    Cet article vise Ă  rĂ©sumer la façon dont les participants aux Jeux de l’Acadie, aux Jeux franco-ontariens et aux Jeux francophones de l’Alberta ont tendance Ă  se reprĂ©senter leur identitĂ© et celle de leur communautĂ©. La synthĂšse des discours, tantĂŽt Ă  prĂ©dominance gĂ©nĂ©alogique, tantĂŽt plutĂŽt linguistique, que reprennent et recomposent les jeunes, permet de dĂ©crire les particularitĂ©s discursives du caractĂšre francophone des jeunes et de chacun des jeux, et de souligner quelques pistes de rĂ©flexion sur les conditions qui influencent la reproduction discursive de l’appartenance francophone chez les jeunes.This article summarizes the ways that participants in the Jeux de l’Acadie, the Jeux franco-ontariens and the Jeux francophones de l’Alberta tend to conceive of their identity and that of their community. Our analysis of the predominantly genealogical or linguistic discourse that the youth reproduce and reconstruct enables us to describe the discursive features of their Francophone character and that of each of the games, and leads to some initial thoughts on the conditions that shape the discursive reproduction of Francophone identity among young people

    La contestation des discours sur l’obĂ©sité : YouTube et la femme « grosse » parrhĂ©siaste

    Dans la sociĂ©tĂ© occidentale, les constructions discursives dominantes dĂ©finissent les « gros » corps comme des corps malades qui manquent de volontĂ© pour gĂ©rer leurs dĂ©sirs. Ces discours nĂ©olibĂ©raux laissent aussi entendre que le corps serait un objet mesurable distinct du corps vĂ©cu. Les femmes « grosses » sont assujetties aux discours nĂ©olibĂ©raux sur l’obĂ©sitĂ©, mais aussi au « savoir » sur la fĂ©minitĂ© (hĂ©tĂ©rosexuelle); elles sont donc jugĂ©es et dĂ©nigrĂ©es puisqu’elles ne se conforment pas aux normes sociales de ces deux espaces discursifs. Toutefois, certaines d’entre elles rĂ©sistent en problĂ©matisant l’identitĂ© fĂ©minine que gĂ©nĂšrent les discours dominants sur l’obĂ©sitĂ©. En effet, dans une recherche procĂ©dant Ă  une analyse foucaldienne des discours de quatre vidĂ©os produites sur YouTube par des femmes qui se disent « grosses » et des commentaires que ces vidĂ©os ont suscitĂ©s, les auteures ont constatĂ© que YouTube offre aux femmes une plateforme pour pratiquer la parrhĂ©sie, ce que Foucault dĂ©crit comme l’acte de dire la vĂ©ritĂ© sans peur. Utiliser YouTube pour parler sans peur de ses difficultĂ©s avec les constructions discursives dominantes du corps fĂ©minin « gros » favorise une forme d’attention Ă  soi qui peut aider les femmes Ă  contester les discours dominants sur l’obĂ©sitĂ©. MĂȘme si elles ne parviennent pas Ă  se libĂ©rer complĂštement de ces discours, elles peuvent, grĂące Ă  l’auto(re)prĂ©sentation parrhĂ©siastique dans YouTube, se voir sous un jour nouveau et contribuer Ă  la production et Ă  la reproduction de contre-discours sur le corps fĂ©minin « gros ».In Western society, dominant discursive constructions of ?fat? bodies position them as diseased bodies that lack the will to manage their own desires. These neo-liberal discourses also suggest that bodies are measurable objects separate from the lived body. ?Fat? women are judged and denigrated since they are subject to and subject of these neo-liberal obesity discourses, as well as the ?knowingness? of (heterosexual) femininity, since they do not conform to social norms in both discursive spaces. However, some of them resist by problematizing the feminine identity that is tied to dominant discourses of obesity. Indeed, results from a pilot foucauldian discourse analysis of four YouTube videos featuring women who self-identify as ?fat? and their associated comments suggest that YouTube is a platform that allows women to practice parrhesia, what Foucault describes as the act of fearlessly speaking the truth. Speaking fearlessly about one’s struggle with dominant discursive constructions of the ?fat? female body on YouTube fosters a kind of attentiveness toward the self which can help women challenge dominant discourses of obesity. Although it was found that women can not completely break away from dominant discourses of obesity, through parrhesiastic self-(re)presentation on YouTube, they can look at themselves in new ways, and contribute to the production and reproduction of counter-discourses about the ?fat? feminine body

    Regard sur la dĂ©lĂ©gation du Nouveau-Brunswick aux Jeux de la francophonie canadienne: un renforcement de l’identitĂ© francophone des jeunes qui mise sur l’excellence

    The FJFNB participated in the deliberations that led to the creation of the Jeux de la francophonie canadienne (JeuxFC) and continues to organize the New Brunswick delegation for this largest gathering of Francophone youths in Canada. This text first provides a look back at the genesis of the JeuxFC. Although the goal of the games was and still is agreed upon, different formulas have been put forward to achieve it. The strategy favoured by the FJFNB, which focuses on excellence, represents one of the poles in this debate, which has led to a new formula for the JeuxFC that ensures their continued success while allowing provincial youth associations to participate with distinct approaches. The purpose here is to document the FJFNB’s choices in these discussions in comparison to other delegations to the games and in the context of the positions that guide other Francophone games. The study of New Brunswick’s strategy in 2008 and 2017, which focuses on performance while promoting sociability, then highlights some variations in the continuity of the principles that guide the delegation towards its objectives. The text then looks at the experience of New Brunswick participants in the 2008 and 2017 JeuxFC. These teenagers clearly took advantage of these two editions to rekindle and express their Francophone pride, to stand out on the sports, arts, and leadership scene, and to interact with young Francophones from across the country.La FJFNB a participĂ© aux dĂ©libĂ©rations qui ont menĂ© Ă  la crĂ©ation des Jeux de la francophonie canadienne (JeuxFC) et elle organise la dĂ©lĂ©gation du Nouveau-Brunswick pour ce plus grand rassemblement de la jeunesse francophone au Canada. Ce texte propose d’abord un retour sur la genĂšse des JeuxFC. Quoique le but des Jeux faisait et fait encore l’unanimitĂ©, diffĂ©rentes formules ont Ă©tĂ© mises de l’avant pour y arriver. La stratĂ©gie privilĂ©giĂ©e par la FJFNB, qui mise sur l’excellence, reprĂ©sente l’un des pĂŽles dans ce dĂ©bat qui a abouti Ă  une formule inĂ©dite pour les Jeux FC assurant leur succĂšs continu tout en permettant aux associations jeunesse provinciales d’y participer selon des approches distinctes. Le but ici est de documenter les choix de la FJFNB dans ces dĂ©libĂ©rations comparativement Ă  d’autres dĂ©lĂ©gations aux JeuxFC et dans le contexte des prises de position qui orientent d’autres jeux francophones. L’étude de la stratĂ©gie nĂ©o-brunswickoise en 2008 et en 2017, axĂ©e sur la performance tout en favorisant la sociabilitĂ©, souligne dans un deuxiĂšme temps quelques variations dans la continuitĂ© des principes qui guident la dĂ©lĂ©gation vers ses objectifs. Le texte aborde, dans un troisiĂšme temps, l’expĂ©rience des participants du Nouveau-Brunswick aux JeuxFC de 2008 et de 2017.     Evaluation of intensive versus standard blood pressure reduction and association with cognitive decline and dementia : a systematic review and metaAnalysis

    Importance: Optimal blood pressure (BP) targets for the prevention of cognitive impairment remain uncertain. Objective: To explore the association of intensive (i.e. lower than usual) BP reduction compared to guidelines on the incidence of cognitive decline and dementia in adults with hypertension. Data Sources and Study Selection: We conducted a systematic review and meta-analysis of randomized controlled trials that evaluated the association of intensive systolic BP lowering on cognitive outcomes by searching MEDLINE, Embase, CENTRAL, Web of Science, CINAHL, PsycINFO, ICTRP and ClinicalTrials.gov for data up to October 27, 2020. Data Extraction and Synthesis: Data screening and extraction were performed independently by two reviewers based on PRISMA guidelines. The risk of bias was assessed using the Cochrane risk-of-bias 2 tool. We used random-effects models using the inverse variance method for our pooled analyses. We evaluated the presence of potential heterogeneity with the I2 index. Main Outcomes and Measures: Our primary outcome was cognitive decline. Secondary outcomes included the incidence of dementia, mild cognitive impairment (MCI), cerebrovascular events, serious adverse events, and all-cause mortality. Results: From 7,755 citations, we identified sixteen publications from five trials (17,396 participants, mean age 65.7 years, 60.5% males) and two additional ongoing trials. All five trials included in quantitative analyses were considered at unclear to high risk of bias. The mean followup duration was 3.3 years (range 2.0 to 4.7 years). Intensive BP reduction was not significantly associated with global cognitive performance (SMD 0.01, 95% CI -0.04 to 0.06, I2 = 0%, four trials, 5,246 patients), incidence of dementia (RR 1.09, 95% CI 0.32 to 3.67, I2 = 27%, two trials, 9,444 patients) or incidence of MCI (RR 0.91, 95% CI 0.73 to 1.14, I2 = 74%, two trials, 10,774 patients) when compared to standard treatment. However, we found a reduction of cerebrovascular events in the intensive arm (RR 0.79, 95% CI 0.67-0.93, I2 = 0%, five trials, 17,396 patients) without an increased risk of serious adverse events or mortality. Conclusions and Relevance: We did not detect a significant association between BP reduction and lower risk of cognitive decline, dementia or MCI. The certainty of this evidence is low due to the limited sample size, the risk of bias of included trials and the observed statistical heterogeneity.     Multicenter phase II trial of brequinar sodium in patients with advanced squamous-cell carcinoma of the head and neck

    A total of 19 patients with advanced squamous-cell carcinoma of the head and neck who had not previously been exposed to chemotherapy were treated with brequinar sodium as first-line chemotherapy. Brequinar was given intravenously at a median weekly dose of 1,200 mg/m 2 . The toxicity was moderate, with 7 patients (37%) experiencing grade 3 or 4 toxicity. In all, 16 patients who were evaluable for efficacy showed no objective response.
