Variance reduction and signal-to-noise ratio: Reducing uncertainty in spectral ratios
This paper uses an unusually large dataset to study scatter in site-effect estimation, focusing on how the events that increase uncertainty can be removed from the dataset. Four hundred seventy-three weak motion earthquake records from the surface and bedrock of a 178-m-deep borehole in Aegion, Gulf of Corinth, Greece, are used to evaluate spectral ratios. A simple statistical tool, variance reduction (VR), is first used to identify two groups of events that lie closest and farthest from the average, which is considered here as the initial best estimate of the site response. The scatter in the original dataset is found to be due to the group of events with smallest VR. These events can be removed from the dataset in order to compute a more reliable site response. However, VR is not normally used to choose records for site-effect studies, and it cannot be applied to the usual small datasets available. The signal-to-noise ratio (SNR) is normally used to this end, for which reason we investigate whether SNR can be used to achieve similar results as VR. Signal-to-noise ratio is estimated using different definitions. Data selection based on SNR is then compared to that using VR in order to define an SNR-based criterion that discriminates against events that, according to VR, increase scatter. We find that defining the SNR of a surface record as the mean value over a frequency range around the resonant peak (here, 0.5–1.5 Hz) and using a cutoff value of 5 may be used in this case to exclude most events for which VR is small. This process is also applied to the downhole station, where we obtain similar results for a cutoff value of 3
Estructura del subsuelo de la Sabana de Bogotá. Análisis de dispersión de ondas superficiales a partir de sismos y ruido sísmicos
We present the results of the analysis of vertical component from earthquake and seismic noise records obtained in the Sabana de Bogotá to determine subsoil structure in that region. Rayleigh waves present in the earthquake data were processed to estimate to estimate group velocities. In the case of seismic noise, we used seismic interferometry to estimate Green’s functions between stations from average cross-correlation. Rayleigh wave pulses were recovered between stations from seismic noise. We processed the results to estimate group velocities in all cases. The results show the impact of geology on group velocities in the studied region. Different station pairs show varying group velocities depending on the age of the rocks along the paths. Our estimated impedance contrast between surficial deposits and underlying bedrock suggest possible amplification of ground motion larger than a factor of 10. The poor quality and the limited quantity of our data limited the comprehensiveness of our results. The frequency ranges of the results showed poor overlap, hindering comparisons among paths. In spite of their limitations, our results highlight the importance of site effects in the Sabana de Bogotá and the need to take them into account to estimate ground motion for future events in that region

Atomic surface segregation and structural characterization of PdPt bimetallic nanoparticles
"Bimetallic nanoparticles are of interest since they lead to many interesting electrical, chemical, catalytic, and optical properties. They are particularly important in the field of catalysis since they show superior catalytic properties than their monometallic counterparts. The structures of bimetallic nanoparticles depend mainly on the synthesis conditions and the miscibility of the two components. In this work, PdPt alloyed-bimetallic nanoparticles (NPs) were synthesized through the polyol method, and characterized using spherical aberration (Cs) corrected scanning transmission electron microscopy (STEM). High-angle annular dark-field (HAADF)-STEM images of bimetallic nanoparticles were obtained. The contrast of images shows that nanoparticles have an alloy structure with an average size of 8.2 nm. Together with the characterization of nanoparticles, a systematic molecular dynamics simulations study focused on the structural stability and atomic surface segregation trends in 923-atom PdPt alloyed-bimetallic NPs was carried out.
Long‐term safety, efficacy, and quality of life in patients with juvenile idiopathic arthritis treated with intravenous abatacept for up to seven years
ClinicalTrials.gov identifier: NCT00095173[Abstract] Objective. The efficacy and safety of abatacept in patients with juvenile idiopathic arthritis (JIA) who experienced an inadequate response to disease‐modifying antirheumatic drugs were previously established in a phase III study that included a 4‐month open‐label lead‐in period, a 6‐month double‐blind withdrawal period, and a long‐term extension (LTE) phase. The aim of this study was to present the safety, efficacy, and patient‐reported outcomes of abatacept treatment (10 mg/kg every 4 weeks) during the LTE phase, for up to 7 years of followup.
Methods. Patients enrolled in the phase III trial could enter the open‐label LTE phase if they had not achieved a response to treatment at month 4 or if they had received abatacept or placebo during the double‐blind period.
Results. One hundred fifty‐three (80.5%) of 190 patients entered the LTE phase, and 69 patients (36.3%) completed it. The overall incidence rate (events per 100 patient‐years) of adverse events decreased during the LTE phase (433.61 events during the short‐term phase [combined lead‐in and double‐blind periods] versus 132.39 events during the LTE phase). Similar results were observed for serious adverse events (6.82 versus 5.60), serious infections (1.13 versus 1.72), malignancies (1.12 versus 0), and autoimmune events (2.26 versus 1.18). American College of Rheumatology (ACR) Pediatric 30 (Pedi 30) responses, Pedi 70 responses, and clinically inactive disease status were maintained throughout the LTE phase in patients who continued to receive therapy. Improvements in the Child Health Questionnaire physical and psychosocial summary scores were maintained over time.
Conclusion. Long‐term abatacept treatment for up to 7 years was associated with consistent safety, sustained efficacy, and quality‐of‐life benefits in patients with JIA
Systolic blood pressure and the risk of kidney replacement therapy and mortality in patients with chronic kidney disease stage 4-5
Introduction
In patients with chronic kidney disease stage 4 and 5 (CKD stages 4-5) without dialysis and arterial hypertension, it is unknown if the values of systolic blood pressure (SBP) considered in control <120 mmHg are associated with kidney replacement therapy (KRT) and mortality.
Methods
In this retrospective cohort study, hypertensive CKD stages 4-5 patients attending the Renal Health Clinic at the Hospital Civil de Guadalajara were enrolled. We divided them into those that achieved SBP 120 mmHg), the uncontrolled group. Our primary objective was to analyze the association between the controlled group and KRT; the secondary objective was the mortality risk, and if there were subgroups of patients that achieved more benefit. Data were analyzed using Stata software, version. 15.1.
Results
During 2017 to 2022 a total 275 hypertensive CKD stages 4-5 patients met the inclusion criteria for the analysis, 62 in the controlled group and 213 in the uncontrolled group; mean age 61 years, 49.82% were male, SBP was significantly lower in the controlled group (111 mmHg) compared to the uncontrolled group (140 mmHg), eGFR was similar between groups (20.41 ml/min/1.73m2). There was a tendency to increase the mortality risk in the uncontrolled group (HR 6.47 [0.78-53.27]; p= 0.082) and an association by the Kaplan-Meir analysis (Log-rank p= 0.043). The subgroup analysis for risk of KRT in the controlled group revealed that patients ≥ 61 years had a lower risk of KRT (HR 0.87 [95% CI, 0-76-0.99]; p=0.03, p of interaction = 0.005), but no differences were found in the subgroup analysis for mortality. In a follow-up of 1.34 years, no association was found in the risk of KRT according to the controlled or uncontrolled groups in a multivariate Cox analysis.
Conclusion
In a retrospective cohort of patients with CKD stages 4-5 and hypertension, SBP >120 mmHg was not associated with risk of KRT but could be associated with the risk of death. Clinical trials are required in this group of patients to demonstrate the impact of reaching the SBP goals recommended by the KDIGO guidelines
A study of the dry forest communities in the Dominican Republic
This paper is a floristic and phytosociological study of the dry forest communities of the Dominican Republic. A total of 69 relevés in dry forest biotopes were carried out. The samples were subsequently subjected to Detrended Correspondence Analysis for the determination and study of possible groupings. The study does not cover tree formations growing on serpentines, nor the so-called semideciduous forests, peculiar to areas with higher rainfall. A total of nine phytocoenoses were identified. The most significant results led to the description of six new phytosociological associations: Simaroubetum berteroani (thorny dry forest on coastal dunes), Phyllostylo rhamnoidis-Prosopidetum juliflorae (southern Dominican disturbed dry forest), Consoleo moniliformis-Camerarietum linearifoliae (dry forest on hard limestones), Lemaireocereo hystricis-Prosopidetum juliflorae (northern Dominican disturbed dry forest), Lycio americani-Prosopidetum juliflorae (disturbed dry forest on saline soils) and Guettardo ellipticae-Guapiretum discoloris (dry forest on flat-topped hillocks in Montecristi). This is an important step forward in the phytosociological and floristic studies of the Caribbean territories. This research paper was possible thanks to the sponsorship of the Agencia Española de Cooperación Internacional para el Desarrollo (AECID), under the auspices of the Ministerio de Asuntos Exteriores y de Cooperación de España, which funded the project (cod. Impact of treatment and clinical characteristics on the survival of children with medulloblastoma in Mexico
IntroductionData on medulloblastoma outcomes and experiences in low- and middle-income countries, especially in Latin America, is limited. This study examines challenges in Mexico’s healthcare system, focusing on assessing outcomes for children with medulloblastoma in a tertiary care setting.MethodsA retrospective analysis was conducted, involving 284 patients treated at 21 pediatric oncology centers in Mexico.ResultsHigh-risk patients exhibited markedly lower event-free survival than standard-risk patients (43.5% vs. 78.3%, p<0.001). Influential factors on survival included anaplastic subtype (HR 2.4, p=0.003), metastatic disease (HR 1.9, p=0.001); residual tumor >1.5cm², and lower radiotherapy doses significantly impacted event-free survival (EFS) and overall survival (OS). Platinum-based chemotherapy showed better results compared to the ICE protocol in terms of OS and EFS, which was associated with higher toxicity. Patients under 3 years old displayed notably lower OS and EFS compared to older children (36.1% vs. 55.9%, p=0.01)
