    Radioimaging Diagnosis of Vaterian Ampulloma: Technique, Semiology, and Differential Diagnosis - Review

    The lining of the Vater papilla represents a complex set of folds of the mucous membrane which acts as an anti-reflux valve and participates in regulating the flow in pancreatic and biliary secretion. The constitutive smooth muscles of the Oddi sphincter differ both anatomically and especially embryologically from the parietal duodenal muscles. This is one of the reasons why some authors believe that the vaterian ampulloma is a distinct oncologic entity from the rest of the duodenal or pancreatic primitive neoplasia. The variations of the implantation manner in the papilla of the common bile duct and pancreatic one simultaneously determine different ways of surgical approach of malignancies of this region. Radioimaging diagnostic methods in suspected vaterian ampulloma require a more special technique. The purpose of this paper is to determine the potential semiological, radiological, and imaging characteristics that will allow a diagnosis of vaterian ampulloma, considering that the surgical therapy, as well as the survival rate in patients, is different from the other neoplasm of the region

    The Indicators’ Inadequacy and the Predictions’ Accuracy

    In this article, we proposed the introduction in literature of a new source of uncertainty in modeling and forecasting: the indicators’ inadequacy. Even if it was observed, a specific nominalization in the context of forecasting procedure has not been done yet. The inadequacy of indicators as a supplementary source of uncertainty generates a lower degree of accuracy in forecasting. This assumption was proved using empirical data related to the prediction of unemployment rate in Romania on the horizon 2011-2013. Four strategies of modeling and predicting the unemployment rate were proposed, observing two types of indicators’ inadequacy: the use of transformed variables in order to get stationary data set (the difference between the unemployment rates registered in two successive periods was used instead of the unemployment rate) and the utilization of macro-regional unemployment rates whose predictions are aggregated in order to forecast the overall unemployment rate in Romania. The results put in evidence that the predictions of the total unemployment rate using moving average models of order 2 are the most accurate, being followed by the forecasts based on the predictions of active civil population and number of unemployed people. The strategies based on the aggregation of the predictions for the four macro-regional unemployment rates imply a higher inadequacy and consequently a lower degree of forecasts’ accuracy

    Governance structure of nonprofit sports organizations involves three elements: the board, the governing board, and one or more subcommittees. The administrative board is made up of individuals or organizations registered as members, which have the right to vote on their membership status. Members of a nonprofit sports organization may include individual players or athletes, coaches, officials, administrators or other individuals. The governing board is responsible for making decisions during a mandate on behalf of the council and non-profit sports organizations. The governing board acts as the main forum of government where most organizational decisions are taken. In order to complete the objectives of governing, most non-profit sports organizations operate a system of subcommittees. The debt of the subcommittee is to focus on specific issues such as technical rules of committees, developing the training, financial management, human resources management or commercial business. Governance structures of nonprofit sports organizations were designed and analyzed extensively from three different perspectives: organizational theory, models of governance and inter-organizational relations. Each of these perspectives provide useful ways to consider the significant and recurring problems in the governance structure of nonprofit sports organizations. The organizational theory confirms that bigger organizations have become more formal, with several roles and departments of specialists and several levels of management in comparison with smaller organization

    According to statistical data published in 2019, breast cancer is among the leading causes of death in women worldwide. The serine/threonine kinase (AKT) or protein kinase B (PkB) signaling pathway is activated by phosphorylation processes, which further is associated with cell growth, proliferation, and survival, but also with activation of glucose metabolism. Mutations of the AKT signaling pathway components (especially PI3KCA and PTEN) have been observed in breast cancer patients, which are associated with resistance to hormonal treatment. Many clinical trials are testing the effect of AKT inhibition in order to block the growth and proliferation of breast cancer cells. The purpose of this review is to present the incidence of this neoplastic disease, to describe AKT signaling pathways activation, mutations that occur at its level, and inhibitors that can block this protein kinase

    Dyslipidemia constitutes a well-known factor that can lead to acute pancreatitis. Hypertrigliceridemia and hypercholesterolemia are part of dyslipidemia. In a prospecitve study, we analyzed the role of hypercholesterolemia in triggering episodes of acute pancreatitis and the capacity of cholesterol blood level to predict the severity and the evolution of acute pancreatitis. In our prospective study, a preexistent cholesterol blood level above 240 mg/dl proved to be a trigger for pancreatitis and an increasing cholesterol level in evolution predicts a pancreatitis with organ failure (moderately-severe or severe)

    Inflammatory bowel diseases (IBD), namely Crohn’s disease and ulcerative colitis, are relatively rare diseases in our country, known as a low prevalence geographic region. IBD are a multidisciplinary problem, that implies gastroenterologists, as well as surgeons. Surgical management in inflammatory bowel disease is often impaired by a high complication rate and a significant recurrence rate, specific mostly for Crohn’s disease.     Over the past six decades, abdominal compartment syndrome (ACS) remained a very controversial subject, both in surgical and non-surgical specialties. Doctors failed to understand why critically ill patients died in the ICU with distended abdomens without fi nding any cause or why postoperative patients with wound defects such as dehiscence died after suturing the wound again „very tightly”. After the concept of intra-abdominal pressure (IAP) was established and methods for measuring it and diagnosing intra-abdominal hypertension (IAH) were available for clinicians to use it, it became clearer that ACS was a very serious and life threating pathology and the need for a correct treatment is essential. In this article we will try to make a literature review of the past decade and see when and how to diagnose correctly a patient with ACS and also how the diagnostic and treatments methods changed over the years
