Panel discussion: Proposals for improving OCL
During the panel session at the OCL workshop, the OCL community discussed, stimulated by short presentations by OCL experts, potential future extensions and improvements of the OCL. As such, this panel discussion continued the discussion that started at the OCL meeting in Aachen in 2013 and on which we reported in the proceedings of the last year's OCL workshop. This collaborative paper, to which each OCL expert contributed one section, summarises the panel discussion as well as describes the suggestions for further improvements in more detail.Peer ReviewedPostprint (published version
Markov Chain Monte Carlo Random Effects Modeling in Magnetic Resonance Image Processing Using the BRugs Interface to WinBUGS
A common feature of many magnetic resonance image (MRI) data processing methods is the voxel-by-voxel (a voxel is a volume element) manner in which the processing is performed. In general, however, MRI data are expected to exhibit some level of spatial correlation, rendering an independent-voxels treatment inefficient in its use of the data. Bayesian random effect models are expected to be more efficient owing to their information-borrowing behaviour.
To illustrate the Bayesian random effects approach, this paper outlines a Markov chain Monte Carlo (MCMC) analysis of a perfusion MRI dataset, implemented in R using the BRugs package. BRugs provides an interface to WinBUGS and its GeoBUGS add-on. WinBUGS is a widely used programme for performing MCMC analyses, with a focus on Bayesian random effect models. A simultaneous modeling of both voxels (restricted to a region of interest) and multiple subjects is demonstrated. Despite the low signal-to-noise ratio in the magnetic resonance signal intensity data, useful model signal intensity profiles are obtained. The merits of random effects modeling are discussed in comparison with the alternative approaches based on region-of-interest averaging and repeated independent voxels analysis.

Treatment threshold for intra-operative hypotension in clinical practice-a prospective cohort study in older patients in the UK
Intra-operative hypotension frequently complicates anaesthesia in older patients and is implicated in peri-operative organ hypoperfusion and injury. The prevalence and corresponding treatment thresholds of hypotension are incompletely described in the UK. This study aimed to identify prevalence of intra-operative hypotension and its treatment thresholds in UK practice. Patients aged ≥ 65 years were studied prospectively from 196 UK hospitals within a 48-hour timeframe. The primary outcome was the incidence of hypotension (mean arterial pressure 20%; systolic blood pressure 20% reduction in systolic blood pressure from baseline and 77.5% systolic blood pressure <100 mmHg. The mean (SD) blood pressure triggering vasopressor therapy was mean arterial pressure 64.2 (11.6) mmHg and the mean (SD) stated intended treatment threshold from the survey was mean arterial pressure 60.6 (9.7) mmHg. A composite adverse outcome of myocardial injury, kidney injury, stroke or death affected 345 patients (7.3%). In this representative sample of UK peri-operative practice, the majority of older patients experienced intra-operative hypotension and treatment was delivered below suggested thresholds. This highlights both potential for intra-operative organ injury and substantial opportunity for improving treatment of intra-operative hypotension
Increased ICAM-1 Expression Causes Endothelial Cell Leakiness, Cytoskeletal Reorganization and Junctional Alterations
Tumor necrosis factor (TNF)-induced ICAM-1 in endothelial cells (EC) promotes leukocyte adhesion. Here we report that ICAM-1 also effects EC barrier function. Control- or E-selectin-transduced human dermal microvascular EC (HDMEC) form a barrier to flux of proteins and to passage of current (measured as transendothelial electrical resistance or TEER). HDMEC transduced with ICAM-1 at levels comparable to that induced by TNF show reduced TEER, but do so without overtly changing their cell junctions, cell shape, or cytoskeleton organization. Higher levels of ICAM-1 further reduce TEER, increase F/G-actin ratios, rearrange the actin cytoskeleton to cause cell elongation, and alter junctional zona occludens 1 and vascular endothelial-cadherin staining. Transducing with ICAM-1 lacking an intracellular region also reduces TEER. TNF-induced changes in TEER and shape follow a similar time course as ICAM-1 induction; however, the fall in TEER occurs at lower TNF concentrations. Inhibiting NF-κB activation blocks ICAM-1 induction; TEER reduction, and shape change. Specific small-interfering RNA knockdown of ICAM-1 partially inhibits TNF-induced shape change. We conclude that moderately elevated ICAM-1 expression reduces EC barrier function and that expressing higher levels of ICAM-1 affects cell junctions and the cytoskeleton. Induction of ICAM-1 may contribute to but does not fully account for TNF-induced vascular leak and EC shape change
The Effects of Thawing Procedure and Supplementation on the Motility and Viability of Frozen-thawed Boar Semen
The effect of two thawing procedures on frozen boar semen and supplementations to the fertilization media were studied. Frozen boar semen was thawed using either Percoll gradient or phosphate buffered saline (PBS)procedure. Supplementations were 1.0 mM L-glutamate, 1.0 mM N-acetylcysteine (NAC) , and 1.0 mM NAC-amide (NACA). Spermatozoa were analyzed for forward progressive motility (FPM) and viability every 0.5 h for 3 .0 h post-thawing. There were significantly (P \u3c 0.05) higher numbers of viable (76.0 ± 5.1 %) and FPM (30 .0 ± 2.4%) spermatozoa at 3.0 h post thawing using the PBS procedure compared to the Percoll gradient thawed spermatozoa (65.0 ± 3.9%; 10.0 ± 4.5 %, respectively). Supplementation of 1.0 mM L-glutamate, 1.0 mM NAC, or 1.0 mM NACA had no significant effect on spermatozoa viability regardless of the time post-thaw.Supplementation of 1.0 mM L-glutamate, 1.0 mM NAC , or 1.0 mM NACA had no significant effect on FPM up to 1.0 h post-thaw. Spermatozoa with no supplementation or 1.0 mM L-glutamate had significantly higher (P \u3c 0.05) FPM compared to the 1.0 mM NAC and 1.0 mM NACA supplemented groups at 1.5, 2.0, 2 .5, and 3.0 h post-thaw. There was no significant difference between no supplementation or 1.0 mM L-glutamate on FPM regardless of the time post-thaw. There was no significant difference between 1.0 mM NAC or 1.0 mM NACA on FPM regardless of the time post-thaw. These results indicate that thawing procedure has an effect on spermatozoa viability and FPM but supplementation does not have an effect on the overall viability of spermatozoa during thawing, but may reduce FPM
Adverse Events and Treatment Discontinuations of Antimuscarinics for the Treatment of Overactive Bladder in Older Adults: A Systematic Review and Meta-Analysis
Introduction
Antimuscarinics should be used with caution in older adults with overactive bladder (OAB) due to anticholinergic adverse events (AEs). Systematic reviews and meta-analyses (SRMAs) have analyzed safety-related outcomes but have not specified risk in the elderly, the population at highest risk for AEs. The aim of this review is to explore and evaluate AEs and treatment discontinuations in adults 65 or older taking antimuscarinics for OAB.
Methods
Keywords were searched in MEDLINE, EMBASE, SCOPUS, and Cochrane Central Register for Controlled Trials. Randomized controlled trials (RCTs) along with sub-analyses and pooled analyses that compared antimuscarinics to placebo or another antimuscarinic were performed in February 2015. Studies assessing AEs or treatment discontinuations in a population of adults 65 or older were included. The Jadad Criteria and McHarm Tool were used to assess the quality of the trials.
Results
A total of 16 studies met the inclusion criteria. Eighty AEs and 27 reasons for treatment discontinuation were described in the included studies and further explored. Anticholinergic AEs were more common in antimuscarinics compared to placebo. Incidence of dizziness, dyspepsia, and urinary retention with fesoterodine, headache with darifenacin, and urinary tract infections with solifenacin were significantly higher compared to placebo. Treatment discontinuation due to AEs and dry mouth were higher in the antimuscarinics when compared to placebo in older adults.
Conclusions
Treatment for overactive bladder using antimuscarinics in adults aged 65 or older resulted in significant increases in risk for several AEs compared to placebo including anticholinergic and non-anticholinergic AEs

Centaurus A: multiple outbursts or bursting bubble?
We present new radio observations of the brighter region of the northern lobe
(the Northern Middle Lobe, NML) of Centaurus A obtained at 20 cm with the
Australia Telescope Compact Array. The angular resolutions are ~50 and ~130
arcsec, therefore much higher than for the previously available radio images of
this region. The most interesting feature detected is a large-scale jet that
connects the inner radio lobe and the NML and that is imaged for the first
time. The NML itself appears as diffuse emission with a relatively bright ridge
on the eastern side. The radio morphology of Centaurus A and, in particular,
its NML could be the result of a precessing jet that has undergone a strong
interaction with the environment at least in the northern side. The very big
drop in intensity between the inner jet and the large-scale jet can be
explained with a sequence of bursts of activity at different epochs in the life
of the source. Alternatively (or additionally) a ``bursting bubble'' model is
proposed which could also explain the good collimation of the large-scale jet.
In this model, the plasma accumulated in the inner lobe would be able to
``burst'' out only through one nozzle that would be the region where the
large-scale jet forms. From the comparison between the radio emission and the
regions of ionized gas we find that the inner optical filament falls about 2
arcmin (~2 kpc) away from the large-scale radio jet. Thus, this filament does
not seem to have experienced a direct interaction with the radio plasma. The
outer filaments appear to be, in projection, closer to the radio emission,
arguing for a direct interaction with the radio jet. However, also in this case
