    Factors Affecting the Extrusion Rate of Ventilation Tubes

    ObjectivesThe objective of this study was to determine the various factors that affect the extrusion rate of ventilation tubes (VTs), including the nature of the middle ear effusion.MethodsA retrospective chart review of 82 pediatric patients (177 ears) who received VT insertion surgery under general anesthesia was carried out to evaluate the relationship between various factors and the VT extrusion rate. The factors we analyzed included age, gender, the adenoid size, the amount and content of the middle ear effusion after myringotomy, bleeding events, associated adenoidectomy and the findings of the tympanic membrane status, the tympanometry and the audiometry of the air bone gap.ResultsThe mean extrusion time was 254 days (range, 11 to 809 days). The patients with no history of previous VT insertion had a longer extrusion time (mean, 279 days) than did the patients who had undergone previous VT insertion (mean, 203 days). The patients with serous effusion had the shortest extrusion time (mean, 190 days) as compared to those patients with glue (273 days) and pus (295 days) effusions. Other factors had no statistical significant relationship with the extrusion time.ConclusionThe mean VT extrusion time was 254 days. The VT extrusion time was significantly related to the characteristics of the middle ear effusion and a history of previous VT insertion. Thus, the nature of middle ear effusion can provide a clinical clue to predict the VT extrusion time

    Effect of repetitive lysine–tryptophan motifs on the bactericidal activity of antimicrobial peptides

    Previous studies identified lysine- and tryptophan-rich sequences within various cationic antimicrobial peptides. In the present study, we synthesized a series of peptides composed of lysine (K)-tryptophan (W) repeats (KW)(n) (where n equals 2, 3, 4 or 5) with amidation of the C-terminal to increase cationicity. We found that increases in chain length up to (KW)(4) enhanced the peptides’ antibacterial activity; (KW)(5) exhibited somewhat less bactericidal activity than (KW)(4). Cytotoxicity, measured as lysis of human red blood cells, also increased with increasing chain length. With (KW)(5), reduced antibacterial activity and increased cytotoxicity correlated with greater hydrophobicity and self-aggregation in the aqueous environment. The peptides acted by inducing rapid collapse of the bacterial transmembrane potential and induction of membrane permeability. The mode of interaction of the peptides and the phosphate groups of lipopolysaccharide was dependent upon the peptides’ ability to permeate the membrane. Longer peptides [(KW)(4) and (KW)(5)] but not shorter peptides [(KW)(2) and (KW)(3)] strongly bound and partially inserted into negatively charged, anionic lipid bilayers. These longer peptides also induced membrane permeabilization and aggregation of lipid vesicles. The peptides had a disordered structure in aqueous solution, and only (KW)(4) and (KW)(5) displayed a folded conformation on lipid membranes. Moreover, (KW)(4) destroyed and agglutinated bacterial cells, demonstrating its potential as an antimicrobial agent. Collectively, the results show (KW)(4) to be the most efficacious peptide in the (KW)(n) series, exhibiting strong antibacterial activity with little cytotoxicity.     Minimal Width for Universal Property of Deep RNN

    A recurrent neural network (RNN) is a widely used deep-learning network for dealing with sequential data. Imitating a dynamical system, an infinite-width RNN can approximate any open dynamical system in a compact domain. In general, deep networks with bounded widths are more effective than wide networks in practice; however, the universal approximation theorem for deep narrow structures has yet to be extensively studied. In this study, we prove the universality of deep narrow RNNs and show that the upper bound of the minimum width for universality can be independent of the length of the data. Specifically, we show that a deep RNN with ReLU activation can approximate any continuous function or LpL^p function with the widths dx+dy+2d_x+d_y+2 and max{dx+1,dy}\max\{d_x+1,d_y\}, respectively, where the target function maps a finite sequence of vectors in Rdx\mathbb{R}^{d_x} to a finite sequence of vectors in Rdy\mathbb{R}^{d_y}. We also compute the additional width required if the activation function is tanh\tanh or more. In addition, we prove the universality of other recurrent networks, such as bidirectional RNNs. Bridging a multi-layer perceptron and an RNN, our theory and proof technique can be an initial step toward further research on deep RNNs

    Possible link between Arctic Sea ice and January PM10 concentrations in South Korea

    In this study, we investigated the possible teleconnection between PM10 concentrations in South Korea and Arctic Sea ice concentrations at inter-annual time scales using observed PM10 data from South Korea, NCEP R2 data, and NOAA Sea Ice Concentration (SIC) data from 2001 to 2018. From the empirical orthogonal function (EOF) analysis, we found that the first mode (TC1) was a large-scale mode for PM10 in South Korea and explained about 27.4% of the total variability. Interestingly, the TC1 is more dominantly influenced by the horizontal ventilation effect than the vertical atmospheric stability effect. The pollution potential index (PPI), which is defined by the weighted average of the two ventilation effects, is highly correlated with the TC1 of PM10 at a correlation coefficient of 0.75, indicating that the PPI is a good measure for PM10 in South Korea at inter-annual time scales. Regression maps show that the decrease of SIC over the Barents Sea is significantly correlated with weakening of high pressure over the Ural mountain range region, the anomalous high pressure at 500 hPa over the Korean peninsula, and the weakening of the Siberian High and Aleutian low. Moreover, these patterns are similar to the correlation pattern with the PPI, suggesting that the variability of SIC over the Barents Sea may play an important role in modulating the variability of PM10 in South Korea through teleconnection from the Barents Sea to the Korean peninsula via Eurasia
