    Radiofrequency ablation versus hepatic resection for hepatocellular carcinoma within the Milan criteria – A comparative study

    AbstractBackgroundTo compare the results of radiofrequency ablation (RFA) with hepatic resection in the treatment of hepatocellular carcinoma (HCC) within the Milan criteria.MethodsA nonrandomized comparative study was performed with 111 consecutive patients who underwent laparoscopic RFA (n = 31) or curative hepatic resection (n = 80) for HCC within Milan criteria.ResultsProcedure related complications were less often and severe after RFA than resection (3.2% vs. 25%). There was no significant difference in hospital mortality (0% vs. 3.8%). Hospital stay was significantly shorter in the RFA group than in the resection group (mean, 3.8 vs. 6.8 days). The 1-, 3-, and 5-year disease-free survival rates for the RFA group and the resection group were 76%, 40%, 40% and 76%, 60%, 60%, respectively. Disease-free survival was significantly lower in the RFA group than in the resection group. The corresponding 1-, 3-, and 5-year overall survival rates for the RFA group and the resection group were 100%, 92%, 84%, and 92%, 75%, 71%, respectively. The overall survival for RFA and resection were not significantly different.ConclusionsOur result showed comparable overall survival between RFA and surgery, although RFA was associated with a significantly higher tumor recurrence rate. RFA had the advantages over surgical resection in being less invasive and having lower morbidity

    Synthesis of Cobalt Nanowires on Porous Anodic Alumina Template Using Electrochemical Deposition

    Electrochemical deposition has been widely used for synthesis of metal nanowires (NWs) on the porous template. In this paper, the effect of potential and electrolyte concentration on cobalt (Co) NWs formation through porous anodic alumina template has been investigated using direct-current electrodepostion at 0.75~2 V together with the high 0.5 M and low 0.1 M cobalt sulfurate based electrolyte. Scanning electron microscopy and grazing incidence X-ray diffraction were used to examine the nanostructure, morphology and phase of Co NWs. The current vs time curve was recorded for understanding the growth behavior. Too low potential of 0.75 V is not favored for Co NWs formation due to insufficient driving force while too high potential of 2 V ruins the NWs growth owing to hydrogen generation in reduction reaction. The uniform crystalline Co NWs can be obtained by the proper potential of 1V and concentration of 0.5 M at an average growth rate of 964 nm/min

    A Performance Evaluation of Introducing Balanced Scorecard to High-tech Related Industries in Taiwan

    Balanced Scorecard (BSC) has become the most desirable performance evaluation tool for industries in Taiwan; however, difficulty or bad performance of system introduction has occurred due to an incomplete understanding of the implementation approaches and correct objectives of the BSC system, causing the risk of cost loss. Two domestic high-tech companies in the high-tech related industry were served as the object of study in this research. The contents of four perspectives of the BSC were converted to twenty key performance evaluation indicators in terms of modern business administration as the variables in the research. Based on the DMAIC model, the importance and satisfaction of BSC implementation factors in high-tech related industry are defined first. The performance indices of satisfaction and importance of implementing BSC are standardized by fuzzy methods for evaluation and a performance matrix with the target line and upper and lower performance control lines are established. Management can analyze the performance level and compare the performance indices and matrixes of two companies after introducing BSC according to the coordinates of satisfaction and importance of implementation factors in the matrixes. These two-dimension matrices will then be converted to one-dimension coordinates for cross performance matrices of four quadrants. Next, performance improvement strategies will be devised in accordance with the aspects of the theory of constraints. After carrying out improvement strategies, the cross performance matrix will be constructed to verify the effect and ascertain the factors of bad performance. In this way, improvement strategies can be re-devised and the most appropriate distribution of resources will be made to sustain the optimum state of ability and cost during the process of introducing the BSC system. Requirements for a short period of time and low cost to evaluate the performance of BSC introduction can be met via this simple and convenient evaluation model presented in this research. Resources will be invested to enhance satisfaction for the implementation factors of high importance and low satisfaction. Likewise, resources will be adjusted to reduce the cost of system introduction for the implementation factors of low importance and high satisfaction. As a result, the time efficiency of introducing the BSC system will be promoted effectively

    Gene Expression Profiles as Markers of Aggressive Disease-EGFR as a Factor

    We previously reported that 43 (58%) of 75 head and neck squamous cell carcinoma (HNSCC) tumors harbor increased epidermal growth factor receptor (EGFR) gene copy numbers as determined by fluorescent in situ hybridization. In this study, an increased EGFR copy number was associated with decreased progression-free survival and overall survival of HNSCC patients. However, activated EGFR protein levels are difficult to quantify by immunohistochemistry and are subject to dynamic regulation, specifically receptor downregulation on ligand binding. Therefore, we generated an activated EGFR gene expression signature in an in vitro HaCaT keratinocyte model system to further study genes involved in the EGFR signaling pathway in HNSCC. The results from this model system have suggested that the activated EGFR signature might reflect the activated state of the EGFR pathway in human HNSCC tumors and that it is associated with the increased EGFR gene copy number by fluorescent in situ hybridization. Furthermore, the activated EGFR signature has provided additional leads, because they are related to co-regulated molecular pathways and associated gene products on activation of EGFR. These could be exploited to refine and optimize combination therapies to be used in conjunction with available EGFR inhibitors in individual HNSCC patients

    Absence of hole pairing in a simple t-J model on the Shastry-Sutherland lattice

    The Shastry-Sutherland model is a two-dimensional frustrated spin model whose ground state is a spin gap state. We study this model doped with one and two holes on a 32-site lattice using exact diagonalization. When t'>0, we find that the diagonal dimer order that exists at half-filling are retained at these moderate doping levels. No other order is found to be favored on doping. The holes are strongly repulsive unless the hopping terms are unrealistically small. Therefore, the existence of a spin gap at half-filling does not guarantee hole-pairing in the present case

    Coloured network motifs are small subgraphs that enable to discover and interpret the patterns of interaction within the complex networks. The analysis of three-nodes motifs where the colour of the node reflects its high – white node or low – black node centrality in the social network is presented in the paper. The importance of the vertices is assessed by utilizing two measures: degree prestige and degree centrality. The distribution of motifs in these two cases is compared to mine the interconnection patterns between nodes. The analysis is performed on the social network derived from email communication

    Insulin-like growth factor-1 receptor inhibitor, AMG-479, in cetuximab-refractory head and neck squamous cell carcinoma

    Background Recurrent head and neck squamous cell carcinoma (HNSCC) remains a difficult cancer to treat. Here, we describe a patient with HNSCC who had complete response to methotrexate (MTX) after progressing on multiple cytotoxic agents, cetuximab, and AMG-479 (monoclonal antibody against insulin-like growth factor-1 receptor [IGF-1R]). Methods The clinical information was collected by a retrospective medical record review under an Institutional Review Board-approved protocol. From 4 tumors and 2 normal mucosal epithelia, global gene expression, and IGF-1R and dihydrofolate reductase (DHFR) protein levels were determined. Results Effective target inhibition in the tumor was confirmed by the decreased protein levels of total and phospho-IGF-1R after treatment with AMG-479. Decreased level of DHFR and conversion of a gene expression profile associated with cetuximab-resistance to cetuximab-sensitivity were also observed. Conclusion This suggests that the combination of AMG-479 and MTX or cetuximab may be a promising therapeutic approach in refractory HNSCC

    Crystal Structures of the HslVU Peptidase–ATPase Complex Reveal an ATP-Dependent Proteolysis Mechanism

    AbstractBackground: The bacterial heat shock locus HslU ATPase and HslV peptidase together form an ATP-dependent HslVU protease. Bacterial HslVU is a homolog of the eukaryotic 26S proteasome. Crystallographic studies of HslVU should provide an understanding of ATP-dependent protein unfolding, translocation, and proteolysis by this and other ATP-dependent proteases.Results: We present a 3.0 Å resolution crystal structure of HslVU with an HslU hexamer bound at one end of an HslV dodecamer. The structure shows that the central pores of the ATPase and peptidase are next to each other and aligned. The central pore of HslU consists of a GYVG motif, which is conserved among protease-associated ATPases. The binding of one HslU hexamer to one end of an HslV dodecamer in the 3.0 Å resolution structure opens both HslV central pores and induces asymmetric changes in HslV.Conclusions: Analysis of nucleotide binding induced conformational changes in the current and previous HslU structures suggests a protein unfolding–coupled translocation mechanism. In this mechanism, unfolded polypeptides are threaded through the aligned pores of the ATPase and peptidase and translocated into the peptidase central chamber
