Note On Certain Inequalities for Neuman Means
In this paper, we give the explicit formulas for the Neuman means ,
, and , and present the best possible upper and lower
bounds for theses means in terms of the combinations of harmonic mean ,
arithmetic mean and contraharmonic mean .Comment: 9 page
Thoracic aorta pseudoaneurysm with hemopericardium: unusual presentation of warfarin overdose
There have been few case reports which discuss a relationship between warfarin overdose and aortic pseudoaneurysm leakage. We report the case of a female receiving warfarin who presented with dsypnea. Her international normalized ratio was > 10. Chest radiograph revealed cardiomegaly, and chest computed tomography (CT) showed a bulging pouch-like lesion below the aortic arch greater than 6x6 cm in size and a fluid collection suggesting blood in the pericardium. Thoracic endovascular aneurysm repair (TEVAR) was successfully performed by a cardiovascular surgeon. Aortic pseudoaneurysm formation and leakage may be considered as a rare complication in patients receiving warfarin therapy. Further study regarding warfarin use and the incidence of pseudoaneurysm leakage is needed
Retrospective study of necrotizing fasciitis and characterization of its associated Methicillin-resistant Staphylococcus aureus in Taiwan
<p>Abstract</p> <p>Background</p> <p>Methicillin-resistant <it>Staphylococcus aureus </it>(MRSA) has emerged as a prevalent pathogen of necrotizing fasciitis (NF) in Taiwan. A four-year NF cases and clinical and genetic differences between hospital acquired (HA)- and community-acquired (CA)-MRSA infection and isolates were investigated.</p> <p>Methods</p> <p>A retrospective study of 247 NF cases in 2004-2008 and antimicrobial susceptibilities, staphylococcal chromosomal cassette <it>mec </it>(SCC<it>mec</it>) types, pulsed field gel electrophoresis (PFGE) patterns, virulence factors, and multilocus sequence typing (MLST) of 16 NF-associated MRSA in 2008 were also evaluated.</p> <p>Results</p> <p>In 247 cases, 42 microbial species were identified. <it>S. aureus </it>was the major prevalent pathogen and MRSA accounted for 19.8% of NF cases. Most patients had many coexisting medical conditions, including diabetes mellitus, followed by hypertension, chronic azotemia and chronic hepatic disease in order of decreasing prevalence. Patients with MRSA infection tended to have more severe clinical outcomes in terms of amputation rate (p < 0.05) and reconstruction rate (p = 0.001) than those with methicillin-sensitive <it>S. aureus </it>or non-<it>S. aureus </it>infection. NF patients infected by HA-MRSA had a significantly higher amputation rate, comorbidity, C-reactive protein level, and involvement of lower extremity than those infected by CA-MRSA. In addition to over 90% of MRSA resistant to erythromycin and clindamycin, HA-MRSA was more resistant than CA-MRSA to trimethoprim-sulfamethoxazole (45.8% <it>vs</it>. 4%). ST59/pulsotype C/SCC<it>mec </it>IV and ST239/pulsotype A/SCC<it>mec </it>III isolates were the most prevalent CA- and HA-MRSA, respectively in 16 isolates obtained in 2008. In contrast to the gene for γ-hemolysin found in all MRSA, the gene for Panton-Valentine leukocidin was only identified in ST59 MRSA isolates. Other three virulence factors TSST-1, ETA, and ETB were occasionally identified in MRSA isolates tested.</p> <p>Conclusion</p> <p>NF patients with MRSA infection, especially HA-MRSA infection, had more severe clinical outcomes than those infected by other microbial. The prevalent NF-associated MRSA clones in Taiwan differed distinctly from the most predominant NF-associated USA300 CA-MRSA clone in the USA. Initial empiric antimicrobials with a broad coverage for MRSA should be considered in the treatment of NF patients in an endemic area.</p

Recurrent stress cardiomyopathy with different electrocardiographic abnormalities on each presentation in a depressed post-menopausal woman
AbstractStress cardiomyopathy (SCM) is a syndrome of transient cardiac abnormalities precipitated by intense emotional or physical stress. Differentiating SCM from acute myocardial infarction is often difficult but vital to avoid subjecting SCM patients to unnecessary reperfusion therapy and invasive coronary angiography. For accurate diagnosis, it is important that physicians be familiar with the current diagnostic criteria, most susceptible populations, and typical triggers for SCM. SCM occurs almost exclusively in post-menopausal women, a group with a high frequency of psychiatric disorders. Thus, in addition to typical trigger events, comorbid psychiatric disorders may contribute to SCM onset. We report a rare case of recurrent SCM with distinct electrocardiographic abnormalities during each presentation in a post-menopausal woman with depression.<Learning objective: Recurrence of SCM was thought to be infrequent before but it has been increasingly reported recently. Although the underlying mechanism remains unclear, comorbid psychiatric disorders may contribute significantly to the pathogenesis of SCM and its influence may have been underestimated. Routine mental health screening should be done accompanying therapy for SCM.

The Economic Impact on Leisure Activities
This study explores the influence of the economic recession on leisure activities because economic impact is a significant factor that can change people’s travel decisions. A methodology, Ridit analysis, is applied in this study, and it adopts cumulative probability scores instead of ordinal scores. Results show that current economic crisis has had an impact on people’s ability to take time away from work
GW190814: Circumstantial Evidence for Up-Down Quark Star
Within a confining quark matter model which considers phenomenologically the
quark confinement and asymptotic freedom as well as the chiral symmetry
restoration and quark deconfinement at high baryon density, we find that if the
up-down quark matter (QM) is more stable than nuclear matter and strange
quark matter (SQM), the maximum mass of static quark stars with QM is
under agreement with both the constraints on star tidal
deformability from gravitational wave signal GW170817 and the mass-radius of
PSR J0030+045 measured simultaneously by NICER. In contrast, the conventional
strange quark star with SQM has a maximum static mass of only
and its radius significantly deviates from NICER's constraint. Our results thus
provide circumstantial evidence suggesting the recently reported GW190814's
secondary component with a mass of could be an
up-down quark star.Comment: 5 pages, 2 figure
