Pomiar ciśnienia tętniczego w domu przez pacjenta jako najlepsza metoda długotrwałego monitorowania skuteczności terapii hipotensyjnej
Pomiar ciśnienia tętniczego przez pacjentów w domu jest coraz częściej
wykorzystywany w monitorowaniu skuteczności terapii hipotensyjnej.
Ponadto, domowy pomiar ciśnienia jest jedną z metod poprawiających
współpracę chorego z lekarzem oraz stopień kontroli nadciśnienia tętniczego.
Konieczne wydaje się zaproponowanie schematu, dzięki któremu
wartości zmierzone przez chorych będą rzeczywistym i powtarzalnym
odwzorowaniem ciśnienia tętniczego mierzonego poza gabinetem
lekarskim. W wielu badaniach, w których oceniano domowe pomiary
ciśnienia tętniczego, wykazano ich przydatność w ocenie ryzyka sercowo-
naczyniowego. Do optymalnego wykorzystania pomiarów domowych
w praktyce konieczne są proste i zrozumiałe dla pacjentów wytyczne
wskazujące, jak stosować tę metodę na co dzień. Pacjenci oraz
lekarze powinni być poinformowani, jak często, o jakich porach dnia
oraz ile pomiarów w czasie jednej sesji należy wykonywać, aby w pełni
wykorzystać możliwości metody
Prevalence of hypertension and metabolic disturbances in a population-based cohort of postmenopausal women. The Gniewkowo Women Study
Background Metabolic syndrome (MS) constitutes a major determinant of cardiovascular (CV) morbidity and mortality in adults. This phenomenon in women before menopause is attenuated by gonadal hormonal activity. After menopause the CV disease (CVD) incidence among women increases dramatically, and often overtakes the prevalence observed among men. Little is known about the CV-risk characteristic of early postmenopausal women in
our country. Therefore the aim of our study was to investigate the frequency and distribution of metabolic syndrome components in early postmenopausal women residing urban-rural area in central Poland.
Material and Methods All women aged 65–75 years, opted for one general practice form an urban-rural area in Central Poland were invited to participate. The anthropometric measures, blood pressure and blood sampling were performed. For the purpose of the study, the modified NCEP ATP III criteria for the diagnosis of metabolic syndrome were used.
Results 59.5% of eligible women accepted the invitation. Additional 5.2% of randomly selected women from initially non-responders group were added to the investigated population. In the studied group only 15.7% had normal body weight and 82.4% met the criteria for abdominal obesity (92% according to IDF). 81.9% of women were characterized by hypertension, even more met the NCEP ATP III criterion for elevated blood pressure (BP) (91.5%). Only 24.2% of studied women were treated effectively; moreover, despite treatment with three antihypertensive drugs 17.4% hypertensive women did not reach target BP values. Over one quarter of studied women suffered from diabetes mellitus, among whom 25.5% were not aware of the presence of the disease. Another 37.1% of women presented with prediabetic condition. Lipid disturbances were noted in the 25.3% and 18.4% of women for triglycerides (TG) increase and low HDL-C levels, respectively. According to modified NCEP ATP III criteria we were able to diagnose 58.8% of studied women with metabolic syndrome. Additionally, 31.9% of postmenopausal women were active cigarette smokers.
Conclusions Metabolic profile of early postmenopausal women in central Poland is strikingly abnormal. High incidence of metabolic syndrome is mainly determined by extremely prevalent increased blood pressure and abdominal obesity. With relation to all identified metabolic syndrome components, the strategy aimed at the improvement of body mass status, diabetes diagnosis and especially blood pressure control appear to be of highest priority

Wstęp Nadciśnienie tętnicze i otyłość brzuszna są
jednymi z czynników ryzyka sercowo-naczyniowego.
Istnieją dwie równoległe definicje otyłości
brzusznej. Third Report of the National Cholesterol
Education Program Expert Panel on Detection, Evaluation,
and Treatment of High Blood Cholesterol in
Adults (ATP III) zaleca rozpoznawanie otyłości przy
obwodzie pasa powyżej 102 cm dla mężczyzn i powyżej
88 cm dla kobiet, natomiast International Diabetes
Federation (IDF) odpowiednio: 94 cm i poniżej
80 cm. W nowych wytycznych Polskiego Towarzystwa
Nadciśnienia Tętniczego przyjęto definicję proponowaną
przez IDF. Celem pracy było określenie
wpływu zmiany kryteriów rozpoznawania otyłości
brzusznej na częstość jej występowania u chorych
z nadciśnieniem tętniczym.
Materiał i metody Pacjentom zgłaszającym się do
wybranych gabinetów lekarzy rodzinnych w ramach
badania IDEA wykonano badanie podmiotowe
i przedmiotowe, w tym pomiar obwodu pasa, według
standardów obowiązujących we wszystkich
ośrodkach. Analizowano częstość występowania otyłości
brzusznej wśród osób z nadciśnieniem tętniczym
według kryteriów ATP III oraz IDF.
Wyniki Zbadano 5371 osób (2024 mężczyzn, 3347
kobiet). Według kryteriów ATP III otyłość brzuszną
stwierdzono u 36,7% kobiet bez nadciśnienia tętniczego
oraz u 75,5% z nadciśnieniem tętniczym (p <
0,001); wśród mężczyzn odpowiednio u 23,1% oraz
54% (p < 0,001). Według kryteriów IDF otyłość
brzuszna występowała u 60,9% kobiet bez nadciśnienia
tętniczego oraz 91,2% kobiet z nadciśnieniem
tętniczym (p < 0,001) oraz u mężczyzn odpowiednio
u 51,1% oraz 80,2% (p < 0,001). Stosowanie
kryteriów IDF spowodowało wzrost częstości otyłości
brzusznej wśród chorych z nadciśnieniem tętniczym
z 67,2% do 86,9%. Zarówno dla kryteriów ATP
III oraz IDF częstość otyłości brzusznej wzrastała
z wiekiem do 70. roku życia u kobiet z nadciśnieniem
tętniczym. W grupie mężczyzn zależność ta
istniała jedynie dla kryteriów ATP III. Różnice częstości
występowania otyłości brzusznej dla obu kryteriów
były znamienne we wszystkich przedziałach
wiekowych, szczególnie w młodszych grupach.
Wnioski Nieprawidłową masę ciała stwierdza się
u 2/3 osób zgłaszających się do lekarza. Otyłość
brzuszna, według ATP III, występuje dwukrotnie
częściej u chorych z nadciśnieniem tętniczym niż
z prawidłowym ciśnieniem tętniczym. Kryteria IDF
prowadzą do wzrostu częstości występowania otyłości
brzusznej wśród chorych z nadciśnieniem tętniczym
z 67,2% do 86,9%.

cardiovascular risk factors. There are two parallel definitions
of abdominal obesity — the ATP III defines abdominal
obesity as a waist circumference more than 102 cm
for men and 88 cm for women, whereas the IDF criteria
are 94 cm for men and 80 cm for women. In new PTNT
guidelines, the IDF criteria are recommended. The aim
of our paper is to asses the influence of change in definition
on abdominal obesity prevalence among hypertensive
patients.
Material and methods We studied consecutive patients
visiting general practitioners enrolled into the IDEA Study.
Subjects underwent physical examination with waist circumference
assessment. The prevalence of abdominal
obesity among hypertensives was analyzed according to
the ATP III and IDF criteria.
Results Fife thousand three hundred seventy one patients
were included into the study (2024 men, 3347 women).
According to the ATP criteria, visceral obesity was more
prevalent in hypertensives than in normotensives (75.5%
vs. 36.7% in women, and 54.0% vs. 23.1 in men; p < 0.001
for both comparisons). Similarly, hypertension was associated
with higher prevalence of visceral obesity when IDF
criteria were used (91.2% vs. 60.9% in women, and 80.2%
vs. 51.1 in men; p < 0.001 for both comparisons). Application
of IDF criteria increased abdominal obesity prevalence
increase among hypertensives from 67.2% to 86.9%.
Application of both ATP III and IDF criteria resulted in
significant increase in prevalence of abdominal obesity
among hypertensive women in all age groups, whereas for
men it was true only for the ATP III criteria. The difference
between abdominal obesity prevalence according to
the ATP III and IDF criteria was significant in all age
groups, especially among younger patients.
Conclusions Two thirds of Polish patients are either overweight
or obese. Abdominal obesity, according to ATP III
criteria is twice more prevalent among hypertensives than
subjects with normal blood pressure. The application of
IDF criteria increases abdominal obesity prevalence among
Polish hypertensives from 67.2% to 86.9%.

Jak poprawić stopień przestrzegania zaleceń terapeutycznych i jakość współpracy lekarz - pacjent
Good cooperation between patient and physician is a very important part of treatment, especially in the case of chronic diseases. Previous studies conducted by the World Health Organization show that, on average, every second patient doesn’t follow therapeutic recommendations. In Poland, this percentage is even higher, and in the case of some diseases exceeds 70%. Importantly, these results are based primarily on patient statements, obtained by using questionnaire reviews, so in practice, the percentage of not properly cooperating patients may be even larger.The reasons for this phenomenon lie both on the patients and health care professionals side. The greatest impact on patients health behavior have psychological and socio-economical factors. First group includes primarily cognitive function, life satisfaction, personality, sense of control and mental state. The second group is associated mainly with the material status, but as the cyclic surveys on the Polish population show, the impact of income on treatment adherence from year to year is becoming smaller. Causes related with Health Service concern invalid communication between doctor and patient as well as lack of patient’s involvement in setting plan of therapy.Previous studies indicate how important is the quality of the relationship between physician and patient. Healthcare professionals should recognize patient’s needs and possibilities and fit treatment process to them. Better cooperation can be achieved by guiding motivation dialogue and patient’s engagement in therapy plan determination.Dobra współpraca lekarza z pacjentem jest bardzo ważnym elementem leczenia, zwłaszcza w przypadku chorób przewlekłych. Dotychczasowe badania prowadzone przez Światową Organizację Zdrowia wskazują, że przeciętnie co drugi chory nie przestrzega prawidłowo zaleceń terapeutycznych. W Polsce odsetek ten jest jeszcze wyższy i w przypadku niektórych chorób sięga ponad 70%. Co ważne, wyniki te opierają się przede wszystkim na deklaracjach pacjentów uzyskanych na postawie kwestionariuszowych narzędzi badawczych, zatem w praktyce odsetek chorych niewspółpracujących w sposób prawidłowy może być jeszcze większy.
Przyczyny tego zjawiska leżą zarówno po stronie pacjentów, jak i pracowników służby zdrowia. Na chorych najbardziej wpływają czynniki psychologiczne oraz społeczno-ekonomiczne. Do tych pierwszych należy zaliczyć przede wszystkim funkcjonowanie poznawcze, satysfakcję z życia, osobowość, poczucie kontroli oraz stan psychiczny. Druga grupa wiąże się przede wszystkim z sytuacją materialną, jednak — jak pokazują cykliczne badania w polskiej populacji — wpływ dochodów na przestrzeganie zaleceń terapeutycznych z roku na rok jest coraz mniejszy. Powody związane z opieką medyczną to przede wszystkim nieprawidłowa komunikacja z lekarzem i nieangażowanie chorego w ustalanie planu terapii.
Dotychczasowe wyniki badań wskazują, jak istotna dla przestrzegania zaleceń terapeutycznych jest jakość relacji lekarz–pacjent. Pracownicy służby zdrowia powinni poznać chorego i dostosować proces leczenia do jego potrzeb i możliwości. Polepszenie współpracy można osiągnąć, prowadząc dialog motywujący i angażując chorego w ustalanie planu terapii
Prevalence of postural hypotension in patients with treated hypertension
Wstęp Uważa się, że do występowania ortostatycznych
spadków ciśnienia tętniczego (OH) predysponują
nadciśnienie tętnicze, terapia hipotensyjna oraz
podeszły wiek badanych. Celem niniejszej pracy było
ustalenie związku między częstością OH a wiekiem,
płcią, BMI, otyłością brzuszną i glikemią u pacjentów
z nadciśnieniem tętniczym.
Materiał i metody Badaniami objęto 250 osób (125
kobiet i 125 mężczyzn) z nadciśnieniem tętniczym
w wywiadzie. Ciśnienie tętnicze mierzono w pozycji
siedzącej i stojącej po 1 minucie pionizacji. Analizowano
częstość spadku skurczowego ciśnienia tętniczego
(SBP) o 20 i 10 mm Hg.
Wyniki Spadek ciśnienia tętniczego po pionizacji nie
zależał od wieku, płci, stopnia i rodzaju otyłości oraz
od obecności zaburzeń gospodarki węglowodanowej.
Spadek SBP o 10 mm Hg występował znamiennie częściej w grupie chorych powyżej 60 rż. oraz w grupie
z BMI powyżej 30 kg/m2. Zaobserwowano tendencję
do częstszego występowania spadku SBP
o 10 mm Hg w grupie z otyłością brzuszną i zaburzeniami
tolerancji glukozy. Nie stwierdzono innych
zależności między spadkiem ciśnienia tętniczego po
pionizacji a badanymi zmiennymi.
Wnioski 1. Kryterium spadku SBP o 20 mm Hg
wydaje się zbyt mało czułym wskaźnikiem rozpoznania
OH u chorych z nadciśnieniem tętniczym.
2. Przy zastosowaniu kryterium spadku SBP
o 10 mm Hg zaobserwowano znamienny wpływ wieku
i BMI, ale nie otyłości brzusznej, na częstość OH.
3. Wydaje się, że OH u chorych z leczonym nadciśnieniem
tętniczym nie jest już tak częstym problemem
klinicznym jak podaje dotychczasowe piśmiennictwo.
Można przypuszczać, iż jest to efektem
stosowania leków o korzystniejszym profilu farmakokinetycznym.Background The presence of hypertension, antihypertensive
therapy and elderly age are well known as common
causes of orthostatic hypotension. The aim of the study
was to evaluate the relationship between postural hypotension
and age, gender, BMI, visceral obesity and glucose
tolerance in hypertensive patients.
Material and methods A total of 250 patients (125 females
and 125 males) with history of hypertension were recruited
for the study. The collected data included standing and
supine blood pressure measurements, glucose level, BMI
and waist circumference. The data were analyzed using
criteria of 20 mm Hg and 10 mm Hg SBP decreases in
response to standing.
Results Mean blood pressure decrease did not depend on
age, sex, BMI, presence of visceral obesity or glucose level.
SBP pressure decrease of 10 mm Hg was more prevalent
in older (> 60 yrs) and obese (BMI > 30 kg/m2) patients.
There was a trend towards the correlation of 10 mm Hg
SBP decrease with visceral obesity and impaired glucose
levels. There were no other statistically significant correlation
between PH and investigated variables.
Conclusions 1. SBP decrease of 20 mm Hg is not sensitive
enough to diagnose postural hypotension among patients
with hypertension. 2. Prevalence of 10 mm Hg SBP decrease
in response to standing is related to age and BMI,
but not to visceral obesity or to blood glucose level. 3.
Prevalence of orthostatic hypotension in the present study
was less common than that suggested by earlier studies.
This might be attributed to more favorable farmacokinetic
profile of newer anti-hypertensive drugs

Wstęp Występowanie zespołu metabolicznego wiąże
się ze zwiększonym ryzykiem sercowo-naczyniowym.
Celem niniejszej pracy była ocena częstości zespołu
metabolicznego w populacji chorych na nadciśnienie
tętnicze w zależności od płci.
Materiał i metody Badaniem objęto grupę 572 chorych
z nadciśnieniem tętniczym (275 kobiet i 297 mężczyzn). Średnia wieku badanej grupy chorych wynosiła
53,0 ± 14,6 roku (zakres 18-85 lat), a wskaźnik
masy ciała 28,5 ± 5,0 kg/m2. Definicję zespołu metabolicznego
przyjęto zgodnie z zaleceniami ATP III.
Wyniki Otyłość brzuszną stwierdzono u 44,8% chorych.
Otyłość centralna dotyczyła aż 50,9% kobiet
i 39,2% badanych mężczyzn (p < 0,05). Przynajmniej
jedną dodatkową - poza nadciśnieniem tętniczym
- składową zespołu metabolicznego zaobserwowano
u 66% kobiet i aż u 75% mężczyzn (p < 0,01). Zespół
metaboliczny, rozpoznawany na podstawie kryteriów
ATP III, stwierdzono u 40,5% badanych. Częstość
zespołu metabolicznego (co najmniej 3 kryteria) była
znamiennie większa u mężczyzn (45%) niż u kobiet
(35%) (p < 0,01).
Wnioski 1. Zespół metaboliczny, rozpoznawany na
podstawie kryteriów ATP III, występuje u chorych
z nadciśnieniem tętniczym dwukrotnie częściej niż
w populacji ogólnej. Relatywny wzrost częstości zespołu
metabolicznego u chorych z nadciśnieniem tętniczym
jest większy w wypadku mężczyzn niż kobiet. 2. Istnieją
znamienne różnice dotyczące częstości poszczególnych
składowych zespołu metabolicznego u kobiet
i mężczyzn z nadciśnieniem tętniczym. Otyłość brzuszna
występuje częściej u kobiet, a zaburzenia gospodarki
węglowodanowej, hipertriglicerydemia i niskie stężenia
cholesterolu frakcji HDL są znamiennie częstsze
u mężczyzn z nadciśnieniem tętniczym.Background Metabolic syndrome is associated with increased
cardiovascular risk. The aim of the present study
was to evaluate prevalence of metabolic syndrome in hypertensive
patients with respect to gender.
Material and methods We enrolled 572 patients with essential
hypertension (275 females and 297 males). Mean age of
the subjects was 53.0 ± 14.6 years (range 18-85 years), and
mean body mass index 28.5 ± 5.0 kg/m2. Metabolic syndrome
was diagnosed according to ATP III criteria.
Results Abdominal obesity was found in 44,8% of the patients.
Central obesity was detected in 50.9% of females
and in 39,2% of males (p < 0.05). At least one component
of the syndrome, additional to hypertension, was observed
in 66% of females and in 75% of male hypertensives
(p < 0.01). Metabolic syndrome, defined by the ATP III
criteria, was found in 40,5% of the subjects. Metabolic syndrome
(at least 3 criteria) was more prevalent in males
(45%) than in females (35%) (p < 0.01).
Conclusions 1. In hypertensive patients, prevalence of metabolic
syndrome is increased two-fold in comparison to general
population. Relative increase in the prevalence of metabolic
syndrome in hypertensives is greater in male than in
female patients. 2. There are gender-specific differences in
the prevalence of various components of metabolic syndrome in patients with hypertension. While abdominal obesity
is more prevalent in females, glucose intolerance, high
triglyceride levels and low HDL cholesterol levels are more
frequent in male patients with hypertension
C-reactive protein is not related to ambulatory blood pressure or target organ damage in treated hypertensives
Background Publication of the JUPITER trial has renewed the interest in the use of CRP in cardiovascular risk prediction. The aim of the study was to assess the relationship between CRP, ambulatory blood pressure and target organ damage in a cohort of treated hypertensive males without overt cardiovascular disease.Materials and methods The studied group consisted of 299 male hypertensive patients. Patients were stratified into low (≤ 2 mg/L), intermediate (2–5 mg/L), and high (> 5 mg/L) CRP groups. We measured ambulatory blood pressure, pulse wave velocity, left ventricular function and structure, carotid intima media thickness and ankle-brachial index.Results Twenty-six percent of the patients had CRP in the range of 2 to 5 mg/L, and 12% had CRP levels exceeding 5 mg/L. Ambulatory blood pressure and heart rate were not different across the three groups. Patients with high CRP had lower HDL cholesterol levels and higher plasma fibrinogen levels. Carotid femoral pulse wave velocity, carotid intima media thickness and ankle-brachial index were not different across the three groups. Echocardiographic data were also not related to the CRP level.Conclusions1. Elevated levels of CRP are frequently observed among treated patients with hypertension.2. CRP elevation is associated with higher fibrinogen and glucose levels, and lower HDL cholesterol independently of obesity and smoking status.3. Kontrowersje w postępowaniu z chorymi na nasieniaka jądra w I stopniu zaawansowania klinicznego
Osiemdziesiąt procent nasieniaków sklasyfikowanych jest w I stopniu zaawansowania klinicznego, stanowiąc około40% wszystkich guzów jądra. Postępowanie z tą grupą chorych po pierwotnym leczeniu polegającym na orchiektomiijest przedmiotem kontrowersji. Rozpatrywane są trzy opcje: obserwacja, radioterapia oraz chemioterapia.Aktywna obserwacja z leczeniem wdrażanym w przypadku wznowy, uzupełniające napromienianie lub adiuwantowamonochemioterapia karboplatyną pozwalają na uzyskanie około 100% przeżyć całkowitych i są alternatywnymi,równoważnymi metodami postępowania. Na podstawie aktualnych danych z literatury przedstawiono zalety i wadykażdej z trzech omawianych opcji
The tolerance of proton radiotherapy — preliminary results
Introduction. Because the specific proton beam dose distribution (i.e. the so-called ’Bragg curve’), proton radiotherapy ensures that the high-dose region is precisely confined to the target volume while minimizing the dose delivered to healthy tissues/critical organs surrounding the tumour or to those lying in the path of the proton beam. This method has been used for patients in Kraków since November 2016.
Aim. To report the early tolerance outcomes to proton radiotherapy in patients completing their treatment just before the end of August 2017.
Materials and methods. Study subjects were 47 patients who had completed their treatment before the end of August 2017 with a mean age of 41.6 years (range: 16–76, median: 40). The most frequent diagnoses were skull base tumours (22 pts. — 46.8%) and brain G1 or G2 gliomas (17 pts. — 36.2%), whereas the most frequent histological types were chordomas (17 pts. — 36.2%). Proton radiotherapy was administered by pencil beam scanning and consisted of using the intensity modulated proton therapy (IMPT) technique. The total dose given per cancer type averaged as follows: (i) 70 and 74 Gy(RBE), for respectively chodrosarcomas and chordomas, (ii) 54 Gy(RBE) for brain gliomas and (iii) 70 Gy(RBE) for paranasal sinuses tumours. Early tolerance was prospectively evaluated and measured according to the CTCAE scale, version 4.03.
Results. In all, 91 side effects (SE) were recorded in 44 patients. The intensity of SEs were as following: 62 SEs (68.1%) were of grade 1 intensity, 21 SEs (23.1%) were of grade 2 and 8 SEs (8.8%) were of grade 3. The most frequently developed SEs were skin reactions (29 pts. — 61.7%) or oral/pharyngeal mucositis (20 pts. — 42.6%). Because the patient follow-up period was short, presented results only describes the early tolerance to this therapy. Our findings of mild intensities for the most early side effects, at (grades 1 or 2) are consistent with other published studies

Because the specific proton beam dose distribution (i.e. the so-called ’Bragg curve’), proton radiotherapy ensures that the high-dose region is precisely confined to the target volume while minimizing the dose delivered to healthy tissues/critical organs surrounding the tumour or to those lying in the path of the proton beam. This method has been used for patients in Kraków since November 2016.
Aim. To report the early tolerance outcomes to proton radiotherapy in patients completing their treatment just before the end of August 2017.
Materials and methods. Study subjects were 47 patients who had completed their treatment before the end of August 2017 with a mean age of 41.6 years (range: 16–76, median: 40). The most frequent diagnoses were skull base tumours (22 pts. — 46.8%) and brain G1 or G2 gliomas (17 pts. — 36.2%), whereas the most frequent histological types were chordomas (17 pts. — 36.2%). Proton radiotherapy was administered by pencil beam scanning and consisted of using the intensity modulated proton therapy (IMPT) technique. The total dose given per cancer type averaged as follows: (i) 70 and 74 Gy(RBE), for respectively chodrosarcomas and chordomas, (ii) 54 Gy(RBE) for brain gliomas and (iii) 70 Gy(RBE) for paranasal sinuses tumours. Early tolerance was prospectively evaluated and measured according to the CTCAE scale, version 4.03.
Results. In all, 91 side effects (SE) were recorded in 44 patients. The intensity of SEs were as following: 62 SEs (68.1%) were of grade 1 intensity, 21 SEs (23.1%) were of grade 2 and 8 SEs (8.8%) were of grade 3. The most frequently developed SEs were skin reactions (29 pts. — 61.7%) or oral/pharyngeal mucositis (20 pts. — 42.6%). Because the patient follow-up period was short, presented results only describes the early tolerance to this therapy. Our findings of mild intensities for the most early side effects, at (grades 1 or 2) are consistent with other published studies.
