.30 \u20ac for PROC-MEL group. The results suggest that adopting analgesia and anaesthesia showed notable cost increases for farmers. This might be expected and justifiable when the management is improved to reach a higher standard quality, such as in the case of welfare-friendly surgical castration, but might be questionable when also considering the result of increased piglet mortality in the lactation period.HIGHLIGHTS To use meloxicam + azaperone or + procaine on farm during piglets castration, increases labour of workers by 76.8 and 56.5%, respectively. At the same time, also piglets mortality risk increases by 48% during lactation. Total costs for each male piglet castrated with meloxicam + azaperone or + procaine increased by 3.14 and 3.30 \u20ac, respectively

    Designs and methodologies to recreate in vitro human gut microbiota models

    The human gut microbiota is widely considered to be a metabolic organ hidden within our bodies, playing a crucial role in the host’s physiology. Several factors affect its composition, so a wide variety of microbes residing in the gut are present in the world population. Individual excessive imbalances in microbial composition are often associated with human disorders and pathologies, and new investigative strategies to gain insight into these pathologies and define pharmaceutical therapies for their treatment are needed. In vitro models of the human gut microbiota are commonly used to study microbial fermentation patterns, community composition, and host-microbe interactions. Bioreactors and microfluidic devices have been designed to culture microorganisms from the human gut microbiota in a dynamic environment in the presence or absence of eukaryotic cells to interact with. In this review, we will describe the overall elements required to create a functioning, reproducible, and accurate in vitro culture of the human gut microbiota. In addition, we will analyze some of the devices currently used to study fermentation processes and relationships between the human gut microbiota and host eukaryotic cells

    Proton pump inhibitors: use and misuse in the clinical setting

    Introduction: The introduction of proton pump inhibitors (PPIs) into clinical practice has greatly improved our therapeutic approach to acid-related diseases for their efficacy and safety. Areas Covered: The following evidence-based indications for PPI use are acknowledged by many scientific societies: treatment of the various forms and complications of gastroesophageal reflux disease, eradication of H. pylori infection in combination with two or more antibiotics, short- and long-term therapy of H. pylori-negative peptic ulcers, healing, and prevention of NSAID/COXIB-associated gastric ulcers, co-therapy with endoscopic procedures to control upper digestive bleeding and medical treatment of Zollinger Ellison syndrome. Expert Commentary: Despite the above well-defined indications, however, the use of PPIs continues to grow every year in both western and eastern countries and the endless expansion of the PPI market has created important problems for many regulatory authorities for two relevant features: the progressive increase of the costs of therapy and the greater potential harms for the patients. The major reasons for the misuse of PPIs are the prevention of gastro-duodenal ulcers in patients without risk factors and the stress ulcer prophylaxis in non-intensive care units, steroid therapy alone, anti-platelet or anti-coagulant treatment in patients without risk of gastric injury and the overtreatment of functional dyspepsia

    Volumetric analysis of gingival crevicular fluid and peri-implant sulcus fluid in healthy and diseased sites: A cross-sectional split-mouth pilot study

    Background: Researchers have recently drawn attention to the analysis of gingival crevicular fluid (GCF) and peri-implant sulcus fluid (PISF) for the implementation of the diagnosis of periodontal and peri-implant disease. Nevertheless, the measurements of volume and biomarkers concentration can be critically biased when data collected from studies with parallel group design are compared, given the technical difficulties, methodological variables, as well as the variability of crevicular fluid characteristics among different individuals. Objective: The aim of the present study was to assess the GCF and PISF volumes in healthy and diseased sites belonging to the same patient. Method: Ten patients presenting a periodontally healthy tooth, a tooth with periodontitis, an implant with healthy peri-implant tissues and an implant with peri-implantitis were enrolled. Samples of GCF and PISF were collected from each site of interest and their volume measured with a Periotron 8000 device. Non-parametric statistical analysis was performed to test the significance of the differences in GCF and PISF volumes between i) sites of teeth and dental implants with the same condition of health or disease and ii) healthy and diseased sites of both teeth and dental implants subgroups. The correlation between probing pocket depth (PPD) and fluid production was also tested (p<0.05). Results: Healthy periodontal and peri-implant tissues produced comparable amounts of fluid that was significantly lower than in diseased sites (p<0.05). In the presence of diagnosed disease, the volumes of GCF and PISF were similar, too. The correlation between PPD and fluid production was significant only in healthy sites (PPD/GCF, \u3c1=0.890, p<0.001; PPD/PISF, \u3c1=0.810; p<0.005). Conclusion: The periodontal and peri-implant tissues behaved similarly in terms of fluid production in condition of both health and active disease

    "It's All COVID's Fault!": Symptoms of Distress among Workers in an Italian General Hospital during the Pandemic

    Background: Since the outbreak of the COVID-19 pandemic, healthcare workers (HCWs) have been faced with specific stressors endangering their physical and mental health and their functioning. This study aimed to assess the short-term psychological health of a sample of Italian HCWs and the related influencing factors. In particular, the study focused on the differences related to HCWs' gender and to having been directly in charge of COVID-19 patients or not. Methods: An online survey was administered to the whole staff of the Modena General University Hospital three months after the onset of the pandemic, in 2020. Demographic data and changes in working and living conditions related to COVID-19 were collected; mental health status was assessed by the Depression, Anxiety and Stress Scale (DASS-21) and the Impact of Event Scale-Revised (IES-R). Results: 1172 out of 4788 members returned the survey (response rate = 24.5%), the male/female ratio was 30/70%. Clinically significant symptoms assessed according to the DASS-21 emerged among 21.0% of the respondents for depression, 22.5% for anxiety and 27.0% for stress. Symptoms suggestive of a traumatic reaction were reported by 19.0% of the sample. Symptoms of psychological distress were statistically associated with female gender, job role, ward, changes in lifestyle, whereas first-line work with COVID-19 patients was statistically associated with more stress symptoms. HCWs reported a significant level of psychological distress that could reach severe clinical significance and impact dramatically their quality of life and functioning. Conclusions: Considering the persistence of the international emergency, effective strategies to anticipate, recognize and address distress in HCWs are essential, also because they may impact the organization and effectiveness of healthcare systems

    Cortico-Subcortical White Matter Bundle Changes in Cervical Dystonia and Blepharospasm

    Dystonia is thought to be a network disorder due to abnormalities in the basal ganglia-thalamo-cortical circuit. We aimed to investigate the white matter (WM) microstructural damage of bundles connecting pre-defined subcortical and cortical regions in cervical dystonia (CD) and blepharospasm (BSP). Thirty-five patients (17 with CD and 18 with BSP) and 17 healthy subjects underwent MRI, including diffusion tensor imaging (DTI). Probabilistic tractography (BedpostX) was performed to reconstruct WM tracts connecting the globus pallidus, putamen and thalamus with the primary motor, primary sensory and supplementary motor cortices. WM tract integrity was evaluated by deriving their DTI metrics. Significant differences in mean, radial and axial diffusivity between CD and HS and between BSP and HS were found in the majority of the reconstructed WM tracts, while no differences were found between the two groups of patients. The observation of abnormalities in DTI metrics of specific WM tracts suggests a diffuse and extensive loss of WM integrity as a common feature of CD and BSP, aligning with the increasing evidence of microstructural damage of several brain regions belonging to specific circuits, such as the basal ganglia-thalamo-cortical circuit, which likely reflects a common pathophysiological mechanism of focal dystonia

    Case report: KETOLAND the psychoeducation program for ketogenic diet

    Glucose transporter type 1 deficiency syndrome (GLUT1DS) is a rare neurological disorder characterized by a wide spectrum of symptoms: epilepsy, movement disorders and neurocognitive impairment. The gold standard treatment for GLUT1DS are ketogenic dietary therapies (KDTs), specifically classical ketogenic diet (CKD). Despite the benefits, CKD often represents a challenge for patients and their families since meal preparation is extremely demanding and deviates a lot from normal diet. To assure an optimal compliance to CKD a psychological support for parents and patients with GLUT1DS is highly recommended. Specifically, a psychoeducational intervention that ameliorates the knowledge about the illness and its therapy improves treatment' s adherence and efficacy. The aim of this case report is to investigate the effectiveness of a psychoeducational program, partially implemented through telepsychology, based on the theoretical model of Cognitive Behavioral Play Therapy (CBPT) to support KDT knowledge and adherence in a patient with GLUT1DS who presented a worsening of her clinical picture due to a sparse knowledge of KDTs principles which determined a low adherence. Thus, with this case report we propose a model of intervention with psychoeducation in a patient with a complex chronic disease
