    Factors predicting physical activity among children with special needs.

    IntroductionObesity is especially prevalent among children with special needs. Both lack of physical activity and unhealthful eating are major contributing factors. The objective of our study was to investigate barriers to physical activity among these children.MethodsWe surveyed parents of the 171 children attending Vista Del Mar School in Los Angeles, a nonprofit school serving a socioeconomically diverse group of children with special needs from kindergarten through 12th grade. Parents were asked about their child's and their own physical activity habits, barriers to their child's exercise, and demographics. The response rate was 67%. Multivariate logistic regression was used to examine predictors of children being physically active at least 3 hours per week.ResultsParents reported that 45% of the children were diagnosed with attention deficit hyperactivity disorder, 38% with autism, and 34% with learning disabilities; 47% of children and 56% of parents were physically active less than 3 hours per week. The top barriers to physical activity were reported as child's lack of interest (43%), lack of developmentally appropriate programs (33%), too many behavioral problems (32%), and parents' lack of time (29%). However, child's lack of interest was the only parent-reported barrier independently associated with children's physical activity. Meanwhile, children whose parents were physically active at least 3 hours per week were 4.2 times as likely to be physically active as children whose parents were less physically active (P = .01).ConclusionIn this group of students with special needs, children's physical activity was strongly associated with parental physical activity; parent-reported barriers may have had less direct effect. Further studies should examine the importance of parental physical activity among children with special needs

    A structured management approach to implementation of health promotion interventions in Head Start.

    Improving the health and health literacy of low-income families is a national public health priority in the United States. The federal Head Start program provides a national infrastructure for implementation of health promotion interventions for young children and their families. The Health Care Institute (HCI) at the Anderson School of Management at the University of California, Los Angeles, developed a structured approach to health promotion training for Head Start grantees using business management principles. This article describes the HCI approach and provides examples of implemented programs and selected outcomes, including knowledge and behavior changes among Head Start staff and families. This prevention-focused training platform has reached 60,000 Head Start families in the United States since its inception in 2001. HCI has demonstrated consistent outcomes in diverse settings and cultures, suggesting both scalability and sustainability

    Tumor Biology and Racial Disparities in Reconstruction After Mastectomy: A SEER Database Analysis

    Introduction: Significant disparities in immediate breast reconstruction after mastectomy have persisted, and may even be increasing, despite large-scale efforts to minimize them, such as the Women’s Health and Cancer Rights Act of 1998. Immediate breast reconstruction has been shown to lead to higher rates of surgical satisfaction, minimize delay in post-operative cancer treatment, and improve the quality of life and overall well-being of mastectomy patients. However only 25-40% of eligible women in the United States receive reconstruction. The rate of reconstruction is even lower in African American and Hispanic women compared to White women. To better understand this disparity, this study uses national population-based data to examine how demographic factors, socioeconomic factors, and disease characteristics interact and affect the rate of immediate breast reconstruction (IBR) after mastectomy. Methods: Women with AJCC7 Stage 0-III breast cancer who underwent mastectomy from 2010 to 2012 were identified in the Surveillance, Epidemiology and End Results Program (SEER) database. Race, Hispanic ethnicity, age, marital status, insurance status, tumor grade, AJCC7 stage and hormone receptor/ Her2Neu profile were compared between women undergoing mastectomy with IBR and mastectomy alone using univariate and multivariate analysis. Results: We identified 51,115 women who underwent mastectomy for Stage 0-III breast cancer from 2010-2012, of whom 15,389 (30.1%) received IBR. On multivariate analysis, age (p Conclusion: The decision to undergo reconstruction after mastectomy is influenced by many factors. Our results show that even after adjusting for tumor characteristics, socioeconomic factors are independently associated with receiving IBR after mastectomy. Further research is needed to elucidate the factors that influence the decision to undergo IBR in order to eliminate these persistent disparities

    Common Genetic Variant Association with Altered HLA Expression, Synergy with Pyrethroid Exposure, and Risk for Parkinson's Disease: An Observational and Case-Control Study.

    Background/objectivesThe common non-coding single nucleotide polymorphism (SNP) rs3129882 in HLA-DRA is associated with risk for idiopathic Parkinson's disease (PD). The location of the SNP in the major histocompatibility complex class II (MHC-II) locus implicates regulation of antigen presentation as a potential mechanism by which immune responses link genetic susceptibility to environmental factors in conferring lifetime risk for PD.MethodsFor immunophenotyping, blood cells from 81 subjects were analyzed by qRT-PCR and flow cytometry. A case-control study was performed on a separate cohort of 962 subjects to determine association of pesticide exposure and the SNP with risk of PD.ResultsHomozygosity for G at this SNP was associated with heightened baseline expression and inducibility of MHC class II molecules in B cells and monocytes from peripheral blood of healthy controls and PD patients. In addition, exposure to a commonly used class of insecticide, pyrethroids, synergized with the risk conferred by this SNP (OR = 2.48, p = 0.007), thereby identifying a novel gene-environment interaction that promotes risk for PD via alterations in immune responses.ConclusionsIn sum, these novel findings suggest that the MHC-II locus may increase susceptibility to PD through presentation of pathogenic, immunodominant antigens and/or a shift toward a more pro-inflammatory CD4+ T cell response in response to specific environmental exposures, such as pyrethroid exposure through genetic or epigenetic mechanisms that modulate MHC-II gene expression

    The Mass Power Spectrum in Quintessence Cosmological Models

    We present simple analytic approximations for the linear and fully evolved nonlinear mass power spectrum for spatially flat cold dark matter (CDM) cosmological models with quintessence (Q). Quintessence is a time evolving, spatially inhomogeneous energy component with negative pressure and an equation of state w_Q < 0. It clusters gravitationally on large length scales but remains smooth like the cosmological constant on small length scales. We show that the clustering scale is determined by the Compton wavelength of the Q-field and derive a shape parameter, \Gamma_Q, to characterize the linear mass power spectrum. The growth of linear perturbations as functions of redshift, w_Q, and matter density \Omega_m is also quantified.     Three-dimensional Characterization of Interorganelle Contact Sites in Hepatocytes using Serial Section Electron Microscopy

    Transmission electron microscopy has been long considered to be the gold standard for the visualization of cellular ultrastructure. However, analysis is often limited to two dimensions, hampering the ability to fully describe the three-dimensional (3D) ultrastructure and functional relationship between organelles. Volume electron microscopy (vEM) describes a collection of techniques that enable the interrogation of cellular ultrastructure in 3D at mesoscale, microscale, and nanoscale resolutions. This protocol provides an accessible and robust method to acquire vEM data using serial section transmission EM (TEM) and covers the technical aspects of sample processing through to digital 3D reconstruction in a single, straightforward workflow. To demonstrate the usefulness of this technique, the 3D ultrastructural relationship between the endoplasmic reticulum and mitochondria and their contact sites in liver hepatocytes is presented. Interorganelle contacts serve vital roles in the transfer of ions, lipids, nutrients, and other small molecules between organelles. However, despite their initial discovery in hepatocytes, there is still much to learn about their physical features, dynamics, and functions. Interorganelle contacts can display a range of morphologies, varying in the proximity of the two organelles to one another (typically ~10-30 nm) and the extent of the contact site (from punctate contacts to larger 3D cisternal-like contacts). The examination of close contacts requires high-resolution imaging, and serial section TEM is well suited to visualize the 3D ultrastructural of interorganelle contacts during hepatocyte differentiation, as well as alterations in hepatocyte architecture associated with metabolic diseases

    Simple Local Computation Algorithms for the General Lovasz Local Lemma

    We consider the task of designing Local Computation Algorithms (LCA) for applications of the Lov\'{a}sz Local Lemma (LLL). LCA is a class of sublinear algorithms proposed by Rubinfeld et al.~\cite{Ronitt} that have received a lot of attention in recent years. The LLL is an existential, sufficient condition for a collection of sets to have non-empty intersection (in applications, often, each set comprises all objects having a certain property). The ground-breaking algorithm of Moser and Tardos~\cite{MT} made the LLL fully constructive, following earlier results by Beck~\cite{beck_lll} and Alon~\cite{alon_lll} giving algorithms under significantly stronger LLL-like conditions. LCAs under those stronger conditions were given in~\cite{Ronitt}, where it was asked if the Moser-Tardos algorithm can be used to design LCAs under the standard LLL condition. The main contribution of this paper is to answer this question affirmatively. In fact, our techniques yield LCAs for settings beyond the standard LLL condition

With the advances in computing technologies, both in hardware and software, there is the opportunity to bring together the vast amount of engineering, hazard and risk data and handle them in an efficient way to manage and improve safety throughout the life time of a plant. This paper proposes keeping such data in a process plant safety information repository which can be easily accessed by different safety-related tools to enhance the efficiency of safety identification and analysis tasks
