Antiepileptic drugs in pregnancy
Background: Antiepileptic drugs are prescribed for chronic conditions such as epilepsy and bipolar disorder. Without adequate management, such conditions can have detrimental effects in pregnancy. However, first trimester use of some antiepileptic drugs is associated with a two-threefold increase in the risk of major congenital malformations. When women and their health care professionals consider treatment regimens, quantified relative risks can help decide which drug, if any, would be taken during pregnancy. / Methods: Three studies were performed using UK primary care data from The Health Improvement Network (THIN). Prescribing patterns of antiepileptic drugs in pregnancy were examined. A validation study for recording of major congenital malformations and perinatal death was performed. Lastly, a cohort study of pregnant women prescribed antiepileptic drugs prior to pregnancy was conducted to examine the risk of major congenital malformations or perinatal death in different first trimester antiepileptic drugs regimens. / Results: One in 200 women were prescribed antiepileptic drugs in pregnancy. Carbamazepine, sodium valproate and lamotrigine were the most commonly used antiepileptic drugs in pregnancy between 1994 and 2012. In this period, 353,171 pregnancies were identified in THIN. The incidence of major congenital malformations was 1.9% and perinatal death was 0.4%. Amongst 1,633 pregnant women regularly prescribed antiepileptic drugs before pregnancy, there were 54 cases of major congenital malformations and perinatal deaths (3.3%, 95% CI 2.5-4.3%). The risk amongst women prescribed sodium valproate polytherapy was 12% (95% CI 5.9-21.0%) - significantly greater than those prescribed carbamazepine monotherapy (IRR 2.72, 95% CI 1.23-5.99), 5 sodium valproate monotherapy (IRR 3.42, 95% CI 1.35-8.66) and lamotrigine monotherapy (IRR 5.03, 95% CI 1.99-12.74). / Conclusions: Women taking sodium valproate polytherapy face a greater risk of major congenital malformations or perinatal death compared to other common monotherapy regimens. Further research is needed to corroborate these findings, however women and their physicians should aim to avoid sodium valproate polytherapy if possible

OBJECTIVE: The aim of this study was to examine the prevalence of major congenital malformations associated with antiepileptic drug (AED) treatment in pregnancy. PATIENTS AND METHODS: Using data from The Health Improvement Network, we identified women who have given live birth and their offspring. Four subgroups were selected based on the AED treatment in early pregnancy, valproate, carbamazepine, lamotrigine and women not receiving AED treatment. We compared the prevalence of major congenital malformations within children of these four groups and estimated prevalence ratios (PRs) using Poisson regression adjusted for maternal age, sex of child, quintiles of Townsend deprivation score and indication for treatment. RESULTS: In total, 240,071 women were included in the study. A total of 229 women were prescribed valproate in pregnancy, 357 were prescribed lamotrigine and 334 were prescribed carbamazepine and 239,151 women were not prescribed AEDs. Fifteen out of 229 (6.6%) women prescribed valproate gave birth to a child with a major congenital malformation. The figures for lamotrigine, carbamazepine and women not prescribed AEDs were 2.7%, 3.3% and 2.2%, respectively. The prevalence of major congenital malformation was similar for women prescribed lamotrigine or carbamazepine compared to women with no AED treatment in pregnancy. For women prescribed valproate in polytherapy, the prevalence was fourfold higher. After adjustments, the effect of estimates attenuated, but the prevalence remained two- to threefold higher in women prescribed valproate. CONCLUSION: The results of our study suggest that lamotrigine and carbamazepine are safer treatment options than valproate in pregnancy and should be considered as alternative treatment options for women of childbearing potential and in pregnancy

circumstellar discs within the next 20 years from both observational and
theoretical sides. We first present the expected improvements in our knowledge
of protoplanetary discs as for their masses, sizes, chemistry, the presence of
planets as well as the evolutionary processes shaping these discs. We then
explore the older debris disc stage and explain what will be learnt concerning
their birth, the intrinsic links between these discs and planets, the hot dust
and the gas detected around main sequence stars as well as discs around white

A CLASP-modulated cell edge barrier mechanism drives cell-wide cortical microtubule organization in Arabidopsis
It is well known that the parallel order of microtubules in the plant cell cortex defines the direction of cell expansion, yet it remains unclear how microtubule orientation is controlled, especially on a cell-wide basis. Here we show through 4D imaging and computational modelling that plant cell polyhedral geometry provides spatial input that determines array orientation and heterogeneity. Microtubules depolymerize when encountering sharp cell edges head-on, whereas those oriented parallel to those sharp edges remain. Edge-induced microtubule depolymerization, however, is overcome by the microtubule-associated protein CLASP, which accumulates at specific cell edges, enables microtubule growth around sharp edges and promotes formation of microtubule bundles that span adjacent cell faces. By computationally modelling dynamic 'microtubules on a cube' with edges differentially permissive to microtubule passage, we show that the CLASP-edge complex is a 'tuneable' microtubule organizer, with the inherent flexibility to generate the numerous cortical array patterns observed in nature

Early mobilisation in mechanically ventilated patients:A systematic integrative review of definitions and activities

Current evidence suggests that early mobilisation of these patients can be an effective intervention in improving their outcomes. However, what constitutes early mobilisation in mechanically ventilated patients (EM-MV) remains unclear. We aimed to systematically explore the definitions and activity types of EM-MV in the literature. Whittemore and Knafl's framework guided this review. CINAHL, MEDLINE, EMBASE, PsycINFO, ASSIA, and Cochrane Library were searched to capture studies from 2000 to 2018, combined with hand search of grey literature and reference lists of included studies. The Critical Appraisal Skills Programme tools were used to assess the methodological quality of included studies. Data extraction and quality assessment of studies were performed independently by each reviewer before coming together in sub-groups for discussion and agreement. An inductive and data-driven thematic analysis was undertaken on verbatim extracts of EM-MV definitions and activities in included studies. Seventy-six studies were included from which four major themes were inferred: (1) , (2) , (3) and (4) . The first theme indicates that EM-MV is either not fully defined in studies or when a definition is provided this is not standardised across studies. The remaining themes reflect the diversity of EM-MV activities which depends on patients' characteristics and ICU settings; the negotiated decision-making process between patients and staff; and their interdependent relationship during the implementation. This review highlights the absence of an agreed definition and on what constitutes early mobilisation in mechanically ventilated patients.
