Functional Activities and Immunohistochemical Cellular Distribution of Glutathione S-Transferases in Normal, Dysplastic, and Squamous Cell Carcinoma Human Oral Tissues
Clinical data show a strong correlation between tobacco and alcohol use and the development of oral squamous cell carcinoma (SCC). While this association implies that the oral mucosa actively metabolizes carcinogens, there is little information which depicts the carcinogen metabolizing enzymes within the oral cavity. Glutathione S-transferases (GSTs) primary function is to detoxify carcinogens by increasing their water solubility, GSTs represent key carcinogen metabolizing enzymes. Notably, individuals with a null phenotype for certain GST isoforms are at an increased risk to develop cancer. This study investigated the function and distribution of GSTs in human oral tissues. Our results from this pilot study showed a trend towards higher GST activities in SCC tissues relative to normal mucosa. Also, relative to normal tissues, the SCC and epithelial dysplasia samples showed a more intense and uniform GST intracellular distribution. GST activities are increased in many high grade cancers. Similarly, our data suggest that GST upregulation occurs in at least a subset of precancerous and malignant oral lesions

Hubble Space Telescope WFPC-2 Imaging of Cassiopeia A
The young SNR Cassiopeia A was imaged with WFPC-2 through four filters
selected to capture the complete velocity range of the remnant's main shell in
several important emission lines. Primary lines detected were [O III]
4959,5007, [N II] 6583, [S II] 6716,6731 + [O II] 7319,7330 + [O I] 6300,6364,
and [S III] 9069,9532. About 3/4th of the remnant's main shell was imaged in
all four filters. Considerable detail is observed in the reverse-shocked ejecta
with typical knot scale lengths of 0.2"-0.4" (1 - 2 x 10^16 cm). Both bright
and faint emission features appear highly clumped. Large differences in [S III]
and [O III] line intensities indicating chemical abundance differences are also
seen, particularly in knots located along the bright northern limb and near the
base of the northeast jet. A line of curved overlapping filament in the
remnant's northwestern rim appears to mark the location of the remnant's
reverse shock front in this region. Finger-like ejecta structures elsewhere
suggest cases where the reverse shock front is encountering the remnant's
clumped ejecta. Narrow-band [N II] images of the remnant's circumstellar knots
("QSFs") reveal them to be 0.1"-0.6" thick knots and filaments, often with
diffuse edges facing away from the center of expansion. Three color composite
images of the whole remnant and certain sections along with individual filter
enlargements of selected regions of the bright optical shell are presented and

Hospitalizations for Cardiovascular Disease in African Americans and Whites with HIV/AIDS
Therapeutic advances have resulted in an epidemiological shift in the predominant causes of hospitalization for patients with HIV/AIDS. An emerging cause for hospitalization in this patient population is cardiovascular disease (CVD); however, data are limited regarding how this shift affects different racial groups. The objective of this observational, retrospective study was to evaluate the association between race and hospitalization for CVD in African Americans and whites with HIV/AIDS and to compare the types of CVD-related hospitalizations between African Americans and whites with HIV/AIDS. Approximately 1.5 million hospital discharges from the US National Hospital Discharge Surveys for the years of 1996 to 2008 were identified. After controlling for potential confounders, the odds of CVD-related hospitalization in patients with HIV/AIDS were 45% higher for African Americans than whites (odds ratio [OR]=1.45, 95% CI, 1.39–1.51). Other covariates that were associated with increased odds of hospitalization for CVD included chronic kidney disease (OR=1.43, 95% CI, 1.36–1.51), age≥50 years (OR=3.22, 95% CI, 2.94–3.54), region in the Southern United States (OR=1.17, 95% CI, 1.11–1.23), and Medicare insurance coverage (OR=1.71, 95% CI, 1.60–1.83). Male sex was not significantly associated with the study outcome (OR=0.99, 95% CI, 0.96–1.02). Compared to whites with HIV/AIDS, African Americans with HIV/AIDS had more hospitalizations for heart failure and hypertension, but fewer hospitalizations for stroke and coronary heart disease. In conclusion, African Americans with HIV/AIDS have increased odds of CVD-related hospitalization as compared to whites with HIV/AIDS. Furthermore, the most common types of CVD-related hospitalizations differ significantly in African Americans and whites. (Population Health Management 2012;16:201–207
Fertility preservation in patients with haematological disorders: a retrospective cohort study
This study investigated the factors associated with utilization of fertility preservation and the differences in treatments and outcomes by prior chemotherapy exposure in patients with haematological diseases. This study included all 67 women with haematological diseases seen for fertility preservation consultation at two university hospitals between 2006 and 2011. Of the total, 49% had lymphoma, 33% had leukaemia, 7% had myelodysplastic syndrome and 4% had aplastic anaemia; 46% had prior chemotherapy; and 33% were planning for bone marrow transplantation, 33% pursued ovarian stimulation and 7% used ovarian tissue banking; and 48% of patients did not pursue fertility preservation treatment. All five cycle cancellations were in the post-chemotherapy group: three patients with leukaemia and two with lymphoma. Patients with prior chemotherapy had lower baseline antral follicle count (10 versus 22) and received more gonadotrophins to achieve similar peak oestradiol concentrations, with no difference in oocyte yield (10.5 versus 10) after adjustment for age. Embryo yield was similar between those who had prior chemotherapy and those who had not. Half of the patients with haematological diseases who present for fertility preservation have been exposed to chemotherapy. While ovarian reserve is likely impaired in this group, oocyte yield may be acceptable
The expansion asymmetry and age of the Cassiopeia A supernova remnant
HST images of the young supernova remnant Cas A are used to explore the expansion and spatial distribution of its highest velocity debris. ACS WFC images taken in 2004 March and December with Sloan F625W, F775W, and F850LP filters were used to identify 1825 high-velocity, outlying ejecta knots through measured proper motions of 0."35 - 0."90 yr(-1), corresponding to V-trans = 5500-14,500 km s(-1) assuming d = 3.4 kpc. The distribution of derived transverse expansion velocities for these ejecta knots shows a striking bipolar asymmetry with the highest velocity knots (V-trans >= 10,500 km s(-1)) confined to nearly opposing northeast and southwest "jets'' at P.A. = 45 degrees-70 degrees and 230 degrees-270 degrees, respectively. The jets have about the same maximum expansion velocity of similar or equal to 14,000 km s(-1) and appear kinematically and chemically distinct in that they are the remnant's only S-rich ejecta with expansion velocities above the 10,000-11,000 km s(-1) exhibited by outer nitrogen-rich ejecta, which otherwise represent the remnant's highest velocity debris. In addition, we find significant gaps in the spatial distribution of outlying ejecta in directions that are approximately perpendicular to the jets (P.A. = 145 degrees-200 degrees and 335 degrees-350 degrees). The remnant's central X-ray point source lies some 700 to the southeast of the estimated expansion center ( P.A. = 169 degrees +/- 8.degrees 4) indicating a projected motion toward the middle of the broad southern ejecta knot gap. Extrapolations of measured 9 month proper motions for all 1825 outer ejecta knots and a selected subsample of 72 bright and compact knots suggest explosion dates (assuming no knot deceleration) of 1662 +/- 27 and 1672 +/- 18, respectively. We find some evidence for nonuniform deceleration in different directions around the remnant and find 126 knots located along the northwestern limb among the least decelerated ejecta, suggesting a convergence date of 1681 +/- 19. A remnant age of around 325 yr would imply a +/- 350 km s(-1) transverse velocity for the central X-ray point source
"Best fit" framework synthesis: refining the method
Background
Following publication of the first worked example of the “best fit” method of evidence synthesis for the systematic review of qualitative evidence in this journal, the originators of the method identified a need to specify more fully some aspects of this particular derivative of framework synthesis.
Methods and Results
We therefore present a second such worked example in which all techniques are defined and explained, and their appropriateness is assessed. Specified features of the method include the development of new techniques to identify theories in a systematic manner; the creation of an a priori framework for the synthesis; and the “testing” of the synthesis. An innovative combination of existing methods of quality assessment, analysis and synthesis is used to complete the process. This second worked example was a qualitative evidence synthesis of employees’ views of workplace smoking cessation interventions, in which the “best fit” method was found to be practical and fit for purpose.
Conclusions
The method is suited to producing context-specific conceptual models for describing or explaining the decision-making and health behaviours of patients and other groups. It offers a pragmatic means of conducting rapid qualitative evidence synthesis and generating programme theories relating to intervention effectiveness, which might be of relevance both to researchers and policy-makers
