    Exploring Individualsā€™ Loyalty To Online Support Groups From the Perspective Of Social Support

    Prior research indicates that social support allows individuals to have additional skills to handle stresses, enhance self-esteem, and increase psychological well-being. Online social networks have become an emerging and important source of social support. However, little research has been done to explore how IT usage may improve the well-being and life quality of individuals, especially patients. Little research has been done to empirically examine individualsā€™ loyalty toward online social groups from the perspective of social support. This study aims to develop an integrated research model to the impact of online social support on psychological well-being and group identification, and individualsā€™ subsequent loyalty toward online social groups. Online social support group was operationalized as a second-order construct with four components: informational, emotional, esteem, and network supports. Data collected from 537 users of online support groups for mothers in Taiwan provide strong support for the research model. The results indicate that online social support is positively associated with psychological well-being and group identification, which in turn have a positive effect on momsā€™ loyalty toward online social groups. Implications for theory and practice and suggestions for future research are provided

    Age as a predisposing factor of respiratory alkalosis in accidental carbon monoxide poisoning

    AbstractAimsThe purpose of this study was to determine the frequency of and identify the predisposing factors for respiratory alkalosis in patients with accidental carbon monoxide (CO) poisoning.MethodsPatients presenting to the emergency department with accidental CO poisoning were retrospectively identified and divided into Group A (no respiratory alkalosis) and Group B (respiratory alkalosis). Charts were reviewed for neurologic status, various demographic factors, and laboratory data.ResultsA total 96 patients, 37 (38.5%) men and 59 (61.5%) women, were identified. Of these, the 58 (60.4%) patients without respiratory alkalosis were placed in Group A and the 38 (39.6%) patients with respiratory alkalosis were placed in Group B. Independent multivariate predictors of CO poisoning presenting with respiratory alkalosis were age [odds ratio (OR), 1.04; 95% confidence interval (CI), 1.01ā€“1.08] and respiratory rate (OR, 1.16; 95% CI, 1.01ā€“1.33). The rates of respiratory alkalosis in patients younger than 15 years, 15ā€“29 years, 30ā€“44 years, 45ā€“59 years, and older than 59 years were 17.4%, 32.4%, 51.9%, 75%, and 75%, respectively (p<0.01).ConclusionsRespiratory alkalosis in the patients with CO poisoning is not an uncommon finding, and as age increases, the percentage becomes higher. When emergency physicians are faced with patients presenting with respiratory alkalosis of undetermined cause, CO poisoning should be taken into consideration, especially in the elderly

    Bicycling has become a mainstream activity among the environmental aware generation. Bicycling communities have gradually shown interests in quantitative data of the bicycling experiences such as road roughness, inclination, pollution, etc. Bikers utilize these data to infer the possible stamina cost and quality of surroundings. This supports them to make a better decision. This study assumes that fitness level indexed by stamina cost could enhance a biker's sense of control. The prototype in this paper was developed to provide stamina cost information, which is inferred from the terrain patterns of a biking route. In the system evaluation, participants took a positive attitude toward this prototype and approved the importance of stamina cost feedback.     TIME millimeter wave grating spectrometer

    The Tomographic Ionized-carbon Mapping Experiment (TIME) utilizes grating spectrometers to achieve instantaneous wideband coverage with background-limited sensitivity. A unique approach is employed in which curved gratings are used in parallel plate waveguides to focus and diffract broadband light from feed horns toward detector arrays. TIME will measure singly ionized carbon fluctuations from 5 < z < 9 with an imaging spectrometer. 32 independent spectrometers are assembled into two stacks of 16, one per polarization. Each grating has 210 facets and provides a resolving power R of ~ 200 over the 186ā€“324 GHz frequency range. The dispersed light is detected using 2-D arrays of transition edge sensor bolometers. The instrument is housed in a closed-cycle 4Kā€“1Kā€“300mK cryostat. The spectrometers and detectors are cooled using a dual-stage 250/300 mK refrigerator

    Intraprostatic injection of botulinum toxin type- A relieves bladder outlet obstruction in human and induces prostate apoptosis in dogs

    BACKGROUND: With the increasing interest with botulinum toxin ā€“ A (BTX-A) application in the lower urinary tract, we investigated the BTX-A effects on the canine prostate and also in men with bladder outlet obstruction (BOO) due to benign prostatic hyperplasia (BPH). METHODS: Transperineal injection into the prostate using transrectal ultrasound (TRUS) was performed throughout the study. Saline with or without 100 U of BTX-A was injected into mongrel dogs prostate. One or 3 months later, the prostate was harvested for morphologic and apoptotic study. In addition, eight BPH patients refractory to Ī±-blockers were treated with ultrasound guided intraprostatic injection of 200 U of BTX-A. RESULTS: In the BTX-A treated dogs, atrophy and diffuse apoptosis was observed with H&E stain and TUNEL stain at 1 and 3 months. Clinically, the mean prostate volume, symptom score, and quality of life index were significantly reduced by 18.8%, 73.1%, and 61.5% respectively. Maximal flow rate significantly increased by 72.0%. CONCLUSION: Intraprostatic BTX-A injection induces prostate apotosis in dogs and relieves BOO in humans. It is therefore a promising alternative treatment for refractory BOO due to BPH

    Outcome for self-expandable metal stents inĀ patientsĀ with malignant gastroduodenal obstruction: A single center experience

    SummaryBackgroundMalignant gastric outlet obstruction causes significant malnutrition and morbidity. The implantation of a metallic stent is an alternative palliative treatment to allow the intake of food in these patients.Patients and MethodsThirty-eight consecutive patients with malignant gastric outlet obstruction who had received an uncovered metallic stent placement in our department from April 2010 to April 2012 were enrolled for analysis. The mean follow-up time was 6.3 months. Food intake, measured by the Gastric Outlet Obstruction Scoring System, complications, duration of stent patency, and survival were evaluated.ResultsThe technical and clinical success rates of the procedure were 100% and 94.7%, respectively. The Gastric Outlet Obstruction Scoring System scores were significantly improved at 1 day, 7 days, and 30 days after the implantation compared with those prior to the procedure (pĀ <Ā 0.001). Aspiration pneumonia developed in two patients (5.2%) after the procedure. One of these patients developed respiratory failure and died 3 days later. Stent dysfunction developed in 11 of 38 patients (28.9%) during the follow-up period; one patient (2.6%) experienced migration of the stent 38 days later due to resolution of the stricture; 10 patients (26.3%) had stent restenosis. The median time of stent patency was 120 days. The presence of peritoneal carcinomatosis when the procedure was carried out was a significantly poor predictive factor of stent patency [hazard ratio (HR) 7.9, pĀ =Ā 0.039]. The median survival of the patients was 156 days. Poor performance status ā‰„3; HR 2.647, pĀ =Ā 0.012) and nongastric cancer origin (HR 3.466, pĀ =Ā 0.008) were associated with a significantly short survival time.ConclusionMetallic stent placement is an effective and relatively safe treatment for patients with malignant gastric outlet obstruction

    CircNet: a database of circular RNAs derived from transcriptome sequencing data

    Circular RNAs (circRNAs) represent a new type of regulatory noncoding RNA that only recently has been identified and cataloged. Emerging evidence indicates that circRNAs exert a new layer of post-transcriptional regulation of gene expression. In this study, we utilized transcriptome sequencing datasets to systematically identify the expression of circRNAs (including known and newly identified ones by our pipeline) in 464 RNA-seq samples, and then constructed the CircNet database (http://circnet.mbc.nctu.edu.tw/) that provides the following resources: (i) novel circRNAs, (ii) integrated miRNA-target networks, (iii) expression profiles of circRNA isoforms, (iv) genomic annotations of circRNA isoforms (e.g. 282 948 exon positions), and (v) sequences of circRNA isoforms. The CircNet database is to our knowledge the first public database that provides tissue-specific circRNA expression profiles and circRNAā€“miRNA-gene regulatory networks. It not only extends the most up to date catalog of circRNAs but also provides a thorough expression analysis of both previously reported and novel circRNAs. Furthermore, it generates an integrated regulatory network that illustrates the regulation between circRNAs, miRNAs and genes

    Serum Bone Resorption Markers after Parathyroidectomy for Renal Hyperparathyroidism: Correlation Analyses for the Cross-Linked N-telopeptide of Collagen I and Tartrate-Resistant Acid Phosphatase

    Patients on long-term dialysis may develop secondary hyperparathyroidism (SHPT) with increased serum concentrations of bone resorption markers such as the cross-linked N-telopeptide of type I collagen (NTX) and type-5b tartrate-resistant acid phosphatase (TRAP). When SHPT proves refractory to treatment, parathyroidectomy (PTX) may be needed. Renal patients on maintenance HD who received PTX for refractory SHPT (n=23) or who did not develop refractory SHPT (control subjects; n=25) were followed prospectively for 4 weeks. Serum intact parathyroid hormone (iPTH), NTX, TRAP, and bone alkaline phosphatase (BAP) concentrations were measured serially and correlation analyses were performed. iPTH values decreased rapidly and dramatically. BAP values increased progressively with peak increases observed at 2 weeks after surgery. NTX and TRAP values decreased concurrently and progressively through 4 weeks following PTX. A significant correlation between TRAP and NTX values was observed before PTX but not at 4 weeks after PTX. Additionally, the fractional changes in serum TRAP were larger than those in serum NTX at all times examined after PTX. Serum iPTH, TRAP, and NTX values declined rapidly following PTX for SHPT. Serum TRAP values declined to greater degrees than serum NTX values throughout the 4-week period following PTX
