    Immersive Composition for Sensory Rehabilitation: 3D Visualisation, Surround Sound, and Synthesised Music to Provoke Catharsis and Healing

    There is a wide range of sensory therapies using sound, music and visual stimuli. Some focus on soothing or distracting stimuli such as natural sounds or classical music as analgesic, while other approaches emphasize the active performance of producing music as therapy. This paper proposes an immersive multi-sensory Exposure Therapy for people suffering from anxiety disorders, based on a rich, detailed surround-soundscape. This soundscape is composed to include the users’ own idiosyncratic anxiety triggers as a form of habituation, and to provoke psychological catharsis, as a non-verbal, visceral and enveloping exposure. To accurately pinpoint the most effective sounds and to optimally compose the soundscape we will monitor the participants’ physiological responses such as electroencephalography, respiration, electromyography, and heart rate during exposure. We hypothesize that such physiologically optimized sensory landscapes will aid the development of future immersive therapies for various psychological conditions, Sound is a major trigger of anxiety, and auditory hypersensitivity is an extremely problematic symptom. Exposure to stress-inducing sounds can free anxiety sufferers from entrenched avoidance behaviors, teaching physiological coping strategies and encouraging resolution of the psychological issues agitated by the sound

    The Effect of Cigarette Smoking on Diabetic Peripheral Neuropathy: A Systematic Review and Meta-Analysis.

    OBJECTIVE: Studies suggest that smoking may be a risk factor for the development of microvascular complications such as diabetic peripheral neuropathy (DPN). The objective of this study was to assess the relationship between smoking and DPN in persons with type 1 or type 2 diabetes. RESEARCH DESIGN AND METHODS: A systematic review of the PubMed, Embase, and Cochrane clinical trials databases was conducted for the period from January 1966 to November 2014 for cohort, cross-sectional and case-control studies that assessed the relationship between smoking and DPN. Separate meta-analyses for prospective cohort studies and case-control or cross-sectional studies were performed using random effects models. RESULTS: Thirty-eight studies (10 prospective cohort and 28 cross-sectional) were included. The prospective cohort studies included 5558 participants without DPN at baseline. During follow-up ranging from 2 to 10 years, 1550 cases of DPN occurred. The pooled unadjusted odds ratio (OR) of developing DPN associated with smoking was 1.26 (95% CI 0.86-1.85; I(2) = 74%; evidence grade: low strength). Stratified analyses of the prospective studies revealed that studies of higher quality and with better levels of adjustment and longer follow-up showed a significant positive association between smoking and DPN, with less heterogeneity. The cross-sectional studies included 27,594 participants. The pooled OR of DPN associated with smoking was 1.42 (95% CI 1.21-1.65; I(2) = 65%; evidence grade: low strength). There was no evidence of publication bias. CONCLUSIONS: Smoking may be associated with an increased risk of DPN in persons with diabetes. Further studies are needed to test whether this association is causal and whether smoking cessation reduces the risk of DPN in adults with diabetes

    Calcitonin inhibits phospholipase A2 and collagenase activity of human osteoarthritic chondrocytes

    SummaryCalcitonin (CT) is a known potent inhibitor of bone resorption but its effect on cartilage enzymatic degradation has been incompletely studied. Salmon CT, at a concentration of 0, 0.1, 0.25, 0.5, 2.5 and 50 ng/ml, was added at 24 or 72 h to the culture medium of chondrocytes from human osteoarthritic hips and knees. The spontaneous collagenolytic activity, measured using a radiolabeled type II collagen, was inhibited by CT in a dose-dependent manner. However, CT had no effect on the total collagenolytic activity assayed after APMA activation. Stromelysin and plasmin activity, measured by degradation of casein and a synthetic substrate, were also unaffected by CT. Chondrocyte phospholipase A2 activity, assayed using a labeled specific substrate, was decreased by CT. Chondrocyte pre-incubation with CT significantly decreased the cell binding of labeled TNFα, but did not affect IL-1β cell binding. Attachment of chondrocytes on fibronectin was markedly stimulated by CT, while attachment to type II collagen was not. Significant effects were obtained using at least 2 or 5 ng/ml of CT. CT appears to decrease collagenolytic activity by decreasing its activation and/or increasing its inhibition by tissue inhibitors of metalloproteinases (TIMP). CT might act on osteoarthritic chondrocyte activation via mechanisms such as phospholipase A2 activity, human necrosis factor-α or fibronectin receptor expression

    Canonical Forms in Interactive Exercise Assistants

    Interactive exercise assistants support students in practicing exercises, and acquiring procedural skills. Many mathematical topics can be practiced in such assistants. Ideally, an interactive exercise assistant not only validates final answers, but also comments on intermediate steps submitted by a student, provides hints on how to proceed, and presents worked-out examples. For these purposes, fine control over the symbolic simplification procedures of the underlying mathematical machinery is needed. In this paper, we introduce views for mathematical expressions. A view defines an equivalence relation by choosing a canonical form of mathematical expressions. We use views to track and recognize intermediate answers, to help in presenting expressions to a user, and to control the granularity of the steps in worked-out examples. We develop the concept of a view, discuss the laws it satisfies, and show how views are composed, which means that they can be used for multiple exercise classes.

    Meson-Baryon-Baryon Vertex Function and the Ward-Takahashi Identity

    Ohta proposed a solution for the well-known difficulty of satisfying the Ward-Takahashi identity for a photo-meson-baryon-baryon amplitude (γ\gammaMBB) when a dressed meson-baryon-baryon (MBB) vertex function is present. He obtained a form for the γ\gammaMBB amplitude which contained, in addition to the usual pole terms, longitudinal seagull terms which were determined entirely by the MBB vertex function. He arrived at his result by using a Lagrangian which yields the MBB vertex function at tree level. We show that such a Lagrangian can be neither hermitian nor charge conjugation invariant. We have been able to reproduce Ohta's result for the γ\gammaMBB amplitude using the Ward-Takahashi identity and no other assumption, dynamical or otherwise, and the most general form for the MBB and γ\gammaMBB vertices. However, contrary to Ohta's finding, we find that the seagull terms are not robust. The seagull terms extracted from the γ\gammaMBB vertex occur unchanged in tree graphs, such as in an exchange current amplitude. But the seagull terms which appear in a loop graph, as in the calculation of an electromagnetic form factor, are, in general, different. The whole procedure says nothing about the transverse part of the (γ\gammaMBB) vertex and its contributions to the amplitudes in question.Comment: A 20 pages Latex file and 16 Postscript figures in an uuencoded format.
