Motor neuron disease in blacks

A series of 86 black, Indian and white patients with motor neuron disease were analysed retrospectively. Although the material does not allow statistically valid conclusions, there are sufficient cases among blacks to allow two prima facie observations in this population group: (i) motor neuron disease has an earlier age of onset than in whites and Indians; and (ii) more patients come from peripheral and rural areas than would be expected in prevailing circumstances.S Af Med J 1989; 76: 155-15

Cerebellar Nocardiosis and Myopathy from Long-Term Corticosteroids for Idiopathic Thrombocytopenia

Infection of the central nervous system with Nocardia sp. usually manifests as supratentorial abscesses. Supratentorial and cerebellar abscesses from infection with Nocardia sp. following immunosuppression with long-term corticosteroids for idiopathic thrombocytopenia (ITP) have not been reported. An 83 years-old, human immunodeficiency virus (HIV)-negative, polymorbid male with ITP for which he required corticosteroids since age 53 years developed tiredness, dyspnoea, hemoptysis, abdominal pain, and progressive gait disturbance. Imaging studies of the lung revealed an enhancing tumour in the right upper lobe with central and peripheral necrosis, multiple irregularly contoured hyperdensities over both lungs, and right-sided pleural effusions. Sputum culture grew Nocardia sp. Neurological diagnostic work-up revealed dysarthria, dysphagia, ptosis, hypoacusis, tremor, dysdiadochokinesia, proximal weakness of the lower limbs, diffuse wasting, and stocking-type sensory disturbances. The neurological deficits were attributed to an abscess in the upper cerebellar vermis, myopathy from corticosteroids, and polyneuropathy. Meropenem for 37 days and trimethoprime-sulfamethoxazole for 3 months resulted in a reduction of the pulmonary, but not the cerebral lesions. Therefore, sultamicillin was begun, but without success. Long-term therapy with corticosteroids for ITP may induce not only steroid myopathy but also immune-incompetence with the development of pulmonary and cerebral nocardiosis. Cerebral nocardiosis may not sufficiently respond to long-term antibiotic therapy why switching to alternative antibiotics or surgery may be necessary

Hymns across the water … translated and relocated: The reception of Scottish hymns in translation

In this article a broad overview of the reception of Scottish hymnody in translation is given. Considering the pivotal role the metrical Psalms used to play in the Scottish churches, they are considered first. Only one metrical Psalm made it to be translated widely and to be included in hymnals over the world. It is the metrical setting of Psalm 23, The Lord’s my Shepherd, paired to the tune of Scottish origin, CRIMOND. It is argued that the metrical psalm owes much of its popularity to the tune. A hymn with a text from the Scottish Paraphrases, paired to a Scottish tune, DUNDEE, and for long the only hymn in the Afrikaans churches with a Scottish connection, is discussed with regard to its reception in these churches. It serves as an example of how a hymn could be translated and relocated and function in a new context. In an overview of Scottish hymns translated and included in the newest hymnals in other countries, such as the Netherlands and Norway, it is shown that primarily hymns and songs of the Iona Community are translated and included in the newest hymnals, with John Lamberton Bell being the main exponent as text, hymn and song writer
